This PR successfully implements client recovery, unsaved draft tracking, and complete log history navigation. The changes demonstrate solid engineering with proper handling of race conditions, cursor expiration recovery, and state fencing mechanisms.
Highlights:
- Comprehensive cursor expiration recovery with automatic retry logic in log history loading
- Proper UI state write fencing to prevent late responses from recreating deleted tickets
- Incremental log folding that preserves performance during full history walks
- Draft preservation that correctly handles in-flight saves and background refreshes
With 6,269 passing tests and focused integration coverage, the implementation is well-validated. The code follows existing patterns and includes clear documentation of complex logic.
No blocking issues found. The changes are ready to merge after the prerequisite PR #167.

4. Reordered beads show false changes 🐞 Bug ≡ Correctness
pairExpansionBeads pairs equal-length plan and expanded arrays by position instead of bead identifier, reserving identifier lookup for arrays of different lengths. If refinement preserves the bead count but reorders existing beads, the mismatched pairs feed both rendered per-bead field comparisons and the aggregate addition count.
Agent Prompt
## Issue description
Equal-length plan and expanded bead arrays are paired positionally, so reordering existing beads compares expanded beads against the wrong plan data and produces incorrect change details and aggregate counts.
## Fix Focus Areas
- src/components/workspace/ArtifactContentViewer.tsx[1694-1709]
- src/components/workspace/ArtifactContentViewer.tsx[1749-1768]
## Recommended Fix
Build the plan-bead identifier map for every input shape and match each expanded bead to a plan bead by ID whenever the IDs resolve uniquely, regardless of array lengths. Retain positional pairing only as an explicit fallback when identifier matching cannot establish correspondence, such as renamed or missing/unusable IDs, and add a test covering reordered equal-length arrays.

Preserve every valid field in full ticket patches
When an action route returns its full PublicTicket, this new allowlist drops valid fields such as lockedCouncilMemberVariants, the locked/effective Manual QA, AI-question and Git-hook settings, manualQa, manualQaOrigin, and pendingQuestions. applyTicketActionResult merges this sparse result over the cached pre-action ticket, so starting a ticket can leave the header showing the old unlocked variants/settings, and other actions can retain stale question or Manual QA state until the follow-up refetch succeeds; if that refetch is delayed or fails, the cache remains inconsistent with the successful action. Validate and copy the remaining Ticket fields rather than silently omitting them.

Review by Claude Opus 5 1. Important: a successful create leaves the New Ticket form open and ready to submit a duplicate
What is wrong: the "later edits" fence came from the edit-form pattern, but this is a create form. The inputs stay enabled while the create is in flight (only the two buttons are disabled, How it fails: the user types a title, clicks Create & Start, then fixes a typo while it runs. Ticket A is created and started, and the form stays open with the corrected title. Nothing says A exists, so the user clicks again and gets ticket B, a duplicate. Union Alpha — [P2] Reset the deferred history fold when the query scope changes HEAD 0b8c358: src/hooks/useTicketHistoricalLogs.ts:378-384 and :487-492. A full drain sets drainActiveRef, and the entries memo then returns the previous fold without checking which ticket/view owns it. Visit two log tabs so both newest pages are fresh in React Query, start Go to top on one, then switch back to the cached tab while an older page is pending. The scope-change render returns the old scope’s entries. FullLogView.tsx:447-459 cancels the old owner, but finish deliberately does not bump foldRevision for a canceled owner. With a fresh cached destination there need not be another pages change, so the memo can keep displaying the wrong history after the drain has finished; switching tickets can similarly retain the previous ticket’s rows. The hook’s request/drain promise refs are also shared across query keys. Fix: bind deferred folding and drain/request ownership to queryKey (or a scope generation), immediately fold/reset for a different scope, and ensure cancellation releases the frozen display independently of whether the old caller may publish results. Add a cached-destination scope-switch test while an older request is pending; the current FullLogView scope-switch test fetches an uncached destination, whose later response masks this case. |

Union Alpha — [P2] Do not append older live-overlay rows after the newest historical page HEAD 0b8c358: src/context/logUtils.ts:611-618, :665-668; callers src/components/workspace/FullLogView.tsx:182-185 and PhaseLogPanel.tsx:189-195. The new merge keeps every historical row first and appends every unmatched live/context row. Those incoming rows are not necessarily newer: LogContext retains up to MAX_LIVE_ROWS_PER_PHASE and getAllLogs/getLogsForPhase return their entire retained windows (LogContext.tsx:164-175, :351-363), while a newly selected historical filter starts with only 20 rows. After receiving more than 20 system events, switch to SYS: the server supplies the newest 20, but the older context-only system rows are appended after them. The displayed tail and Go to bottom now point at older history; phase/bead grouping can also run backwards. DEBUG has the same problem, and AI compares rows without server mirror keys as equal rather than locating them chronologically. Fix: merge the two windows with an explicit total ordering/boundary rule that preserves historical file order for non-AI views while placing older overlay-only rows before that window (or exclude overlay-only rows older than the loaded historical boundary until paged). Test a live window larger than the initial historical page, not only an empty overlay or one new row. |
|
Union Alpha — [P2] Move the retained project draft into edit mode after create succeeds HEAD 0b8c358: src/components/project/ProjectForm.tsx:343-355. Create a project, then change its name/color/setting while the POST is pending. The new success handler correctly keeps the form open, but ignores the returned Project and leaves isEditing false (derived solely from the original project prop at :98). useCreateProject invalidates the project list (useProjects.ts:199-205); when that refetch includes the newly created project, duplicateShortnameProject/hasProjectIdentityConflict (:156-160) matches this draft’s own shortname and disables Create Project (:924). The later edits are now stranded: no update path targets the project just created. TicketForm has the analogous identity loss at :148-152: submitting its retained draft creates a second ticket instead of saving the later edits to the first. Fix: retain the created entity identity and transition the still-open draft to an update flow (or prevent editing during creation if that is the intended UX). Extend the existing keep-open test through the real list invalidation and a second save, verifying that later edits reach the already-created entity rather than a duplicate/conflict. |

Union Alpha — follow-up review HEAD: b17ae32; actual merge-base: b5f3b47. P2 — Older live-only rows still move behind newest non-AI historyAt HEAD Preserve server-relative order while placing unmatched overlay rows against a durable ordering boundary (or exclude already-persisted rows outside the loaded window), rather than blindly appending them. Add FullLogView/PhaseLogPanel coverage with an older retained overlay row, a newer limited historical page, and subsequent older-page loading; assert phase/bead placement without re-sorting ordinal-ordered history. P2 — Reconnecting without a cursor cannot trigger the required recoveryAt HEAD When reconnecting with no usable cursor, either request the server-supported zero cursor so replay-gap recovery runs, or explicitly arm recovery for that reconnect. Add a test that loses a cursorless connection, completes the onerror snapshots, changes history/artifacts during the outage, and verifies the reconnect recovers those families; retain the no-extra-refetch behavior when a nonzero cursor really bridges the gap. Findings are source-confirmed; no runtime reproduction was performed. P2 — Reset all leaves the active dirty prompt unchangedAt HEAD Give the editor an explicit, request-scoped reset notification and adopt the returned/refetched default for the draft that the user agreed to discard; distinguish edits typed after reset began. Add integrated PromptsDialog coverage for a dirty modified prompt, successful Reset all, default text and clean state, plus a failed reset and edits made while reset is pending. P2 — Browse opens the folder picker behind the Projects modalAt HEAD Define a nested-modal layer above routed dialogs (or an explicit stacking contract for FullScreenModal callers) while retaining the dashboard fix. Add integrated Projects → Browse coverage that checks the effective layer ordering, picker focus, and restoration of the parent after dismissal. P2 — A missing-session scan can advance past unindexed native rowsAt HEAD Bound each read to the captured file extent, or keep the previously indexed shared cursor when scanning only missing sessions and separately ingest the concurrent suffix for all scanned sessions. Add a reader/projection integration test that enumerates an unchanged file, appends a row for an already indexed session during the new-session scan, then refreshes/exports and asserts that row appears exactly once. |

Missing Native Index Migration
Existing project databases already contain execution_log_native_index_files without prefix_hash, and CREATE TABLE IF NOT EXISTS does not add columns to an existing table. On the first historical DEBUG read after an upgrade, native ingestion unconditionally selects and writes this missing column. SQLite therefore raises a missing-column error, breaking native history, navigation, and exports until the database is rebuilt. Add an explicit migration or compatible backfill for prefix_hash.

Partial Tail Escapes Verification
When a native log ends with an unterminated line, that partial row is inserted into the new generation but excluded from indexedHash. The final candidate check now also ignores mtime changes and accepts a same-identity file whose size remains at least the captured size. If that tail is rewritten during indexing, the rewrite can pass validation and publish stale forensic log content. The tail bytes must either be covered by a verification digest or remain uncommitted until the line is terminated.

6. Fallback drafts remain stuck saving 🐞 Bug ☼ Reliability
flushTicketUiStateSnapshot marks the cached draft as flushPending but does not clear that state or emit a success event when sendBeacon accepts the request. On remount, the retained pending snapshot bypasses normal autosaving and completed reads deliberately preserve it, so the editor cannot confirm or retry the draft even if the beacon request later fails server-side.
Agent Prompt
## Issue description
An accepted beacon has no response callback, leaving its cached draft permanently marked as pending and suppressing the normal autosave path after remount.

- src/components/workspace/approvalHooks.ts[476-530]
- src/hooks/useTickets.ts[894-932]
## Recommended Fix
Represent beacon delivery as unconfirmed rather than permanently pending. On remount, reconcile it with the server and schedule a revision-fenced retry when the server does not contain the same snapshot, while retaining the local draft until confirmation.

7. Long rewinds overwrite newer plans 🐞 Bug ≡ Correctness
handlePutExecutionSetupPlan holds an expiring planning claim across runtime shutdown but neither renews the claim after that await nor uses the content hash when performing the subsequent save. If shutdown outlasts the lease and another request takes ownership, the original request can pass its baseline check before the newer save and then unconditionally overwrite that newer plan.
Agent Prompt

Runtime-rewind saves can continue after their planning lease expires because the post-shutdown path checks only the document baseline and then disables the save content-hash precondition.

8. Malformed responses close open tickets 🐞 Bug ≡ Correctness
normalizeTicketListResponse now catches an invalid entry and silently omits it instead of failing the list query. When the omitted entry is the selected ticket, App interprets its absence from the successful list as deletion and closes the dashboard and repairs its URL.
Agent Prompt
Issue description
A malformed element in an otherwise successful ticket-list response is skipped, which makes the selected ticket look deleted and closes its dashboard.
Fix Focus Areas
- src/lib/ticketNormalization.ts[397-407]
- src/App.tsx[294-299]
Recommended Fix
Do not represent an incomplete ticket list as an authoritative deletion signal. Either fail the list normalization when any entry is malformed, or carry an incomplete-response indicator and suppress selected-ticket deletion reconciliation until a complete list response arrives.

9. Upgrades can wedge artifact writes 🐞 Bug ☼ Reliability
tryAcquire opens every existing ${filePath}.lock as SQLite and rethrows non-contention errors
without recognizing the previous JSON token format. When an upgrade follows a crash that left a
legacy lock behind, atomic appends using that path encounter a database-format error until the file
is manually removed.
Agent Prompt
## Issue description
Existing lock paths from the previous JSON token implementation are opened as SQLite databases without migration, so a stale lock left by a crashed pre-upgrade process can permanently block lock-protected atomic appends after upgrade.
## Fix Focus Areas
- server/io/fileLock.ts[40-53]
- server/io/atomicAppend.ts[40-48]
## Recommended Fix
When opening the SQLite lock fails because the file is not a database, safely confirm that it uses the previous JSON token format and is stale, then atomically migrate or remove only that confirmed legacy lock before retrying acquisition. Preserve normal SQLite contention handling and continue propagating errors for unknown or malformed files. Add an upgrade regression test that creates a stale old-format lock and verifies that an atomic append succeeds.

11. Empty native logs inflate line totals 🐞 Bug ≡ Correctness
Native indexing stores text_lines as String(content ?? message ?? '').split('\n').length, which
produces one for an empty entry. The debug-page total adds this stored native count to persisted
counts, even though persisted indexing explicitly records zero lines for empty content.
Agent Prompt
Issue description
An empty native log entry contributes one text line to debug-history totals, unlike an equivalent persisted entry.
Fix Focus Areas
- server/log/projection.ts[891-915]
- server/log/projection.ts[1496-1511]
Recommended Fix
Compute native `text_lines` from a normalized content string and store zero when it is empty, matching persisted projection indexing. Add coverage for an empty native `content` and `message` entry in debug totals.

Integrate the reviewed client-history and native-log protections onto the security-boundaries mainline. Fence ticket UI-state writes across deletion epochs, keep creation edits attached to the created item, recover historical log pages safely, and verify native tails and model-filtered sessions before publishing them. This linear commit is based on the merged security-boundaries tree so the PR contains only the client-history changes. It preserves the existing public-origin, SSE-capacity, process-ownership, cleanup, and workflow-recovery contracts.

Clarify the deletion-race wording.
“Ticket drafts and log history now survive deletion races” can imply that deleted drafts remain available. The deletion path instead fences late writes and clears ticket-keyed state. Reword this to say that deletion races no longer restore stale drafts or log history.

Make the broken-link fixture Windows-safe.
symlinkSync(...) runs before the assertion. On Windows, it can throw EPERM without Developer Mode or elevated rights, so the test may fail during setup. If Windows is a supported test target, skip this case on win32 or use a Windows-compatible fixture.

Do not update draftSnapshotRef during render.
If React discards a concurrent render, this ref can retain a snapshot that never committed. The git-check callback and create/update completion callbacks read the ref without a commit or generation guard. They can therefore compute incorrect dirtiness, set the wrong baseline, or close the form based on an uncommitted snapshot.
Update the ref after commit with useLayoutEffect.

Re-baseline project hydration without mutating refs during render.
When projects first load, draftSnapshot changes from the initial empty projectId to the default project ID. isDirty is computed before the hydration effect updates ticketBaselineRef, so the dirty-reporting effect can publish true after the hydration effect publishes false. No state update triggers a recomputation, and an untouched form can remain dirty.
Do not move the re-baseline into render. A discarded concurrent render can leave ticketBaselineRef or draftSnapshotRef containing values that never committed. Derive the pending hydrated baseline during render without mutating refs, then commit the baseline in a layout effect and trigger a consistent recomputation. Update draftSnapshotRef in a commit-phase effect as well.

Synchronize ticket refs after commit.
The render-phase assignments can survive an abandoned concurrent render. A stable upsertRequest callback can then read uncommitted ticketsByIdRef.current metadata and store incorrect request metadata.
The recovery poll captures activeTicketIdsRef.current, then resolveSnapshotApplication reads it again when the response arrives. An abandoned render can remove a ticket from that ref, causing the membership guard to reject valid snapshot recovery. The lifecycle cleanup effect does not prevent this because it runs only after a committed activeTicketKey change.
Update both refs in a commit-phase hook, preferably a useLayoutEffect declared before the recovery effects, or pass committed values through callback dependencies.

Nullable boolean overrides drop an explicit null.
manualQaOverride and aiQuestionsOverride are typed boolean | null in Ticket. copyPatchBoolean copies only true/false, so a patch that clears an override with null is discarded. The cached true/false then survives the merge, and the UI keeps showing an override that no longer exists. The adjacent fields handle this correctly: aiQuestionWindowOverride uses copyPatchNullableNumber (Line 638) and lockedManualQaEnabled accepts null explicitly (Line 673).
Add a nullable-boolean copier and use it for the two override fields.
