This PR implements RFC #13: Extend Transport with address (tracked by #511)

RFC is still open and not active yet. This PR provides an implementation for the proposal in the RFC.

YAY! 👍 I think it just makes this more clean than it was before... we kind of had this hacked in at each transport instead of the core interface.
