Closes #338. Replay via IClaimOnceStore (#543, Redis + in-process fallback), logout revocation via a durable ApplicationUser.LoggedOutAt column. Registry is async + scoped; consume-before-read ordering replaces the single in-memory lock. Multi-replica, fail-closed, and at-or-before boundary tests added; #308 single-instance behavior unchanged. One migration (#407), schema docs regenerated (#417).

P2 Badge Use a shared clock for cross-replica logout ordering

When a grant is issued on a replica whose clock is ahead and a later logout is handled by a replica whose clock trails it, the pre-logout grant carries an issuedAt later than the persisted LoggedOutAt, so this comparison admits it after logout. Because both timestamps come from each process's TimeProvider, moving the marker to Postgres does not make logout revocation multi-replica-safe as claimed; issuance and logout need a shared ordering source rather than independently sampled host clocks.

Reworked to fix the review's clock-skew finding: logout revocation now uses a per-user integer epoch (ApplicationUser.StepUpLogoutEpoch) compared for equality instead of comparing two replicas' wall clocks — skew-immune and genuinely multi-replica-safe. Added a deterministic consume-before-read ordering test. Force-pushed.

When a same-user UserManager write loads the row before logout and commits afterward—for example, login's AccessFailedAsync or ResetAccessFailedCountAsync—Identity's full-entity update still matches because this statement leaves ConcurrencyStamp unchanged, then writes its stale StepUpLogoutEpoch back. That rollback makes a captured pre-logout grant equal the stored epoch again and usable after logout. Advance the concurrency stamp in the same statement so an in-flight stale Identity write loses its CAS rather than undoing revocation.

Fixed in 22a1f4dd. RecordLogoutAsync now rotates ConcurrencyStamp in the same ExecuteUpdate as the epoch bump (the repo's #492 FENCE pattern), so a concurrent full-entity UserStore.UpdateAsync loaded before the logout loses its CAS (0 rows) instead of writing the stale epoch back. Guarded by RecordLogout_FencesAStaleFullEntityWrite_SoTheEpochCannotBeReverted — mutation-verified: deleting the stamp-rotation line turns the test red at the CAS assertion.

Fixed the P1 (epoch revert via stale Identity write): RecordLogoutAsync now rotates ConcurrencyStamp in the same ExecuteUpdate (the #492 fence pattern), so a concurrent full-entity UserManager write can no longer write a stale epoch back. Added RecordLogout_FencesAStaleFullEntityWrite_SoTheEpochCannotBeReverted (mutation-verified). Also corrected the stale 'no schema change' comment and the retry-note wording. Force-pushed.

Round-3 fix: ChangeOwnPasswordAsync now maps a lost ConcurrencyStamp CAS to Users.Conflict (via IsConcurrencyFailure + ResetConcurrencyConflict), matching every sibling write path, instead of mislabeling a benign concurrent-writer race (e.g. a simultaneous logout, which since #338 rotates the stamp) as Users.PasswordRejected. Fail-closed either way; this corrects only the error label. No deterministic test added: the CAS-loss window is internal to the method (fresh reload before save) with no clean seam to force it; the mapping mirrors six already-tested sibling paths. Force-pushed.
