Part 2 of #22487. Sibling: #22504

Summary

A failed provider update only reported its exit code (Codex: Update command exited with code 1.), and nothing was logged on the server. The updater's output was already stored in updateState.output, but no client shows that field.

 runCommandAndVerify
   result = runMaintenanceCommand(update)
   if failed
- message = "Update command exited with code 1."
+ message = "Update command exited with code 1: <last non-empty stderr line, else stdout>"
+ logWarning("Provider update command failed", { provider, command, exitCode, timedOut, output })

The web toast, the web provider card, and mobile all render updateState.message, so no client changes are needed. The appended line uses the existing 200-character progress-line cap. Timeouts keep Update timed out.

Evidence

  • Before: [Bug]: Codex update always fails on a remote host with a root-owned system-wide Codex install, and the toast hides the reason #22487: toast <env> · Codex: Update command exited with code 1., and the server trace had no events. Runner test on main:

updater prints stdout "checking install", stderr "Error: Could not detect the Codex installation method." (exit 1)
expect(message) = "Update command exited with code 1: Error: Could not detect ..." ✗ received "Update command exited with code 1."

After: that test passes, along with a new test where a silent failure keeps Update command exited with code 1. (vp test run src/provider/providerMaintenanceRunner.test.ts: 26 passed).

Isolated dev server (web), fake standalone codex whose update prints the real Codex error to stderr and exits 1:

    Update failure toast

Server log from the same run:

    WARN Provider update command failed
{ provider: 'codex', command: 'codex update', exitCode: 1, timedOut: false,
        output: 'Error: Could not detect the Codex installation method. Please update manually: https://developers.openai.com/codex/cli/' }

Not checked: mobile rendering. Mobile shows the same updateState.message string.

Merge Danger

Door: two-way

Blast Radius: messages

Failed-update messages get longer for every provider. Any client that lays out updateState.message in a tight space may wrap or truncate it. Updater output already reached clients through updateState.output, so nothing new is exposed.

Closes #22487 (together with #22504)

The failure message only carried the exit code. It now ends with the updater's last output line, and the server logs a warning with the full output.

Part of pingdotgg#22487.

Approvability

Verdict: Approved at 6a83804

Macroscope's review found this PR approvable — This is a focused server-side fix that preserves provider update behavior while adding the updater’s final error line to failure messages and recording bounded diagnostics. The added test coverage includes both informative output and silent failures, with no defaults, schema, infrastructure, or sensitive-area changes.

📝 Walkthrough

Provider maintenance failures now include the last non-empty command output line in failure messages and emit warning logs with command details. Timeout messages remain unchanged.

Changes: Provider maintenance failure reporting

Layer / File(s) Summary
Failure messages, warning logs, and test coverage
apps/server/src/provider/providerMaintenanceRunner.ts, apps/server/src/provider/providerMaintenanceRunner.test.ts
Failure messages append the last non-empty stderr or stdout line. Failed or timed-out commands emit a warning with provider, command, exit code, timeout status, and output. Tests cover failures with and without command output.

Merge Risk: 🔵 Low · up to 6a838

Failed provider updates may expose sensitive command output in internal logs; sanitize the warning before merging.

Security architecture risk: 🔵 Low · up to 6a838

Failed provider updates now send unredacted command output to server logs and potentially configured telemetry collectors. Output was already available in provider status, and update permissions remain unchanged. The additional logging exposure warrants attention, but the change does not introduce a new command-execution capability.

Retained concerns

  • Low · security · inferred: The new warning sends unredacted updater output to console and configured telemetry paths, potentially extending sensitive-data access and retention beyond the existing provider-status channel. Truncation limits volume but does not remove secrets.
Security review details

Security Blast Radius

  • inferred — The directly evidenced exposure is output from a provider update on the executing server, plus that server's configured logging destinations. Shared collectors could broaden access, but their audience and cross-environment aggregation are unknown; no tenant-wide or fleet-wide compromise is established.

Security Findings and Attack Paths

  • inferred — The retained sensitive-data finding corresponds to updater stdout/stderr reaching a new warning-log sink without redaction. If an updater emits credentials or private installation details, log recipients may receive them. This additional sink is introduced by the PR; the underlying provider-status output exposure predates it. Evidence does not establish arbitrary unauthenticated control of updater output or a concrete leaked credential.

Trust Boundaries and Controls

  • observed — The existing WebSocket handler invokes the runner behind authenticated session handling and the provider-management scope. These controls restrict update initiation; they do not sanitize command output or establish the permissions of downstream log readers.

Resilience and Maintainability Implications

  • observed — The lifecycle retains per-instance duplicate exclusion, installation locking, fresh ownership validation before execution, scoped child-process cleanup, and terminal success/failure construction. Timeout results discard captured output. These controls contain concurrent maintenance work, but static inspection does not conclusively establish terminal-state publication after interruption during execution.

Hardening Proposals

  • proposed — Keep routine warnings to structured failure metadata, or explicitly redact provider output before logging it. If raw diagnostic output is necessary, define its authorized destinations, access policy, and retention separately from ordinary warning logs.
