What: Source agents and clients can read and answer eligible remote descendant questions and approvals through the original environment connection.
Why: Pending tools used local ancestry and source clients had qualified task views without a complete native request action path.
So what: Parent reviews this internal #369 component with both required independent MEDIUM reviewers, then integrates and performs actual client, provider and machine acceptance for #379.

Canonical full author handoff: #369 final candidate, proof, failure responses and explicit clean/stopped transfer.

Implements the approved #369 outcome under frozen #379 v5 approval. This component targets feat/379-cross-machine-parity; it does not close #369 or claim final #379 completion.

Qualified requests retain destination environment, thread, status and a revision covering request identity and question/approval content. A bounded walk verifies real local subagent edges and authenticated delegation pairs, including A→B→C and later work on an earlier environment. Native agents retain local ancestor policy for ordinary approvals. Self, unrelated, missing-caller and unproved outside OAuth responses are denied. Authenticated source-client ingress derives its actor from the real current session, with separate read/operate scopes; an MCP argument cannot create a human actor.

Responses enter the existing serialized native command, event/receipt/outbox transaction and effect worker. Stable caller keys reach the native command; retries recover its original acceptance. Revision/content and pending-state guards inside the transaction prevent intake resolution from being overwritten. Concurrent answer/dismiss, changed answers, stale shapes and lost replies preserve one accepted message/provider effect. Async questions remain nonblocking and provider callbacks that cannot resume are shown honestly.

Web and desktop share the source task request cards and banner. Mobile uses the same source RPC with its own viewer/card and floating attention. Compact source shells feed existing notifications and local ancestor attention. No separately saved destination client connection, foreign local child ID, borrowed bearer/provider credential, generic reverse RPC or response cache is introduced.

The requested existing device fixture repair is included solely in PeerAuthority.workflow.test.ts: real DeviceService construction, real config/filesystem/path, frozen pin entries/sentinels and deterministic external host/process/HTTP leaves. Named #383 device-domain paths are untouched. Later #383 typed tests and corrections are separate finite scratch artifacts on #369, absent from this candidate. The two new #383 shared-owner proposals are also excluded and remain separately sequenced by parent.

Validation is scoped feedback, not the full CI/merge gate:

  • Final repaired source at f450b19826e362b58a7af4c5f085e4d31428a2c4: 100 tests pass across six files, including all eight reproduced CI failures, old-shape/source-only navigation and dismissal presentation. Server/web/client-runtime/shared types and scoped lint pass.
  • The reported web6/WireProjection1 failures were stale fixtures omitting fields already required by incoming contracts. Fixtures now use the real schema/type; no production missing-data fallback was added. The catalog1 failure was repaired with a real dismiss definition, summary and write classification. Original CI logs and local 8-failure reproduction are retained.
  • c3d93a9 removed the unnecessary PendingRequestListTool export; scoped server Knip, lint and types pass. Original515f lint failure is retained.
  • Earlier identity-bound proof:515f registered workflow92 and focused26-file344;1ddd authority/contracts123; earlier MCP HTTP/toolkit29. These are not relabeled as new full f450 runs.
  • Genuine failing-before evidence retained for missing callers, async multi-select, intake-after-planning resolution, unrelated source-client reads and stale content revisions.
  • Current42-feature inventory and diff whitespace pass. No repo-wide gate was run locally.

Current stable HEAD is 42051c47813c354682b80677009b199d86fef1a0, tree f9efef2d7df1b8b91ac64b09018386e79ccc1d45, unchanged base ab1bf53640e2a6541a2ba9f316839cedf81d4f46. Same author repaired accepted R1-S1/S2 after genuine registered/native and card reproductions: request content and revision now share one native local SQL read snapshot, with network ancestry authorization outside; explicitly offered empty answers remain distinct from absent answers and cancellation. Native validation, both cards and actual runtime projection export are fixed at their owners. The no-injection runtime export test was red before repair. Current proof: full native workflow94 pass/324.29s, focused120/6files, Pi/card5, scoped server/web/mobile types, lint/format, server Knip and real fork/inventory checks pass. No old proof is relabeled.

The first full repair run had93pass/1fail native initialization404 in the existing joint-created/link-retirement case. Its original body is unavailable and cause UNKNOWN; the later94pass does not diagnose it. Full logs/limits and every prior CI repair response are preserved in the canonical handoff.

R1 on immutable4cf9: independent Sol REQUEST_CHANGES (two reproduced P2 findings), independent Grok APPROVE (no additional finding); parent aggregate REQUEST_CHANGES. Both complete unchanged reports and full parent adjudication are embedded in the updated canonical handoff. Neither approval nor old successful4cf9 CI transfers. Parent owns BOTH NEW independent MEDIUM whole-candidate reviews, new exact-head CI, integration and actual acceptance. This writer is clean, pushed and STOPPED at42051c; no base/topology or383 change.

Cost is UNKNOWN. Supported Observer sync succeeded, but task lookup returned unknown task and publish returned nothing to publish: no sessions in scope; no cost comment or manual estimate was fabricated.

Upstream recheck found no merged remote pending-request/ancestry equivalent on current pingdotgg/t3code main. Open #20240 has the matching native async multi-select fix; only its relevant Orchestrator answer formatting/option validation was adapted after a local reproduction. Its restart conversion policy was not imported. #20037 is adjacent local approval wait metadata, without this remote response policy.

Parent owns fresh independent Codex/gpt-6.1-sol/MEDIUM and installed Grok/grok-4.7/MEDIUM reviews, current CI and actual product verification. No live server/browser/device/provider operation was run by this worker. The native tests use real registered HTTP/auth/control/WS/SQLite/orchestrator/intake/effect owners with deterministic external leaves. The mobile component test reuses the installed web renderer with native view leaves, not a real native client. Actual UI screenshots, physical cross-machine/mobile/all-provider acceptance and full production CLI composition are parent-owned outstanding proof. No version bumps, merges, deployment or shipment.

Requirements and who asked: User #379/#369 requires complete eligible remote question/approval lifecycle and source-only client parity under frozen v5; parent also assigned the bounded shared device fixture repair.
Deleted: Missing-caller bypass, unproved actor claims, fake local shells, blanket human-only approval ban, credential borrowing, response cache, speculative #383/#384 hooks and historical approval transfer.
Bottleneck: One native pending policy/ancestry/revision/acceptance owner with complete source attention/action consumers, followed by both exact reviews and parent actual acceptance.
Checked myself: Actual source callers/owners and live binding/selection, original red controls, native registered disk/HTTP/WS/effect tests, source consumer interactions, current diff and incoming #380/#381/#382 preservation. Remaining independent review/CI/live proof is explicitly pending.

Authored with Codex, gpt-6.1-sol, medium reasoning effort; no fallback or additional delegates.

What: The parent integration stack is repaired and published without merge commits, with exactly unchanged source bytes.
Why: Parent introduced merge11cab while aligning main; the repository’s fork check correctly rejects it anywhere in the inherited topic stack.
So what: Parent waits for #369’s clean stopped handoff, then cascades its own commits onto the repaired base and requires new exact-head CI and both reviews.

Original failure: PR416 head515f3497d026d23eadddc4149d9f66a31352e16a, Fork39113304201, merge commits in the fork stack (rebase, do not merge): 11cab3c029 wip: align #379 integration with current main, retained /tmp/chromeria-369-fork-ci-failure.log. This is parent topology error, not unknown CI or a worker defect. Gate remains unchanged.

Parent fetched actual fork main, still dfb9235811e8d9b76f4713c383301e1a220364ce, verified clean exclusive integration, retained local AND pushed branch rollback/379-pre-linear-repair-11cab, then rebased its three component commits onto origin/main. New integration ab1bf53640e2a6541a2ba9f316839cedf81d4f46, tree 1928ccd2599a9d340f3cde6b49eab1b0c1943529, EXACTLY matches original11cab tree. Full range-diff shows all three patches unchanged: ac4ce6b=b6df8ad784, c8423b4=4bb5fc46fe,3214509afd=ab1bf53640. No component code or current-main ancestry lost. bash scripts/fork-check.sh --upstream upstream PASS,42features, /tmp/chromeria-379-linear-repair-fork-check.log.

Published ONLY feature integration with --force-with-lease=refs/heads/feat/379-cross-machine-parity:11cab3c029aaf26b06cb263609860955e1f87a85; actual remote newheadab1bf536, rollback11cab and main dfb verified. No force main, bypass, version bump, stale canonical checkout or worker mutation. Existing component proof retains original identities and tree-equivalence; full final candidate review/CI remains required. PR416 linked through app. No component readiness claimed from parent fork pass alone.

#369 will finish exact515f proof/canonical handoff then stop clean. Parent alone will preserve its rollback ref and rebase only its own commits --onto ab1bf536 11cab, check unchanged full tree and diff, publish exact lease, update current-base evidence, then dispatch BOTH new MEDIUM reviews and current CI. #383 active dirty workspace remains untouched; its eventual owner handoff/checkpoint precedes any topology change. Other worktrees/evidence/resources retained by explicit #379 mandate.

Requirements and who asked: Existing fork policy and #379 require preserved upstream ancestry, current-base acceptance and no bypassed checks.
Deleted: Parent merge commit and any suggestion that CI should accept a merge-containing fork stack.
Bottleneck: Cascade #369 after clean handoff, then exact-candidate CI/reviews; dirty #383 stays under its current writer.
Checked myself: Actual failed CI log, fork scripts/runbook, live main/feature refs, clean parent workspace, exact old/new tree identity, full unchanged range-diff, fork pass and actual published refs.

What: PR416’s lint failure is a concrete unused export, and its fork failure/conflict remain on the old topology.
Why: Knip reports PendingRequestListTool exported but used only within its declaring module; old515f still inherits merge11cab after parent published the repaired base.
So what: The current #369 author fixes the unused export before its clean stopped handoff, then parent cascades that exact candidate onto the repaired base for new CI and both reviews.

Parent obtained completed job logs through gh api repos/toolboxmd/chromeria/actions/jobs/117633898121/logs; aggregate39113304165 was still running, so gh run view --job --log could not return them. Retained /tmp/chromeria-369-ci-lint-515f.log. Decisive failure: Unused exports (1) PendingRequestListTool Tools apps/server/src/mcp/toolkits/thread/tools.ts:157:14. Parent checked source references and sent same-job 379-369-ci-unused-export-fix-v1 for the smallest owner fix and scoped verification. No Knip/gate exception. Job cleanup’s submodule warning is not the lint cause.

The existing Fork39113304201 merge error and PR conflict concern515f on11cab, not the parent repaired ab1bf53 tree. Parent has not modified the active worker workspace or claimed cascade complete. Worker source repair remains authorized; only self-rebase/base changes stay held. New exact head proof/CI/reviews follow clean handoff and parent-only cascade. PR416 remains native-watched; no poller or repeated CI retry.

Requirements and who asked: Required current CI and clean single-writer topology under #379.
Deleted: Unused public export and treating known topology failure as an unexplained CI problem.
Bottleneck: Worker lint repair and clean handoff before exact parent rebase/candidate review.
Checked myself: Actual completed lint job log, decisive Knip error and current source references; code fix and branch cascade remain pending.

What: All three failed PR416 test jobs have concrete failures assigned to the existing #369 author.
Why: Old515f misses a published-tool catalog definition and encounters absent request/task collections in two existing consumer fixtures or compatibility boundaries.
So what: Author resolves the real schema/caller owners and reruns the exact failures before clean handoff; parent then rebases the repaired candidate and starts fresh exact-head review/CI.

Parent retrieved all three completed job logs using GitHub Actions job logs, retained raw and ANSI-clean variants /tmp/chromeria-369-ci-{web,server2,server3}-515f[-clean].log, and inspected decisive failure sections plus current source consumers.

  • Web117633898128: six failures in V2LifecycleRow.remote.test.tsx, transcript/notification/background entrypoints with B connected true/false, RemoteTaskViewer.tsx:76 requests.filter on undefined. 7107 passed,6failed,527files,250.31s. This is not dismissed as React renderer warning noise.
  • Server2117633898076: core.test.ts:127 published-tool definition missing for t3_pending_request_dismiss, null labels/catalog owner. 1176passed,1failed,2skipped,103files,285.00s. Preserve the complete published-tool coverage assertion and add the actual definition.
  • Server3117633898182: WireProjection.test.ts:163 transcript bodies excluded from shell rows, ProjectionStore.remotePendingRequestKind:1556 tasks not iterable. 1353passed,1failed,2skipped,103files,290.35s. Determine whether the absence is a stale cast fixture or a valid old shape at the real schema/import boundary before choosing a fix.

Same author request 379-369-ci-three-test-jobs-fix-v1 explicitly requires tracing consumers/contracts and old381 mixed-version compatibility before adding defaults or editing fixtures. No arbitrary fallback, assertion weakening, timeout change or CI retry. Existing lint unused-export repair remains pending in the same job. Worker may commit/push routine fixes, but cannot self-rebase; clean handoff precedes parent topology cascade. Historical failed CI stays retained and both future reviews receive full findings/responses. PR416 remains watched; these observations are old515f, not a failed run of the future rebased candidate.

Requirements and who asked: #379 requires all client entrypoints, compatible persisted/wire shapes and current required CI.
Deleted: Unexplained/flaky labels, retries as fixes and weakening existing tool/consumer invariants.
Bottleneck: Small actual-owner fixes and focused regression verification before clean candidate handoff.
Checked myself: All three exact job logs and failure counts, affected current consumer/helper sources and native author delivery; repairs remain pending.

What: The five passing checks on c3d93a9 are PR metadata checks, not the required implementation CI.
Why: The current branch still conflicts with the repaired integration base and its check rollup contains only PR Size/Vouch jobs.
So what: Author finishes the three concrete test repairs and clean handoff; parent aligns the exact branch before fresh CI and both required reviews.

Parent inspected live PR416: head c3d93a9321ed3f44df548998f39789f78be8fce8, base feat/379-cross-machine-parity, mergeableCONFLICTING/mergeStateDIRTY. Rollup only Prepare PR size config, Collect PR targets, Label PR416, Label PR size SUCCESS and Sync PR size label definitions SKIPPED. No lint/typecheck/test/build/fork or independent-review acceptance on this head. Current commit fixes the unused export; earlier515f failures remain attached to their exact original run and need actual owner responses. Workspace clean atc3d, but native task still has active continuation, so parent has not taken write/topology ownership yet. Same-job notice 379-369-five-metadata-checks-not-ci-v1 prevents false-green handoff. Native watcher retained.

Requirements and who asked: #379 requires all current CI and both exact reviews, not a subset metadata headline.
Deleted: Five-check watcher headline as complete CI proof.
Bottleneck: Remaining test fixes and explicit clean stopped handoff before parent cascade.
Checked myself: Full live rollup, merge state, actual current branch log/status and native continuation state.

What: PR416 is rebased and published with identical source, and the complete fork check found two final repair paths missing from its registry.
Why: The old merge failure prevented CI from reaching path classification, while final f450 fixture repairs added two carried upstream test files.
So what: Same #369 author adds the two paths to the existing feature owner/inventory, runs the actual full fork check, and returns a new clean exact candidate for both reviews.

Parent read FULL3696248064304 and native-confirmed latest ordinal2 completed with no pending children before mutation. Original f450 preserved local and pushed rollback/369-pre-linear-repair-f450. Rebased exactly five own commits ontoab1bf536 from11cab, clean with no conflicts. Newhead 254f549424b32b008c769fb46296dadac66d94af, tree 32d66c654fa4e8545e90af6b8334aabcd84bc877 EXACTLY equals originalf450. Full range-diff shows all five unchanged:1ddd=781146db6a,34df=7f7afa859a,515f=d4655b117f,c3d=64ec40ddd0,f450=254f549424. No main/canonical/mobile/383 or other worktree mutation.

Actual bash scripts/fork-check.sh --upstream upstream on254f now clears merge-free topology but fails apps/server/src/orchestration-v2/WireProjection.test.ts and packages/client-runtime/src/t3ToolSummary.test.ts absent from carried edit allowlist. Log /tmp/chromeria-369-rebased-fork-check.log. No gate weakened. Published feature candidate with exact lease against oldf450, retaining rollback; normal fresh CI now applies to nonconflicting ancestry. Registry fix is actual missing ownership bookkeeping, not a new behavior defect.

Same explicitMEDIUM job resumed only for scripts/fork-upstream-edits.txt and existing feature ownership in docs/fork-features.md, command 379-369-rebased-registry-two-path-repair-v1. It must run actual upstream fork check/inventory/format/diff, normal commit/push, update complete original failure/response handoff, and stop clean again. No383 seams or topology edits. Both independent reviews wait final corrected exact head. Live catalog verified codex/gpt-6.1-sol/medium and installed grok/grok-4.7/medium available, no fallback; no reviewers dispatched prematurely.

Requirements and who asked: #379 current-base, ancestry and complete required fork policy.
Deleted: Inherited merge conflict, tree changes during rebase and unregistered carried test edits.
Bottleneck: Two-path registry owner fix before fresh exact candidate review/CI.
Checked myself: Full stopped handoff/native state, rollback refs, complete unchanged five-commit range-diff, exact tree equality, actual full fork-check errors and exact-lease publication.

What: Both required independent whole-component reviews are running on PR416 exact4cf9bd4, with both actual selections verified MEDIUM.
Why: The registry-corrected linear candidate is now clean/mergeable and needs fresh independent code proof; historical reviews and metadata checks do not establish it.
So what: Parent adjudicates both full reports and current CI, then performs actual source-client/provider/machine acceptance before internal integration.

Exact HEAD 4cf9bd4cef18fd88686e3ed7870ed834b3d78771, tree d18d25789a1db293b9a5d52b5ce4f18a20af9eed, base ab1bf53640e2a6541a2ba9f316839cedf81d4f46. CI39114466402 in progress; Fork39114466426 SUCCESS. An earlier fingerprint run cancelled while replacement39114524720 was running; no fullCI success claim.

Both NEW visible T3 delegate_task rounds received identical complete39555-character original brief, whole current #369 body and29636-character full canonical handoff3696248064304 including ALL known original CI failures, exact author repairs/responses, full proof counts/limits and parent topology correction. Review target is cumulative58files4147insertions638deletions. No prior #369 CODE reviews exist. Required incoming #380/#381/#382 contracts/histories are mandatory referenced material, design approval remains historical. Reviewers cannot write production/source, launch product servers/browser/device/providers, or alter other checkouts; own worktrees permit focused native fixture tests with external leaves. Parent owns aggregation of both verdicts, not a vote or historical approval transfer.

  • Sol task node:delegated-task:command%3Amcp%3A0dd33a23-3bda-415f-8c7f-53320ec4c2ad%3Adelegate-task%3A379-369-review-r1-codex-medium-4cf9bd4, codex/gpt-6.1-sol/MEDIUM, own /Users/rocky/.t3/worktrees/chromeria/review-369-r1-sol-4cf9bd4.

Complete prompt retained /tmp/chromeria-369-r1-original-review-brief.txt for all future new rounds including full findings/responses. No implementation reviewer success, CI gate, integration or real-client acceptance is claimed. Native PR416 watcher remains active; completion wakes parent.

Requirements and who asked: User explicitly requires BOTH fresh independent SolMEDIUM and installedGrok4.7MEDIUM whole-candidate reviews plus currentCI.
Deleted: Split coverage, historical design approval as code proof, silently inherited effort defaults and metadata-green admission.
Bottleneck: Full independent findings and actual current CI, followed by parent real client/runtime acceptance.
Checked myself: Live Git/PR/base/tree/diff/fork/native author stop, complete prompt/hand-off, returned actual review selections and read-only persisted medium options.

What: Both review worktrees now have isolated locked project dependencies and can execute focused checks.
Why: Native-created review worktrees had no node_modules; Sol’s commands failed127 before any tests, and the configured setup script depends on a PATHvp absent in the parent shell.
So what: Both same R1 reviewers continue exact4cf9 proof; #386 retains the native preparation observation for final delegated-lifecycle acceptance.

Parent inspected t3.json and full scripts/setup-worktree.ts: setup uses vp i -- --yes, then env links/cache warming. No dedicated callable setup-retry tool exists in the exposed worktree catalog; the handoff tool cannot reattach an already-bound review. Actual origin of missing native setup remains unproved, so absent PATHvp in the parent shell is an observation, not a proven native subprocess cause. Existing #197 concerns cleanup/worktree placement, not established same defect.

Parent restored dependencies only in each app-bound reviewer worktree with CI=true PATH=/opt/homebrew/opt/node@24/bin:$PATH pnpm install --frozen-lockfile --offline --ignore-scripts --package-import-method=copy. Both exit0, downloaded0, existing pinned lockfile, no scripts/env links/credentials/other-workspace dependency borrowing or tool/device runtime install. Logs /tmp/chromeria-369-review-sol-dependencies.log and ...grok...,75.7s/74.4s. Exact4cf9 heads and clean Git status unchanged; each node_modules/.bin/vp executable verified. Reviewer source inspection continued while preparation ran, and each received same-job native ready notice. No new review round, provider/model/effort switch or tested-pass claim. Original127 attempts remain not-executed tests. No product processes were launched.

This is preparation recovery, not acceptance of native automatic setup. Final #386 delegated lifecycle must inspect why app-bound workspaces lacked expected setup and prove the supported preparation path truthfully. No globalvp install, automatic job or unrelated cleanup was introduced. PR416 remains watched and both exact-candidate MEDIUM reviews pending.

Requirements and who asked: #379 requires complete native delegated lifecycle and both exact independent code reviews with honest proof gaps.
Deleted: Borrowed worktree dependencies, native setup dispatch as completed preparation, ambient env/credential linking and repeated127 test attempts.
Bottleneck: Review execution unblocked; final native workspace preparation cause/proof remains owned by combined acceptance.
Checked myself: Both missing dependency directories, full configured setup, exposed native tool capability, actual locked offline installs and exitcodes, zero-download logs, exact clean heads and executable readiness.

What: APPROVE exact #369 candidate 4cf9bd4cef18fd88686e3ed7870ed834b3d78771 on PR #416. No supported blocking defect in the inspected ancestry, actor, admission, or source-client paths.
Why: Destination-owned pending requests now require proved local-edge plus delegation-pair ancestry, take human authority only from the authenticated source session, and admit answers inside the native event, receipt, and outbox transaction.
So what: Parent aggregates this verdict with the other independent review and owns review/independent, integration, and live client, provider, and machine acceptance. This review did not merge, push, edit source, or write that status.

Elon record

Requirements and who asked: Parent #379 required one independent whole-candidate review of #369 at this exact head by installed Grok grok-4.7 with reasoning effort medium. The user outcome is local-equivalent sight and answer of eligible remote descendant questions and approvals.
Deleted: A split review, transfer of the frozen v5 design approval or of historical f450 proof, quota nits, and any demand that unapproved #383 device hooks land inside this component.
Bottleneck: One ancestry, actor, revision, and native-acceptance owner, plus source clients that can act without a destination session.
Checked myself: Live catalog and this thread's persisted selection, exact head, tree, and base, the load-bearing server and client owners, and new focused runs on this worktree.

Verdict

Reviewer: installed Grok CLI, provider instance grok, model grok-4.7, option reasoningEffort=medium. The live catalog lists medium for that model. The catalog default shown for Grok 4.7 is high. This thread's persisted selection is medium. No fallback and no nested delegate.

Findings

No required finding. No optional rewrite.

Cleared by source and by the runs below:

  • Mixed ancestry is the transitive closure of app-owned local subagent edges and one authenticated return pair per hop, bounded at 256 threads and 16 pair hops. A to B to C, a later return onto A, and a local grandparent plus a pair are accepted. Self, fork (rows.length !== 1), unrelated callers, missing callers, and an OAuth client without a proved actor are denied. A foreign id is not written into childThreadId. Remote notices omit that field. task_status in the approval case expects childThreadId: null.
  • Human authority is the current authenticated source session. PendingRequestIngress passes only session.sessionId. Read requires orchestration:read and answer or dismiss requires orchestration:operate, rechecked on the source record and again through the request-actor frame. pendingActor returns client only for that session. An MCP body actor or human flag is ignored. The WebSocket test rejects an agent bearer on the source socket. Revocation fails the next call. Ordinary ancestral agent approvals stay on the thread actor path. Narrowing the parent from full-access to auto denies a later answer. A revoked destination grant denies read and respond. There is no blanket human-only ban.
  • Revision is the max event sequence of that request's runtime-request.updated and turn-item.updated rows, so both runtime and question content move it. Respond and dismiss plan through the existing orchestrator commands. pendingResponseAdmission runs in the same commit as the events, receipt, and outbox. The guard reads the local projection and SQLite only. It rejects a non-pending request, a stale revision, and a changed shape. Option and answer shape are checked before planning when expectedRevision is present. Linked clients must send that revision. The same clientRequestId replays one acceptance, including after a lost reply and after destination transport rebuild. A changed answer on that key fails. Concurrent answer and dismiss leave one resolved request. Intake that resolves the request before commit loses to the guard. A native Stop followed by a late live answer adds no response effect. Provider limits stay on the effect: not_resumable is an error, not a successful callback. Async message answers dispatch through the normal message path and do not pretend the provider accepted a live callback.
  • Source clients read and act through remoteTasks.requestRead and remoteTasks.request on the source environment. Web and desktop share ChatView, useSubagentOpen, RemoteTaskViewer, and RemotePendingRequest. Mobile has the same request card and source viewer gates. Negotiated lifecycle version 1 opens the source viewer when B is connected or absent. Old imported tasks keep direct navigation. Stable action keys survive a lost reply. Refresh mints a new key. Dismiss is a real catalog action and a write. Shell attention carries compact remote pending kind up local ancestors. Auth refresh and dynamic tool calls stay out of ancestor answer surfaces.
  • Old peers that do not advertise pending-request protocol version 1 fail the mutation before dispatch. Reads are not given a success fallback.

Proof executed on this head

Worktree only. Node 24 at /opt/homebrew/opt/node@24/bin. ./node_modules/.bin/vp. No product server, browser, device, provider, or other checkout.

  1. vp test run of the ten fixture files: core catalog, WireProjection, web remote lifecycle rows, shared presentation, client-runtime remote tasks, tool summary, web and mobile request cards, thread shell, agent awareness. 122 passed, 10 files, 7.13s.
  2. vp test run apps/server/src/peer/PeerAuthority.workflow.test.ts -t 'pending369:'. 8 passed, 84 skipped, 43.72s. Those eight use disk SQLite, authenticated HTTP or WebSocket, and the native orchestrator. They cover A to B replay and changed intent, A to B to C, source-session human read and answer plus return to A, one provider callback, dismiss versus answer, production WebSocket scope and revocation, intake-before-commit, and local grandparent plus pair.

Live PR rollup observed for this head, not treated as merge acceptance: Lint, Typecheck, Build, Test, Test Web, Test Server 1 through 6, Fork Stack Model, Rust, Release Smoke, and Check are success. One earlier Native fingerprint diff is cancelled. EAS Preview, Deploy web preview, Sync PR size label definitions, and Mobile Native Static Analysis are skipped. I did not rerun those jobs.

Limits

  • Not live verification. No physical client click, no real provider process, no cross-machine acceptance home.
  • The other 84 workflow cases, and the author's earlier 344, 123, and 29 case groups, were not rerun here. Their old logs stay on their original SHAs. Tree equivalence is not a new execution.
  • Repository-wide lint, typecheck, and test were not run locally. CI above is an observation of GitHub's rollup.
  • feat(peer): view and drive remote browsers and devices from a source-only client #383 typed-device production paths and per-effect recheck remain a separate gap. This candidate does not regress the assigned native device fixture repair, and this review does not require those feat(peer): view and drive remote browsers and devices from a source-only client #383 hooks.
  • Desktop was reviewed as the shared web client. It was not launched.
  • Cost remains unknown. No Observer total was invented.
  • review/independent was left for the parent aggregate. This comment does not set it.

Responses to the known 515f failures

Those failures are fixed in this tree, not waived. Unused PendingRequestListTool export is gone while registration remains. The six viewer crashes were stale fixtures missing required requests. The dismiss catalog entry exists and core still expects it. WireProjection decodes the real projection, including required subagents, instead of a stale cast. This head's 122-case run and the eight native cases passed without a production fallback for missing snapshot fields.

What: R1 verdict is REQUEST_CHANGES on exact 4cf9bd4cef18fd88686e3ed7870ed834b3d78771: two reproduced P2 findings remain.
Why: A read can pair old request content with a newer revision, and the new response validation rejects an explicitly supported empty-string choice.
So what: Parent adjudicates these findings with the other independent whole-candidate review and owns repairs, fresh candidate review, aggregate status, CI and acceptance.

Identity and independence

R1-S1, P2: return request content and its revision from one snapshot

Location: pendingRequests.ts:42-83, particularly the new revision query at lines71-83. Consumer: PendingRequestService.execute returns that captured item and revision; Orchestrator.ts:8323-8338 later compares the supplied revision with current events.

Reachable trigger: A native turn-item.updated changes a pending question/approval after readThread has loaded its content but before the separate MAX(sequence) query. The projection owner's read transaction has already finished. Approval reads also perform ancestry/control-channel work between these operations, increasing the available interval. Runtime intake is not serialized by this read.

Actual: The read returns the old question plus the new revision. A subsequent response with exactly that returned revision loads the new content, passes both revision and commit-shape checks, and applies the caller's answer to content it never read. This defeats the stale-content protection even though response admission itself is transactional. The same read helper serves source-client cards and ancestor MCP reads.

Independent executed evidence: A disposable extension of the existing registered workflow uses actual A/B authenticated native MCP/control transport, production kernels, disk SQLite and native event/command owners. A scheduling barrier retains the exact result of the real getProjectThreadRecords call, commits a real native turn-item.updated, then releases that result. It changes no policy decision, projection value or SQL result. The public read returns question="Which color?", revision=21; the public response with that revision succeeds, sequence=29; the native persisted message is "Which UPDATED color?\nBlue" and the request is resolved. The invariant that the read must either show the update or return a revision that rejects the answer fails. Log: /tmp/review-369-r1-sol/probes.log, R1 READ RACE EVIDENCE.

Required: Obtain request/item content and its corresponding revision within one native local read snapshot, then perform network authorization outside that SQL transaction. Keep the current transactional response admission and content/options/status checks. Add the read-versus-intake interleaving regression; do not fix this by weakening revision checks or moving reverse authorization inside SQL.

R1-S2, P2: preserve an explicitly offered empty-string answer

Locations: Orchestrator.ts:8400-8416, web RemotePendingRequest.tsx:44-53, and the identical mobile completeness check at lines44-53.

Reachable trigger: A delegated Pi extension emits an input dialog, or a select whose advertised value is "". This is an existing supported provider path, not a hypothetical optional schema. Pi adapter:3384-3405 emits Submit empty value, value "", without initialAnswer; its response owner at lines3418-3422 explicitly distinguishes an empty value from cancellation.

Actual: Selecting that offered choice leaves both new source-client cards incomplete because they require trimmed nonempty text. An ancestor using the public remote pending tool instead is rejected with The answer does not match the current pending request. The rejection is introduced by the expectedRevision-only validation. The equivalent existing local runtime response accepts answers: {color:""} and resolves the same request. A remote human or parent therefore cannot choose a valid native answer that is available locally.

Independent executed evidence: The unchanged Pi adapter case offers an explicit empty value for extension input dialogs passes and observes an actual deterministic adapter response value="", with no cancelled flag. A separate registered A/B production-kernel probe seeds that exact advertised question shape through native events, reads it through MCP (responseType=live, revision=23), and submits the empty choice. Remote: error above. Local native command control without the newly added revision validation: Success, sequence26, stored resolved request with answers:{color:""}. Its required remote-equivalence assertion fails. Log /tmp/review-369-r1-sol/empty-option.log, R1 EMPTY VALUE EVIDENCE. No live provider was launched. UI disabled-state conclusion is source-reviewed, not claimed as a browser/device observation.

Required: Treat an explicitly selected advertised empty value as a valid answer in the native validator and both cards, while continuing to reject genuinely unanswered required questions and invalid option values. Preserve the provider's empty-value versus cancellation distinction. Cover the actual offered-empty shape in native response and client interaction tests.

Whole-candidate assessment and retained contracts

Reviewed the cumulative 58-file base..HEAD diff, including tests, registration/docs/inventory and relevant unchanged owners/consumers. Applied canonical operations verification/code-review/review-method/impact-analysis, current project instructions/direction/glossaries, Effect services, Great Library README, full current #379 body/comments and frozen-v5 execution contract, full #369 body/canonical supplied handoff, and incoming #380/#381/#382 original/final handoffs relevant to preserved guarantees. Historical approvals are evidence about their original candidates only.

  • Ancestry and actors: Traced real local subagent edges plus pair-bound reverse ancestry, strict-self exclusion, fork/unrelated/missing-caller/OAuth denials, A-B-C and later-back-on-A routing, selected task subtree restriction, actual target checks, current grants/generation/modes and original ALL-contributor authority. Ordinary live ancestral approvals remain eligible; no blanket human-only ban. Human authority originates in authenticated client ingress/current session scopes and never public MCP actor flags. Existing registered controls pass.
  • Native acceptance: Traced runtime and turn-item revisions, native serialized planning, option/shape/status guards, Stop fencing, commit guard/event/receipt/outbox transaction and effect claim/execution. No new network work inside that SQL guard. Concurrent answer/dismiss, changed intent, lost committed reply, renewal/reconstruction and one callback are exercised in the passing workflow. S1 is the distinct pre-response read-snapshot hole.
  • Consumers: Inspected compact source-shell attention and local-ancestor propagation, shared web/desktop and separate mobile cards/viewers, composer banner, transcript/notification/background/task opens, permission guards, source-only connection addressing with B connected/absent, old imported/local navigation and lifecycle negotiation. Async message questions remain nonblocking, and unavailable callbacks remain truthful. No browser/device acceptance claimed.
  • Incoming safeguards: Original feat(peer): remote children return results and delegate onward within durable grants #380 contributor/target/mode/cancellation controls, feat(peer): continue and stop remote delegated jobs after their first result #381 Stop history/original pair/terminal-commit result/version compatibility/production outbox composition, and feat(peer): transfer attachments and make remote retries preserve operation identity #382 original actor/key/generation/native replay/retention controls remain in inspected code and the passing workflow. No new conclusion about old native404/discovery/timing failures. Separate feat(peer): view and drive remote browsers and devices from a source-only client #383 typed-device/shared proposals and fixture artifacts are not demanded as feat(peer): surface and answer remote descendant requests from the parent #369 fixes; only the assigned existing native-device fixture repair is in this diff.
  • Catalog/migration/cost/performance: Dismiss is registered at the actual tool presentation/summary/write owners; current types use optional additive historical fields and required native rosters. Reviewed the unchanged registered tool/effect boundaries and device catalog scope without claiming all 59 device commands were tested here. Shell attention excludes bodies and bounded observations remain bounded. No measured performance/cost improvement or complete all-tool/client acceptance claim. Cost remains UNKNOWN.

The known old515f unused-export, six viewer crashes, missing dismiss presentation and WireProjection missing-roster failures and their complete author response were review input. Current code removes the unused export, uses typed snapshot/native projection decoding and supplies actual dismiss presentation; it does not add a production missing-data fallback. The new registry-only final commit assigns the two real upstream test paths. The repaired tests pass freshly here, independently of the earlier 100/92/344/123/29 reports and tree-equivalence claims. Parent's old merge11cab failure is repaired topology, not an unexplained source failure.

  4. test run packages/provider-pi/src/server/adapter.test.ts -t 'offers an explicit empty value for extension input dialogs': 1 passed, 92 filtered, 3.18s, exit0. pi-empty-control.log.

Those are 407 distinct unchanged passing cases across 20 files, scoped feedback, not the repository gate. Separately, disposable registered probes fail at the intended observations for S1 and S2. An additional optional fixed-choice probe also rejects an unanswered required:false field; no current producing path was established, so it is preserved as a contract-edge observation and is not a third blocking finding. First probe command: test run apps/server/src/peer/PeerAuthority.review369r1.test.ts -t 'review369 R1:', 2 failures/92 filtered, 8.57s. Empty-choice command: same file with -t 'review369 R1: advertised empty', 1 failure/94 filtered, 9.36s. These are reviewer counterexamples, not failures of the unchanged 407-case selections.

  • /tmp/review-369-r1-sol/probes-added.ts, empty-option-probe.ts, raw logs above.

node scripts/fork-features.mjs check: exit0,42features. git diff --check base HEAD: exit0. Final tracked/untracked status is clean, exact head/tree unchanged; disposable test file removed after preserving it. Initial two test attempts exited127 before execution because this newly prepared worktree lacked node_modules/.bin/vp. Parent restored only offline frozen copied dependencies with scripts disabled and then confirmed completion; all executions above followed that confirmation. The original127 attempts remain non-executions, not passes.

One live PR read during review confirms exact head/base target, current ordinary Check/lint/types/build/web/all six server/test/Rust/smoke/fork checks successful, independent review pending; policy-skipped/cancelled ancillary checks remain distinct. /tmp/review-369-r1-sol/live-pr.json retains the full snapshot. This is not my execution of CI or an aggregate acceptance decision, and it does not negate the reproduced findings.

No independent type/lint/Knip rerun, repo-wide test/check, product server, browser/device/simulator, live provider operation, live userdata/parent fixture access, installation, extra agent, fix, commit/push, merge, Issue closure, blocker clearing, deployment or version change. Registered tests use finite loopback transports, native SQLite/filesystem and deterministic external leaves; they do not prove actual clients, physical cross-machine behavior, real provider callbacks/process death or OS crash recovery. Parent retains that acceptance. Earlier author/parent failures and their UNKNOWN causes are not erased or retrospectively explained by these passes.

Requirements and who asked: User/parent requested this independent whole #369 R1 exact-candidate Sol MEDIUM review under frozen #379, preserving native authority, revision, acceptance and source-only parity.
Deleted: Historical approval transfer, narrow split review, extra delegation, policy mocks, authored fixes, speculative #383 requirements, broad local gates and unsupported cause/cost claims.
Bottleneck: Read content/revision consistency and local-equivalent valid answer semantics at their existing native/client owners.
Checked myself: Exact catalog/actual Color/Git/PR binding, all58diff paths and relevant consumers, incoming contracts and CI repair responses,407unchanged scoped passes, two reachable native counterexamples and local/provider controls, final clean source and preserved probe artifacts.

What: #369 R1 aggregate is REQUEST_CHANGES on exact4cf9 despite passing CI: Sol reproduced two P2 defects; Grok approved its inspected scope.
Why: Request content/revision can describe different snapshots, and a valid provider-offered empty value is rejected remotely.
So what: Same author repairs both at their native/client owners, then parent dispatches BOTH NEW independent MEDIUM whole-candidate rounds with full original findings/responses.

Sol whole report #416 (comment) . Grok whole report #416 (comment) . Both tasks terminalcompleted/no pendingchildren, exacthead4cf9/tree d18d2578/baseab1bf536, actualMEDIUMverified earlier. Parent read BOTHfullreports, current owners and raw Sol counterexample logs. Aggregate exact4cf9 review/independent FAILURE. No review vote or one-approval admission.

S1 ACCEPTED: readThread captures native request content before a separate revision MAX(sequence) query. Real turn-item intake between them returns old question with newrevision21; native response accepts sequence29 against updated question text. Fix atomic native local read snapshot for content+revision, network authorization outsideSQL, preserve transactional response admission/current authority/shape/options/status. Reviewer scheduling barrier retains actual native read result, does not replace policy or SQL.

S2 ACCEPTED: actual Pi input/select advertises explicit value emptystring and response owner distinguishes it from cancellation. Native remote expectedRevision validator and both cards reject it; equivalent native local response succeeds sequence26. Fix explicit offered/selected empty option semantics, keep genuinely unanswered required input and invalidoptions denied, native/public and web/mobileinteraction proof. Parent inspected actual Pi producer/response source and originalraw evidence. Optionalfixedchoice probe has no established currentproducer, stays nonblockingcontractedge, not a third finding.

Sol407distinct unchanged scoped cases20files pass plus intended failing counterexamples; Grok122clientcases10files and8pendingnative pass. Neither passing selection covers away the demonstrated interleavings/empty-provider edge. All original source identities/counts retained, no real provider/browser/device/machine acceptance. Worker samejob resumedordinal4, original4cf9 reviewerworktrees remain immutable; baseab1 preserved, no383work or selfrebase. Nextcandidate packet must contain BOTHFULLunchanged reports and allauthorresponses. CI4cf9SUCCESS remains narrower than required defect resolution; no merge/edgeclear/integration/ship.

Requirements and who asked: #379 requires both independent exact-candidate reviews and native local-equivalent request/revision semantics.
Deleted: CI success or one approval as acceptance, stale content/new revision pairing and explicit empty option as unanswered.
Bottleneck: Same-owner S1/S2 red/green repair before both fresh code reviews.
Checked myself: Both complete reports, native taskterminal state, source read/revision/validator/card/Pi owners and raw exact native counterexample evidence.

  4. test run packages/provider-pi/src/server/adapter.test.ts -t 'offers an explicit empty value for extension input dialogs': 1 passed, 92 filtered, 3.18s, exit0. pi-empty-control.log.

These are reviewer counterexamples, not failures of the unchanged 407-case selections.

node scripts/fork-features.mjs check: exit0,42features. git diff --check base HEAD: exit0. Final tracked/untracked status is clean, exact head/tree unchanged; disposable test file removed after preserving it. Initial two test attempts exited127 before execution because this newly prepared worktree lacked node_modules/.bin/vp. Parent restored only offline frozen copied dependencies with scripts disabled and then confirmed completion; all executions above followed that confirmation. The original127 attempts remain non-executions, not passes.

One live PR read during review confirms exact head/base target, current ordinary Check/lint/types/build/web/all six server/test/Rust/smoke/fork checks successful, independent review pending; policy-skipped/cancelled ancillary checks remain distinct. /tmp/review-369-r1-sol/live-pr.json retains the full snapshot. This is not my execution of CI or an aggregate acceptance decision, and it does not negate the reproduced findings.

No independent type/lint/Knip rerun, repo-wide test/check, product server, browser/device/simulator, live provider operation, live userdata/parent fixture access, installation, extra agent, fix, commit/push, merge, Issue closure, blocker clearing, deployment or version change. Registered tests use finite loopback transports, native SQLite/filesystem and deterministic external leaves; they do not prove actual clients, physical cross-machine behavior, real provider callbacks/process death or OS crash recovery. Parent retains that acceptance. Earlier author/parent failures and their UNKNOWN causes are not erased or retrospectively explained by these passes.

FULL UNCHANGED GROK R1 REPORT:

Verdict

APPROVE. No required change.

Reviewer: installed Grok CLI, provider instance grok, model grok-4.7, option reasoningEffort=medium. The live catalog lists medium for that model. The catalog default shown for Grok 4.7 is high. This thread's persisted selection is medium. No fallback and no nested delegate.

Identity: HEAD 4cf9bd4cef18fd88686e3ed7870ed834b3d78771, tree d18d25789a1db293b9a5d52b5ce4f18a20af9eed, base ab1bf53640e2a6541a2ba9f316839cedf81d4f46. Cumulative diff is 58 files, 4147 insertions, 638 deletions. Worktree review-369-r1-grok-4cf9bd4 was clean before and after the runs. PR head still matches.

What: APPROVE exact #369 candidate 42051c47813c354682b80677009b199d86fef1a0 on PR #416. No supported blocking defect in the repaired native snapshot, the offered empty answer, or the rechecked ancestry, actor, admission, and source-client paths.
Why: Content and revision now come from one native SQL snapshot, and an explicitly advertised empty choice is accepted on the native validator and both source cards.
So what: Parent aggregates this verdict with the other independent review and owns review/independent, integration, and live client, provider, and machine acceptance. This review did not merge, push, edit source, or write that status.

Deleted: Transfer of the 4cf9 Grok approval or of CI on that head, a narrow repair-only review, dependency injection as production proof, and any demand that unapproved #383 device hooks land inside this component.
Bottleneck: One native read snapshot for content and revision, and local-equivalent handling of a provider-offered empty choice, with the rest of the actor and admission contract still holding.
Checked myself: Live catalog, this thread's grok-4.7 selection, exact head, tree, and base, the repair and the load-bearing owners, and new focused runs on this worktree.

Identity and independence

Verdict

APPROVE. No required change.

Findings

No required finding. No optional rewrite.

The accepted R1 defects are closed in this tree:

  • S1. readPendingRequest still resolves the caller and prepares any legacy transcript through readThread outside SQL. It discards that projection. With SQL and the production ProjectionStore, it then reads runtimeRequests and turnItems and the request's MAX(sequence) inside one SqlClient.withTransaction. Ancestry and current authority run after that transaction. SQL without a projection owner fails closed. No SQL returns the legacy projection and invents no revision.
  • S2. The native expectedRevision validator and both source cards treat a present value as answered when it equals an advertised option, including "". A missing required answer, a bad array shape, and an unadvertised fixed option stay denied. The Pi adapter still emits { value: "" } and still treats a non-string as cancellation.

Cleared on this head by source and by the runs below:

  • Snapshot mechanism. ProjectionStore.getThreadRecords reads canonical SQL rows and opens its own transaction. Effect SQL 4.0.4 turns a nested withTransaction on the same client into a savepoint, so the revision query stays on the outer transaction. PendingRequestService passes the runtime SqlClient and ProjectionStore into that read. runtimeLayer.layer now exports ProjectionStore.layer, and the production-runtime test reaches that service with no injected store. The nested read in that test finished in the same run, which a second write connection would not do under BEGIN IMMEDIATE.
  • Ancestry and actors. Mixed ancestry remains local subagent edges plus one authenticated return pair per hop, bounded at 256 threads and 16 pair hops. A fork is rows.length !== 1. Self, unrelated callers, missing callers, and an OAuth client without a proved actor stay denied. Remote task status still returns childThreadId: null and puts the foreign id on remoteChild. Human authority is session.sessionId from the authenticated client handlers. Read and operate recheck orchestration:read and orchestration:operate. An MCP body actor flag is not an input to those handlers. Ordinary ancestral agent approvals stay on the thread actor path.
  • Admission. Respond and dismiss still plan through the orchestrator commands. pendingResponseAdmission is attached to that commit with the events, receipt, and outbox. Its guard reads the local projection and SQLite only. Revision still covers runtime-request.updated and turn-item.updated. Stop fencing, shape checks, one clientRequestId, and the intake-before-commit guard are unchanged. The message reply builder still refuses a required answer whose trimmed text is empty. That path is shared by local and remote commands, so an empty message-mode reply stays local-equivalent. The repaired hole is the live expected-revision check, which now keeps answers: { color: "" }.
  • Source clients. Web and desktop share the request card. Mobile has the same completeness rule. The button stays disabled until a real selection, including the offered empty choice, then sends that value, retries the lost reply with the same action key, and mints a new key on refresh. Compact shell attention and qualified addresses are unchanged in this repair.
  • Incoming safeguards. The inspected feat(peer): remote children return results and delegate onward within durable grants #380 contributor and target checks, feat(peer): continue and stop remote delegated jobs after their first result #381 Stop and terminal-commit admission, and feat(peer): transfer attachments and make remote retries preserve operation identity #382 identity and replay guards remain in the owners above and in the passing workflow cases. This candidate does not add feat(peer): view and drive remote browsers and devices from a source-only client #383 production hooks. The assigned native device fixture repair remains in the cumulative diff.

Proof executed on this head

Worktree only. Node v24.18.0 at /opt/homebrew/opt/node@24/bin. ./node_modules/.bin/vp. Dependencies were the parent's completed offline frozen copy with scripts ignored. No product server, browser, device, provider process, or other checkout.

  1. vp test run apps/server/src/peer/PeerAuthority.workflow.test.ts -t 'pending369': 10 passed, 84 skipped, 50.28s, exit 0. Log /tmp/review-369-r2-grok/workflow-pending369.log. The ten cases use disk SQLite and the native orchestrator. They cover A to B replay, A to B to C, source-session human read and answer plus return to A, one provider callback, dismiss versus answer, production WebSocket scope and agent-credential rejection, intake before commit, local grandparent plus pair, the read snapshot, and the empty live option.
  2. The snapshot case kept the real getProjectThreadRecords result across a real turn-item.updated, then released it. The public read returned question: "Which UPDATED color?" and revision: 21. The answer with that revision succeeded at sequence: 29. The stored message is "Which UPDATED color?\nBlue" and the request is resolved. The read showed the update. It did not pair the old question with the new revision.
  3. The empty-option case rejected {} and { color: [""] }, accepted { color: "" } for the Pi advertised option, stored answers: { color: "" } as resolved, and left an unadvertised fixed option pending.
  4. vp test run of the web card, mobile card, and Pi adapter with -t 'answers offered|offers an explicit empty value': 5 passed, 92 skipped, 3.20s, exit 0. Log /tmp/review-369-r2-grok/cards-pi.log.
  5. vp test run apps/server/src/orchestration-v2/runtimeLayer.test.ts -t 'exports the native request read owner': 1 passed, 82 skipped, 9.77s, exit 0. Log /tmp/review-369-r2-grok/runtime-owner.log. The first attempt exited before Vitest because the log directory was not created yet. That was not a test result. The rerun above is the execution.

Limits

  • Live web, desktop, and mobile clicks, a real provider process, and a physical cross-machine acceptance home remain with the parent.
  • The other 84 workflow cases were not rerun. The author's full run of this file, 94 passed in 324.29s, was not rerun here. The earlier run on the repair source, 93 passed and 1 failed in 312.33s with a native initialization 404 and no response body, stays UNKNOWN. This green selection does not explain it and does not show that it was a flake.
  • Repository-wide lint, typecheck, and test were not run. Current CI on this head was in progress at dispatch and is not claimed here. Green CI on 4cf9bd4 belongs to that older head.
  • The registered snapshot barrier holds the initial management read, then the snapshot reads after the update has committed. It proves the initial content is discarded. Same-statement atomicity is the shared SqlClient transaction and the nested savepoint, plus the production runtime read above.
  • Card and mobile checks use their installed renderers, with mocked remote commands in the card test. They are not device or browser acceptance.
  • feat(peer): view and drive remote browsers and devices from a source-only client #383 typed-device production paths remain a separate gap. This review does not require those hooks.
  • Cost remains unknown. No Observer total was invented.
  • review/independent is left for the parent aggregate.

Responses to the accepted R1 findings

S1 and S2 are fixed at the owners named in the adjudication. The optional fixed-choice probe with no established producer stays a nonblocking contract edge, not a third finding. Historical 515f failures and the registry-only 4cf9bd4 commit remain repaired history. They are not new proof on this head and they are not waived.

What: R2 verdict is APPROVE on exact 42051c47813c354682b80677009b199d86fef1a0; no supported blocking finding remains in this whole-candidate review.
Why: S1 now reads native content and revision from one SQL snapshot, and S2 preserves an explicitly offered empty answer without admitting absent required or invalid fixed-choice answers.
So what: Parent aggregates both fresh independent reviews and owns final CI assessment, integration and actual client/provider/machine acceptance; this comment does not set the aggregate status.

Identity and independence

  • Actual Color: Codex harness / codex / gpt-6.1-sol / medium. Live catalog supports medium; inherited provider/model and app thread/worktree match. This run's persisted Codex turn_context independently says model=gpt-6.1-sol, effort=medium, and collaboration reasoning_effort=medium. The catalog default low is not the selected effort. Parent independently confirmed the selection too.
  • PR app-link succeeded. This reviewer leaves review/independent to parent aggregation and does not transfer any R1 or design approval.

Findings and accepted R1 repairs

No required finding. No optional rewrite is requested.

R1-S1 is cleared in the inspected and executed scope. In apps/server/src/childThreads/pendingRequests.ts:42-76, initial readThread resolves caller/target and performs native/legacy preparation outside SQL. That earlier content is discarded on the native SQL path. The helper reads ProjectionStoreV2.getThreadRecords(runtimeRequests, turnItems) and the request's MAX event sequence inside the same outer SqlClient transaction. Both runtime-request and turn-item updates contribute to that sequence. Missing native projection ownership explicitly fails; the original no-SQL composition returns its actual legacy content without inventing a modern revision.

I traced the production runtimeLayer.ts export, existing ProjectionStore construction and server providers to the shared SqlClient. The installed Effect 4.0.4 SqlClient uses its transaction service to retain the connection and nested savepoints; ProjectionStore.getThreadRecords therefore participates in the outer snapshot. This is not an assumption based only on a fixture-injected projection service. The production runtime test constructs RuntimeLayer.layer without injecting ProjectionStore, commits a real native thread, obtains the exported projection and reads its actual event sequence under that SQL context. It passes freshly here.

The original real-management-read scheduling barrier is retained: it holds the exact native read result, commits actual turn-item intake, then releases that old result. The public read/response invariant requires current question content or a stale-revision refusal. The fresh full workflow includes this regression. No policy result, projection payload or SQL result is replaced by the barrier. Reverse ancestry, current authority and target checks remain after the completed read transaction, outside SQL. Existing response planning, revision/shape/options/status/Stop checks and the event/receipt/outbox commit guard remain intact.

R1-S2 is cleared in the inspected and executed scope. Orchestrator.ts:8404-8438 accepts a present value matching an advertised option even when that value is empty. The web/desktop-shared and mobile RemotePendingRequest completeness checks distinguish an absent answer from a selected offered empty value. Both client interactions start disabled, select the empty option, enable submission, send answers:{color:""}, retry the exact same operation/key after a lost acknowledgement, then mint a different action identity on explicit refresh.

The native public regression accepts and persists the offered empty value as resolved. Its absent-answer, invalid array-shape and unadvertised fixed-option controls still deny, with the latter request remaining pending. I also read the actual Pi piQuestion producer and piUiResponse consumer: empty string is an advertised input/select value and differs from cancellation. Its unchanged adapter control passes independently here. No provider implementation was changed. The earlier optional fixed-choice/no-established-producer observation remains nonblocking, not a third accepted finding.

Whole cumulative candidate

Reviewed all 59 cumulative diff paths, 4536 insertions/639 deletions, from base through current head, including implementation, tests, production composition, contracts, docs/map and fork inventory. This is not a ten-file repair review. Read the full supplied current repair packet, both unchanged R1 reports, parent adjudication, author responses and historical original brief; current #369 body/map and #379 body/comments/frozen-v5 execution contract; relevant incoming #380/#381/#382 handoffs and their retained constraints. Canonical operations verification/code review/review method/impact analysis, project direction/instructions, glossaries, Great Library README and Effect service conventions informed the review.

  • Ancestry and actor: Traced strict mixed native subagent/pair ancestry, A-B-C, later return to A and local-grandparent paths. Self, fork, unrelated, missing-caller and outside OAuth actions remain denied. Selected-task membership is separately proved for human reads, including back-on-A. Human actor provenance comes from authenticated source ingress/current session read or operate scope and the bound request-actor control exchange, never public actor flags. Ordinary eligible ancestral runtime approvals remain supported.
  • Authority and native response: Inspected current source/target modes, original frozen ALL contributors, grant/generation/liveness, original actor/key scope and negotiation. Response identity reuses native receipts and deterministic command IDs. Revision, pending status, content/shape/options, Stop and intake-before-commit checks arbitrate races under the existing native acceptance owner. Concurrent identical responses, changed intent, lost committed reply, answer/dismiss races, reconstruction and one provider callback are exercised by the native workflow. Network authority work is outside the native SQL commit guard.
  • Clients and attention: Inspected compact source-shell attention and propagation through real local ancestors, source-only shared web/desktop and separate mobile cards/viewers, composer/floating entrypoints, transcript/notification/background/task opens and permission guards. Negotiated jobs open through A whether B is connected or absent; imported/unmarked/local navigation retains its existing path. Async message questions remain nonblocking; unavailable callbacks remain truthful. Client interactions and guarded source RPC tests are component/state proof, not a browser/device observation.
  • Incoming guarantees: The workflow retains feat(peer): remote children return results and delegate onward within durable grants #380 current-target/contributor/mode/native-credential/cancellation controls, feat(peer): continue and stop remote delegated jobs after their first result #381 original pair/Stop history/newer obligation/terminal-COMMIT result/negotiated lifecycle/production outbox controls and feat(peer): transfer attachments and make remote retries preserve operation identity #382 original actor/generation/native acceptance/replay/retention controls. The assigned native-device fixture now uses the real constructor/config/filesystem with external host/process/HTTP leaves and preserves receipt-before-preparation and configuration uncertainty. Separate feat(peer): view and drive remote browsers and devices from a source-only client #383 production recheck/typed negotiation/viewer work remains outside feat(peer): surface and answer remote descendant requests from the parent #369; it is not demanded here as an unapproved additional condition.
  • Catalog, migration and performance: Dismiss is present at registration, actual presentation, summary and mutation owners. Historical additive fields remain optional where intended; required native request/subagent rosters are not replaced with production fallbacks. Shells carry compact kind information rather than request bodies. Traversals/observations remain bounded. No measured performance improvement, all-59-device-command execution, all-provider/client acceptance or complete migration/rollback acceptance is claimed. Cost remains UNKNOWN.

The historical unused export, six viewer crashes, missing dismiss presentation and missing native roster fixture failures have concrete owner repairs in the cumulative tree. Their tests pass freshly here. The registry additions assign the actual carried paths without changing fork gate policy. Parent's earlier merge topology failure and subsequent correction remain their original Git evidence; no source or approval transfer is inferred from tree equality.

Independent executed proof

All commands ran only after parent's exact-worktree dependency-ready confirmation, with PATH=/opt/homebrew/opt/node@24/bin:$PATH ./node_modules/.bin/vp. No reviewer install, dependency borrowing, env/credential link or pre-ready failed test attempt. Parent's early ERR_PNPM_NO_PKG_MANIFEST occurred before native checkout completion and is preparation evidence, not a test/product failure.

428 distinct passing tests across22files. These are scoped feedback, not the complete repository gate. node scripts/fork-features.mjs check passes with42features; git diff --check base HEAD passes. No independent type/lint/Knip or full fork rerun; I inspected the author's retained scoped logs, final94/120/Pi5 outputs and native/runtime red evidence without relabeling them as my executions.

A final live PR snapshot confirms current 42051c4 CI39116640008 SUCCESS, including lint/types/build/Test/Web/all six server shards/Rust/release smoke/transfer artifact/aggregate Check, and Fork39116640150 SUCCESS. Replacement fingerprint runs are successful; an earlier cancelled fingerprint and policy-skipped ancillary jobs remain distinct. pr-final.json retains the rollup. This is observed CI, not my execution or final acceptance, and it does not diagnose old failures.

Retained failure and proof limits

I inspected /tmp/chromeria-369-r1-workflow-full.log: the author's 93 pass/1 fail of94,312.33s, reports Native initialization failed: 404 in round5: newly joint-created work remains usable after both creators narrow, link retirement, with matching credential/bound loopback origins. Its response body was unavailable. Cause remains UNKNOWN. The author's distinct final94pass324.29s, this review's current runs and green exact-head CI neither erase nor explain that failure. It is not labelled flaky/environmental or equated with historical404. Earlier discovery403/404/timing/interrupted runs retain their original identities and limits too.

Registered native tests use actual authenticated finite loopback transports, native SQLite/filesystem/projection/event/command/receipt/outbox owners and deterministic external leaves. They do not establish actual web/desktop/mobile clicks/images, a physical cross-machine topology, all live provider callbacks, process death/OS crash, full upgrade/rollback or whole #379 acceptance. The mobile interaction uses the compatible installed React renderer and external native view leaves. The no-injection runtime export test proves its stated production-kernel composition, not a booted full CLI/client. Parent retains actual acceptance and both-review aggregation.

No product server, browser, device/simulator, live provider prompt/process, live userdata/parent fixture access, machine/global-tool change, extra agent, source fix, commit/push, merge, Issue closure/blocker clearing, aggregate status write, version bump, release or deployment. All finite reviewer checks completed; source stays clean and exact. No cost total was invented or published.

Requirements and who asked: Parent/user requested this new independent whole #369 R2 Sol MEDIUM exact-candidate review under frozen #379, carrying both complete R1 reports/responses and native local-equivalent authority/revision/answer guarantees.
Deleted: Historical approval transfer, narrow repair-only review, extra delegation, policy/data substitution, source edits, speculative #383 scope, broad local gates and unsupported failure/cost claims.
Bottleneck: Faithful native content/revision snapshot and explicit offered-answer semantics, followed by parent both-review/current-CI/actual acceptance decisions.
Checked myself: Exact live catalog/actual persisted Color and app/Git/PR binding; cumulative59paths and relevant consumers; same-client installed transaction semantics and no-injection runtime composition; current native/client/authority/Pi scoped outputs; retained original failures and final clean identity.

Authored by Codex / gpt-6.1-sol / MEDIUM, Codex harness. This is this reviewer's verdict only; parent owns aggregation.

What: Both required independent R2 whole-candidate reviews APPROVE exact42051c; current CI passes, while actual product acceptance remains outstanding.
Why: Both reviewers verified the repaired consistent read snapshot, offered-empty semantics and native runtime composition while rechecking the cumulative candidate.
So what: Parent records review success, then performs isolated real source-client/provider acceptance before internal integration; final379 delivery remains separate.

What: Exact #369 candidate42051c passed real source-only web and Codex peer request acceptance, including committed replay after both full CLI servers restarted.
Why: Independent tests cannot establish that the production composition, real provider callback and source-only client work together.
So what: The planner accepts this bounded internal component for integration into #379 after a fresh head/base/gate check; combined multi-machine/client acceptance and final shipment remain pending.

Requirements and who asked: User #379 mandates native delegated lifecycle and all permitted tools with two MEDIUM reviewers and real isolated client/provider proof.
Deleted: No fake provider, direct SQL mutation, injected browser RPC, capability expansion, alternate harness or duplicate callback.
Bottleneck: Current candidate integration; #383 shared device authority/discovery and #384/#385/#386 remain outstanding.
Checked myself: Primary planner drove actual source A browser, inspected native B events/provider results, restarted both CLI products, replayed exact original request, and reopened the same source-only result.

Identity: HEAD42051c47813c354682b80677009b199d86fef1a0, treef9efef2d7df1b8b91ac64b09018386e79ccc1d45, baseab1bf53640e2a6541a2ba9f316839cedf81d4f46. Clean author workspace /Users/rocky/.t3/worktrees/chromeria/feat-369-remote-pending-requests. Both full production servers ran that source, with meaningful copied task-owned history in NEW /tmp/chromeria-369-acceptance-a and -b; old382 homes/evidence remained untouched. Node24.18, A server14247/web7663, B14248, source incognito Browser connected ONLY to A950a38f2-e2a5-4242-9171-c17e0cb8412c. Actual B is ef047b58-1edf-4ad6-a1ba-44320188325c. No live userdata write, new grant, physical device or standalone worker server.

Human source-only flow: A actual Codex gpt-6.1-sol/MEDIUM delegated real async B task369-parent-native-question-v1 in existing B project/root. B actual Codex asked request_user_input_async, persisted pending at B sequence288. Primary opened source task viewer from transcript; browser URL stayed A, no B profile. The actual card displayed369 REAL REMOTE QUESTION, ACCEPT_369/DECLINE_369 and disabled Send answer until selection. Primary selected ACCEPT_369 and sent once. B durable request resolved at sequence290, command mcp:request:385bcb2f7e0a7415a3c0d1cda8a0cabb0b7204f83949b505a9f4fe4cb6482210:0, 2026-10-18T10:10:47.347Z. Real provider consumed the answer and completed369_REMOTE_ANSWER=ACCEPT_369. Source returned exact original result and zero pending requests. Not inferred from dispatch acceptance.

Agent tool flow: Separate deliberate B task369-parent-native-agent-answer-v1, same explicit Color. Native delegate mode wait returned on real pending question with waitTimedOut:false, requestCount1, actual qualified target/revision334. Source local list{} correctly returned[]; explicit remote descendant list returned its actual request. Native qualified read returned real369 REAL AGENT QUESTION/options/revision334. Source called t3_pending_request_respond with original target/route/expectedRevision334, answers{0:AGENT_ACCEPT_369}, stable key369-parent-agent-response-v1. Initial AND exact retry both returned sequence340. B request has exactly one pending334 and one resolved336 event; real provider completed369_AGENT_RESULT=AGENT_ACCEPT_369. Two B tasks intentionally created, each ordinal1 completed, no duplicate B run.

Restart proof: Read-only query found zero preparing/queued/starting/running/waiting runs in A/B. Primary stopped only captured own sessions47643/49402 (exit130), restarted same source and same NEW homes in sessions86505/9639. A actual Codex ordinal34 called ONLY the exact original respond input/key after reconstruction: sequence340 again. Postrestart B has no additional request resolution or provider run. Source previous-agent viewer reopened after both restarts with original human result, real accepted answer and zero pending requests. Actual client screenshot inspected before upload.

Evidence: local immutable/raw read-only oracles /tmp/chromeria-369-parent-actual-question-proof.json, /tmp/chromeria-369-parent-native-agent-tool-proof.json, /tmp/chromeria-369-parent-postrestart-proof.json. Full CLI logs /tmp/chromeria-369-acceptance-{a,b}-server.log and -restart.log retained. Native Browser screenshots in existing UNPUBLISHED draft evidence-379, never a public release:

Neither review substitutes for the actual proof above. Full author/R1/R2 packet #369 (comment).

Limits: These are TWO isolated Rocky environments, web/source-only browser and actual Codex provider. No physical cross-machine/tunnel/mobile/native desktop/all-provider/real live approval, durable-dismiss or secret acceptance claim. Registered native tests retain their own broader and S1/S2 race/empty-choice proof. Earlier native initialization404 remains UNKNOWN; later tests/CI/actual positives do not diagnose it. Operational verification earlier used invalid exact=true locator syntax and a stale assumed web port; corrected to supported selectors/actual dev-runner7663 without alternate browser or product changes. Cost supported publish #416 (comment) is partial task379 coverage, not complete cost; old unplaced/unpriced usage remains unknown. Background supported sync still pending at this record, not presumed complete.

States: implemented YES; current focused/native tests YES under their exact records; BOTH reviewed YES; current CI YES; bounded primary product proof YES; internal integration NOT YET at this record; main merged/build/shipped/activated/live combined acceptance NO. No package bump. All rollback/evidence/workspaces retained under explicit #379 instruction until final acceptance.

Why: BOTH new R2 MEDIUM reviews, authenticated current review status, current full CI/Fork/fingerprint and primary real source-only web/Codex/native replay acceptance passed.
So what: Planner carries this component into #379 and sequences #383 shared ownership/base alignment next; Issue369 stays OPEN until combined final outcome delivery.

**What**: Exact #369 candidate42051c passed real source-only web and Codex peer request acceptance, including committed replay after both full CLI servers restarted.
**Why**: Independent tests cannot establish that the production composition, real provider callback and source-only client work together.
**So what**: The planner accepts this bounded internal component for integration into #379 after a fresh head/base/gate check; combined multi-machine/client acceptance and final shipment remain pending.

Requirements and who asked: User #379 mandates native delegated lifecycle and all permitted tools with two MEDIUM reviewers and real isolated client/provider proof.
Deleted: No fake provider, direct SQL mutation, injected browser RPC, capability expansion, alternate harness or duplicate callback.
Bottleneck: Current candidate integration; #383 shared device authority/discovery and #384/#385/#386 remain outstanding.
Checked myself: Primary planner drove actual source A browser, inspected native B events/provider results, restarted both CLI products, replayed exact original request, and reopened the same source-only result.

A keyed run_scheduled_task_now whose dispatch failed with a typed error and
left no native receipt returned outcome_unknown and kept the schedule marked
running forever. Record it as the request's failed outcome, release the
running reservation, and replay the same failure for the same key.
Interrupted or defective dispatches stay outcome_unknown.

* fix(orchestration): accept blank optional answers in remote request admission

Remote pending-request answers with expectedRevision rejected a blank or
omitted optional choice-only question as an unoffered option. Skip blank
optional answers before validating options, matching the message-response
path.

Co-Authored-By: Claude Opus 5.5 <noreply@anthropic.com>

* chore(fork): keep one owner for server shard weights

#581 registered shardWeights.json under run-groups while the integration
already owns it under fork-maintenance; fork-features requires one owner.

Co-Authored-By: Claude Opus 5.5 <noreply@anthropic.com>

* test(server): weight typed peer effects by their CI time

PeerAuthority.typedEffects.workflow.test.ts takes about 197s in a CI shard
but was weighted 46.4s, so Test Server 8 exceeded its 10-minute limit.

Co-Authored-By: Claude Opus 5.5 <noreply@anthropic.com>

* fix(peer): reopened parents follow their remote tasks again

Archiving a parent ended its remote follower, and nothing restarted it on
unarchive, so the reopened parent missed later remote results until a
server restart. The follower now stays registered and waits for the
parent's unarchive or deletion instead of exiting, so reopening resumes
observation without a new entry point or a duplicate follower.

Co-Authored-By: Claude Opus 5.5 <noreply@anthropic.com>

* fix(peer): threads read and answer their own questions again

The replacement pending-request service refused every "own" answerer, so a
thread reading or answering its own user-input question, or an ordinary
full-access MCP client doing so, got capability_denied. Own access is
restored for local, non-linked callers on threads not delegated from
another environment. Own approvals, dismiss, linked clients and remote
delegated children keep their existing refusals.

Co-Authored-By: Claude Opus 5.5 <noreply@anthropic.com>

* fix(web, mobile): remote request cards keep multiline editor answers

Remote pending-request cards rendered every custom answer as a single-line
field, so a multiline editor prefill displayed and was stored flattened.
Editor questions (those with an initialAnswer) now use the Textarea on web
and a multiline, newline-submitting field on mobile, as the local paths do.

Co-Authored-By: Claude Opus 5.5 <noreply@anthropic.com>

* fix(scheduled-tasks): keyed pause and resume replay their receipt

A keyed update_scheduled_task carries clientRequestId on its input, so the
enable-only shortcut never matched: a keyed pause or resume fell through to
the full definition update and was refused without a purposeIntent. The
shortcut now ignores routing fields, and setEnabled records and replays
the keyed mutation receipt like create, update and delete. A retried
keyed pause after a later resume replays instead of retiring the newer
automation certificate.

Co-Authored-By: Claude Opus 5.5 <noreply@anthropic.com>

* fix(scheduled-tasks): post-admission recheck refusals release keyed manual runs

A typed refusal from the recheck after admission, or after the dispatch
claim, now records the key as failed and frees the schedule instead of
leaving it running. The stage-guarded claim keeps uncertain dispatches
outcome_unknown.

* fix(childThreads): stop-disposed remote children stay pending background work

A parent interrupt disposes a delegated completion with disposedBy "stop"
without stopping the remote child. The sparse SQL remote arm dropped that
row while the local arm and pendingDelegatedChildTasks kept it, so shells,
auto-settlement and storage cleanup treated the parent as idle.

* fix(scheduled-tasks): final recheck refusals no longer fail a concurrent same-key dispatch

The final live recheck ran after the prepared -> dispatching claim and
terminalized a refusal from the dispatching stage. For an existing-thread
schedule a same-key retry can read dispatching and send concurrently, so
the refusal failed the request and freed the schedule while that send was
accepted. The final recheck now precedes the claim, so a refusal is only
recorded through the prepared-stage compare-and-set.

* fix(peer): finished remote jobs no longer hold back live Stops

A lifecycle-v1 follower keeps long-polling its job there after the result,
so a later Stop or follow-up is still observed. Each poll held one of the
eight shared peer permits for up to 20 s, so more than eight finished jobs
starved Stops and live observations of other jobs.

Followers whose job has nothing left to do there (result available, no
queued runs, questions or recovery wait, or a confirmed Stop) now wait on
their own semaphore. Stops, unconfirmed and acknowledged stops, live jobs
and follow-ups seen as working keep the existing eight permits.

* fix(peer): stopped remote jobs with new work there keep live permits

A confirmed Stop counted a job as settled whatever its latest snapshot
said, so fresh direct input on the destination child after the Stop
left its follower queued behind idle settled long-polls, also after a
restart.

A job is now settled only while its latest snapshot is quiet (result
available, no queued runs, questions or recovery wait); a confirmed Stop
also needs that quiet snapshot. Anything showing activity uses the
shared network permits.
