refactor(client): client HTTP boundary and ticket normalisation (PR-07) - #141
Conversation
PR-07 §7.1, §7.3, §7.4, §7.9, §7.12. WHAT - `failedResponseError` is now the client's only reader of a failed response body, and `throwIfNotOk(res, summary)` is the one-line form every call site uses. The three competing parsers are gone: `parseErrorBody` in useTickets (category + detail, no status), `parseResponse` in useManualQA (`error` or `message`, no status, degrading to a generic fallback on a non-JSON body), and `readError` in usePrompts. So are the inline `err.json()` branches in useProfile, useProjects and useOpenCodeModels. - `src/lib/apiPaths.ts` adds `apiTicketPath` / `apiProjectPath`, which encode every segment. Every request naming a ticket, project, phase, bead, Manual QA version or evidence file now goes through them. - Query functions take React Query's `signal`: tickets, ticket, interview, ticket UI state, artifacts, Manual QA index and round, projects, worktree size, prompts, profile, startup status, update status, and both model catalogs. `useOpenCodeModels` already had a 5s deadline of its own, so the two are combined with `AbortSignal.any` rather than one replacing the other. - `MODEL_FETCH_RETRY_COUNT` / `MODEL_FETCH_RETRY_DELAY_MS` replace the borrowed `OPENCODE_RETRY_COUNT` and `SSE_RECONNECT_DELAY_MS` at the model catalog's call sites. Same numbers; `OPENCODE_RETRY_COUNT` had no other reader and is removed. - `useSavePrompt` no longer invalidates the catalog and the prompt after a save the server rejected (§7.12), and reads its error description from a clone so the structured `errors` payload still reaches `PromptEditor`. WHY The three parsers produced three different sentences for the same class of failure and two of them dropped the status entirely, so "Ticket is locked" and "the daemon is down" were indistinguishable in the UI. The encoding gap was worse: ticket ids are `<projectId>:<shortname>-<n>` and come from the project's own tracker, so an id containing `/` addressed a different route and one containing `?` truncated the path — and half the call sites encoded while half did not, which is the rot a hand-maintained list always reaches. DECISIONS - `encodeURIComponent`, not a path-segment-only encoder. Every real ticket id carries a colon, so request URLs change from `/api/tickets/1:T-1/...` to `/api/tickets/1%3AT-1/...`. Verified against Hono: it decodes path params, and an encoded slash stays inside its segment rather than splitting the route, which is the whole point. - A body can be read once, so `failedResponseError` is the sole reader. The one caller that needs the body for something else (`useSavePrompt`) clones before reading rather than describing an already-consumed response. - `details` is quoted only when it is a string. Some project routes put a Zod field map there, which the old join printed as `[object Object]`. IMPACT Mutation toasts and banners gain `(HTTP nnn: …)`. This is on the plan's signed-off observable-change list (§7.1). No API route, response body, SSE event name or `doctor --json` check name changes. Tests that keyed on a raw ticket-id URL now encode it; the assertions themselves are unchanged. Co-Authored-By: Claude Opus 5 <noreply@anthropic.com>
PR-07 §7.2 and §7.10, plus the `/api/files/:ticketId/:file` sites the §7.3 sweep missed. WHAT Non-2xx now throws, and `[]` / `''` is reserved for a successful empty answer: - `useTicketPhaseAttempts` — threw away the status and the payload shape. A non-array is now a failure too. - `CodingView.fetchTicketBeads`, and the read-only approval view's PRD and beads fetches (`return ''` on `!ok`). - `PhaseArtifactsPanel`'s bead-commit metadata, converted from an effect that wrote state to a query, because the effect blanked the labels it had already collected when a later request failed. - `LogContext.requestServerLogs` — a 500 was read as "no rows for this scope", written into the module log cache and added to the loaded-scope set, so every later request for that scope was served the empty snapshot instead of retrying. And each consumer that used to render emptiness now renders the failure: `QueryErrorNotice` (a compact sibling of `DataUnavailableBanner`) in the three approval navigators, `BeadDiffViewer`, `CodingView`'s bead rail, the read-only approval attempt view, and `PrdApprovalPane`. §7.10: `useInterviewQuestions(ticketId, options?)` takes an `enabled`, defaulting to `Boolean(ticketId)`. `ReadOnlyApprovalAttemptView` passes `artifactType === 'interview' && !archivedAttempt`; the three interview surfaces are unchanged. §7.3 follow-up: `apiFilePath` covers `/api/files/:ticketId/:file`, which the first commit missed because it is not under `/api/tickets`. Five call sites. `PrdApprovalPane`'s save also read the body before checking `ok`, so a non-JSON error body threw inside `res.json()` before the status was ever consulted. WHY "The beads outline will appear once the artifact is ready" was printed for a 503 as well as for a phase that had not run, so a broken daemon read as work in progress and nobody retried. The log cache was worse than silent: it recorded the failure as a loaded empty result, so the retry never happened even on demand. DECISIONS - `approvalHooks.flushTicketUiStateSnapshot` is deliberately left alone. §7.2 lists it, but it is a `pagehide`/`beforeunload` keepalive write: there is no UI left to show an error to, and its boolean return means "dispatched", which is the only thing a caller can act on. - `PhaseArtifactsPanel`'s metadata query keeps `QUERY_STALE_TIME_5M` and is enabled only while the bead-commits artifact is open, matching the effect it replaces. - `CodingView`'s bead query keeps its runtime `placeholderData`, so a failed refresh still shows the last known beads — the notice says as much rather than blanking the rail. IMPACT §7.2 is on the plan's signed-off observable-change list: errors now surface where emptiness rendered. No API route, response body, SSE event name or `doctor --json` check name changes. PR-07 §7.5, §7.6, §7.7, §7.8 and §7.11. WHAT `src/lib/ticketNormalization.ts` becomes the one door into the ticket cache: - `normalizeTicketResponse` / `normalizeTicketListResponse` run in the `queryFn` of `useTickets` and `useTicket`, so every consumer sees one canonical shape. - `normalizeTicketPatch` runs on `result.ticket` before `mergeTicketInCache` in `useTicketAction`, `useCancelTicket` and `useSkipInterview` — the paths that wrote the raw server shape back over the most-observed ticket. - `RawTicketResponse` records what the wire actually sends: numeric error-occurrence ids, `availableActions` as plain strings, a possibly-absent `runtime`. Typing the fetchers as `Ticket` is what let those differences live on as compensating casts in the components that read them. - The three getters and `normalizeTicketForRender` are no longer exported — nothing outside the module calls them any more. Consequences, in the order the section lists them: - §7.5.1 `normalizeRuntimeBeads` now keeps `completedAt` and `qaOrigin`. Both were on the DTO and on `TicketRuntime` and both were dropped, so a Manual QA fix bead lost its origin badge in log grouping and its completion time anywhere the dashboard-normalised ticket was read. `qaOrigin` gets a real normaliser: `ManualQaOriginCard` indexes into `sourceItems`, `evidence` and `links` unguarded, so a half-formed origin has to become "no origin" rather than a crash. The rest of the mapper was audited against `TicketRuntime` and is complete. - §7.5.4 the five `ticket as unknown as Record<string, unknown>` casts are one `asRawTicket` behind `isRecord`. - §7.5.5 `TicketDashboard` no longer re-normalises, and `DashboardHeader`, `CouncilView`, `WorkspacePhaseSummary`, `kanban/TicketCard` and `navigator/PhaseTimeline` read `ticket.runtime` / `.availableActions` / `.lockedCouncilMembers` directly. `TicketCard`'s `runtime` prop is now required, which is the new invariant stated where it can be checked. - §7.5.7 the boundary validates rather than asserts: a payload that is not a record, or carries no `id` or `status`, throws. - §7.6 the UI-state revision is recorded in `queryFn`, not `select`. Verified by reverting: the old `select` wrote it four times for two observers, the new one writes it once. - §7.7 occurrence ids are converted to strings once, here. `errorTicketSeen`'s snapshot type narrows to `string`. - §7.8 `WORKFLOW_ACTIONS` is a runtime list in `shared/workflowMeta.ts` and `WorkflowAction` is derived from it, so adding an action is still one edit. `getTicketAvailableActions` filters against it instead of casting. - §7.11 `useWorkflowMeta` returns the shared constants. Its query could not reach the network — `initialData` from those same constants, infinite `staleTime`, no mount or focus refetch, no invalidation — so `fetchWorkflowMeta` was unreachable and `isLoading` was always false. `phaseMap` and `isLoading: false` are kept so consumers do not change. DECISIONS - `normalizeTicketPatch` deliberately omits keys the response did not carry. Completing a patch would overwrite a good cached `runtime` with zeroes the moment a route answered without one. `mergeTicketInCache` therefore no longer seeds an absent cache entry from a patch; all three callers invalidate `['ticket', id]` on the next line, so it is fetched whole instead. - `getActiveErrorOccurrence` keeps its `String(...)`. The declared type is now honest, but that call is a lookup against a value that came off the wire, and narrowing a lookup is how an unrecognised stored value silently matches nothing. - `TicketCard` changes precedence: it preferred the ticket's own bead columns over `runtime`, the opposite of every other surface and of the normaliser. All of them now read `runtime`, which already falls back to those columns. - §7.5.6 names "`useSSE`'s ticket-snapshot merges". There are none: `useSSE` only calls `patchTicketStatusInCache`, which writes two status strings. Nothing to route. IMPACT §7.5 is on the plan's signed-off observable-change list: `qaOrigin` and `completedAt` start rendering. An `availableActions` value this client does not recognise now renders no button instead of one whose request it cannot build. No API route, response body, SSE event name or `doctor --json` check name changes; `/api/workflow/meta` still exists and still answers. Three tests asserting per-component fallbacks for a partial ticket were rewritten to go through the boundary, which is where that behaviour now lives. PR-07 §7.13, §7.14, §7.15 and §7.16. WHAT — §7.13, deletion `clearTicketCaches(queryClient, ticketId)` replaces the enumerated list of query families in both the ticket and the project delete paths. It matches by predicate on the ticket id appearing anywhere in the key, so the six families the list had grown to miss — phase attempts, Manual QA, AI details, ticket beads, bead diffs and every `['artifact', id, …]` — go with the rest. A family added next release escapes a list silently; it cannot escape this. `['tickets']` carries no ticket id and is left for its own caller to filter. - In-flight queries are cancelled before the keys are removed, so a response already on the wire cannot repopulate a deleted ticket. - `useDeleteTicket`'s mutationFn waits out any queued UI-state save *before* the DELETE, so a PUT does not reach the server for a row that is gone. - Project deletion enumerates `['ticket', id]` entries as well as the cached lists: a ticket that had been opened and then dropped from the list was recorded nowhere else. - The legacy `'1'` marker is gone from both `needsInputSeen` and `errorTicketSeen`. It counted as acknowledgment for *any* signature, so a reissued `projectId:SHORT-n` inherited the previous ticket's suppression. Nothing has written it for some time. - One query client: the delete path no longer calls `clearTicketArtifactsCache`, which reaches for the module global. `clearPersistedTicketLogs` already calls `clearServerLogCache` for the ticket, so it is called once and not broadened — broadening it takes every other open ticket's cache with it (PR-02). WHAT — §7.14, FolderPicker A generation counter on the picker's requests. Both were debounced, neither was cancelled, so a slow git check for the folder you left could land last and decide whether "Select This Folder" is enabled for the folder you are on. WHAT — §7.15, AIQuestionContext - The aggregate poll depends on `activeTicketKey`, not on `activeTicketIds` — a fresh `Set` on every ten-second tickets refetch, which tore the interval down and re-fetched every open question each time. The id set is read from a ref. - `opencode_question_updated` prunes on the set it carries, the same way `applyTicketSnapshot` does, so a request dropped without a `resolved` event no longer stays answerable until the 30-second poll. An update with no `requests` array is not a statement about the set and prunes nothing. - `refreshTicket` is fenced by a per-ticket generation. Its snapshot prunes, so an older response landing after a newer one deleted live questions. - `waitingTickets` is restricted to active tickets, and requests, timers, dismissals, stop receipts and refresh generations are purged when a ticket goes terminal or leaves the list. Superseded stop receipts are dropped in `applyTimer`, which keeps the current generation's — that is what stops a burst of keystrokes posting Stop twice. WHAT — §7.16, error occurrences - A missing `occurredAt` falls back to `ticket.updatedAt`, not `new Date()`. This runs during render and the list is sorted by it, so a fresh timestamp per pass reordered the list and churned its React keys. - `getActiveErrorOccurrence` prefers the unresolved occurrence while the ticket is `BLOCKED_ERROR`; the remembered id is for historical review. It was offering Retry and Continue against an error already dealt with. - `ErrorView`'s `isSetupRuntimeError` is gated on `visibleOccurrence.blockedFromStatus`, matching the surrounding copy and `explainBlockedError`, so "Edit setup plan" appears after a failure that blocked from `WAITING_EXECUTION_SETUP_APPROVAL` too. - `filterLogsWithinWindow` excludes undated lines, which were pulling every undated row in the phase into the failure window. - `useTicketAction` / `useCancelTicket` / `useSkipInterview` set `previousStatus` from the cached status on a status-only response, as `useSSE` already does, and skip the status patch entirely when the route returned a whole ticket — which carries its own `previousStatus`. VERIFICATION The FolderPicker test was checked by removing the fence and watching it fail on its own assertion. `npx vitest run` (338 files / 4039 passed), lint, typecheck and `verify:version` all clean. Co-Authored-By: Claude Opus 5 <noreply@anthropic.com>
PR-09 §9.57 and §9.58, both reassigned to this stage (PR-03 and PR-04 declined §9.57; see PR-04's DONE block). WHAT — §9.57 `installSessionWatch` wraps `fetch`, so a 401 on any ordinary request latches signed-out. The live stream is an `EventSource`, whose `onerror` carries no HTTP status at all — the browser event has none — so a stream refused with 401 is indistinguishable from a dropped connection. With every ticket terminal the list refetch is off too, so an overnight tab made no ordinary request either: it reconnected forever against an API refusing everything, with nothing on screen saying the session had run out. `probeSessionAfterStreamFailure()` asks an ordinary authenticated route once per connection, on that connection's first failure, and calls `reportSignedOut()` only for a 401. `/api/workflow/meta` is the route: it answers from constants — no database, no filesystem — and sits behind the same session middleware as everything else. (§7.11 in this PR removed its only other client caller, so it is now used for exactly this.) `/api/auth/exchange` stays excluded as before. Three outcomes are kept apart, and each has its own test: 401 latches; any other status does not; a thrown request is an unreachable daemon and says nothing about the session. One probe at a time — a reconnect storm is one question. WHAT — §9.58 `getProfileCouncil` in `src/lib/profileCouncil.ts` parses and validates the two JSON strings on the profile once, for both readers. `ProfileSetup` cast each variant with `v as string` and handed it to `<EffortBadge>`; `DashboardHeader` ran `JSON.parse` during render and cast the result to `string[]` and `Record<string, string>`. Malformed entries are now dropped rather than typed, and the header memoises the parse instead of repeating it per render. `cleanModelId` stays in `ProfileSetup`: it rewrites the keys for that screen's own state, and the header looks up by the raw member id. VERIFICATION `npx vitest run` (339 files / 4050 passed), lint, typecheck and `verify:version` all clean. Co-Authored-By: Claude Opus 5 <noreply@anthropic.com>
Found by the revert pass over this branch's new tests: each fix was reverted one
fact at a time and the assertion that caught it recorded. Twenty of twenty-two
mutations were caught by a named assertion. Two were not, and both were my
tests, not the fixes:
- §7.10's coverage sat entirely in `ApprovalView.test.tsx`, which *mocks*
`useInterviewQuestions` — so it pins that the view passes
`{ enabled: false }` for a PRD or beads attempt (confirmed by mutating that
expression), but nothing exercised the hook's own
`options?.enabled ?? Boolean(ticketId)`. These three tests close that: the
default fetches, an explicit `enabled: false` does not, and neither does an
empty ticket id.
- Removing the `waitingTickets` filter for non-active tickets changed no test.
That one is deliberate and stays: the purge effect added alongside it already
empties the state, and reverting *that* does fail ("lets go of a ticket that
has finished"). The filter guards the render between a ticket going terminal
and the effect running — a guard, not a regression fix.

Summary
This PR successfully implements a clean HTTP boundary and ticket normalization layer for the client. The changes are well-structured and demonstrate careful attention to security and data integrity.
Key Strengths
Strong Defensive Programming: The normalization functions in ticketNormalization.ts thoroughly validate incoming data, gracefully handling malformed payloads, missing fields, and type mismatches with appropriate fallbacks. The separation of wire format (RawTicketResponse) from view model (Ticket) prevents unsafe type assertions from spreading throughout the codebase.
Security-First URL Encoding: The new apiPaths.ts module systematically encodes all URL segments using encodeURIComponent, preventing path injection vulnerabilities. This is particularly important given that ticket IDs come from external issue trackers and may contain special characters like slashes, colons, or percent signs.
Comprehensive Error Handling: The fetchError.ts module properly enriches error messages with HTTP status codes and server-provided error details, making production debugging significantly easier. The defensive readErrorDetail function handles aborted requests, already-consumed bodies, and malformed JSON gracefully.
Robust Session Management: The sessionState.ts module implements fetch interception to detect expired sessions via 401 responses, with careful exclusion of the nonce exchange endpoint. The session probe mechanism handles EventSource error scenarios that don't carry HTTP status codes, preventing infinite reconnect loops.
Safe JSON Parsing: The profileCouncil.ts module validates JSON-stored profile data before use, preventing runtime crashes from malformed stored values that could reach component render functions.
Test Coverage
The PR includes comprehensive test coverage with 22 new tests, each verified by confirming failure when the fix is reverted. The verification approach (revert-test-restore) ensures tests genuinely pin the behavior they claim to protect.
Code Quality
The code is production-ready with no blocking defects identified. All functions include clear documentation explaining their purpose and behavior. Error paths are well-considered, and the normalization boundary ensures consistent data shapes throughout the application.
No critical issues found that would block merge.

**issue (broader_impact):** These ticket artifact queries still use the old non-cancellable query functions and do not expose their error state. A failed beads request leaves `fetchedBeads` undefined and the pane continues down its empty-content path, while a failed execution-setup-plan request makes `generationFailed` true and is presented as plan generation failure rather than an HTTP failure with retry; both requests also continue after their components unmount.
**Triggers:** When the beads or execution-setup-plan endpoint returns a non-2xx response, or when the user leaves the view while the request is in flight.
**Suggested fix:** Accept React Query's `{ signal }`, pass it to `fetch`, and render the query error through the same retryable error notice used by the other artifact surfaces instead of inferring emptiness or generation failure from missing data.
</issue_to_address>Sourcery assessment
Needs a human reviewer. 1 finding to address first, and this changes the authentication boundary by probing an ordinary API route after stream failures and signing the tab out on a 401, while also centralizing many ticket requests and cache writes; a mistake could leave users incorrectly signed out or cause actions and cached views to target the wrong ticket. Reverting restores the prior behavior, but incorrect authorization/session handling or an incorrectly encoded mutation request can affect access or produce actions before the revert.

Greptile SummaryThe PR centralizes the client HTTP boundary, normalizes ticket payloads, strengthens deletion cleanup, and revises AI-question snapshot ordering.
Confidence Score: 3/5The PR is not yet safe to merge because deletion can still leak an old UI-state revision into a reused ticket ID, and stale snapshots can resurrect questions removed by live events. Late UI-state callbacks remain able to undo revision cleanup after deletion, while the revised question-ordering logic applies stale snapshot upserts after authoritative SSE removals. Pull Request Overview
This PR refactors the client-side HTTP boundary and introduces a canonical ticket normalization layer. While the architectural intent is well-addressed, the current implementation is not up to standards due to high levels of code duplication and several new quality issues. Specifically, the requirement to exclude undated lines from log filtering during time-bound failure windows appears to be unaddressed in the logic. Additionally, there is significant boilerplate duplication in query cache management and artifact approval workflows that should be consolidated to improve maintainability.
About this PR
- There is systemic code duplication across the approval panes (PRD, Beads, Interview) and the query cache update logic. This increases the risk of divergent behavior when updating the approval workflow or cache handling.
- The requirement to exclude undated lines from log filtering to prevent historical log leakage into failure windows appears to be missing from the implementation. This is a potential risk for data accuracy during troubleshooting.

Suggestion: Logic for performing artifact approvals and triggering coverage gap fixes is duplicated across multiple panes (PRD, Beads, and Interview). Centralizing these operations into custom mutation hooks would improve the consistency of the approval workflow.Consider extracting the coverage gap fix and approval mutation logic from
PrdApprovalPane.tsxandApprovalView.tsxinto reusable hooks insrc/components/workspace/approvalHooks.ts.
Test suggestions
- API path builder correctly encodes identifiers containing special characters like colons and slashes.
- Ticket normalization handles payloads missing runtime objects or unrecognized workflow actions gracefully.
- Session state probe correctly latches 'signed-out' on 401 responses and ignores connectivity errors.
- Cache clearing logic removes all query families (artifacts, attempts, beads, etc.) when a ticket is deleted.
- Partial ticket patches in the cache merge only the provided keys and do not seed absent entries with defaults.
- UI state revision is recorded once per fetch rather than per observer to avoid false conflicts.
- Log filtering logic excludes undated lines when a time-bound failure window is active.
- Prompt save mutation skips refetching on validation errors to prevent clearing the user's draft.

⚪ LOW RISK
Nitpick: The pattern for iterating over the 'tickets' query cache and applying updates is duplicated across several functions in this file. Extracting this logic into a shared helper would reduce maintenance overhead and ensure unified cache update behavior.
Consider creating a helper function updateTicketsCollection(queryClient, updater) that encapsulates the setQueriesData loop, mapping logic, and change tracking, then refactor mergeTicketInCache and patchTicketStatusInCache to use it.

Clear caches when cancellation also deletes the ticket
When the Cancel dialog submits options.deleteTicket: true, the server deletes the ticket and returns only { success, ticketId }, but this success path merely invalidates three queries and never invokes the newly added clearTicketCaches. Consequently phase attempts, Manual QA data, artifacts, bead diffs, logs, and UI state remain cached, and a later ticket reusing the same ID can display the deleted ticket's data. Branch on variables.options?.deleteTicket here and perform the same removal and comprehensive cleanup as useDeleteTicket.

Review round 1. Seven of eight offline reviewers and four PR bots reported the
same thing: the §7.1 sweep covered the data hooks and stopped at the approval
panes. `fetchError.ts`'s own docstring said it was "the only reader of a failed
response body in the client", and fourteen sites said otherwise.
Converted, all to `throwIfNotOk`:
- `ApprovalView` — the beads query (which also gains `{ signal }`), save,
approve, and fix-coverage-gaps.
- `InterviewApprovalPane` — save and approve.
- `PrdApprovalPane` — approve and fix-coverage-gaps. Its *save* was already
converted in the first commit, which is what made the omission of its two
siblings in the same file visible.
- `ExecutionSetupPlanApprovalPane` — the plan query (also gains `{ signal }`),
save, regenerate and approve.
- `AIQuestionContext` — `submitToRoute`, plus the two best-effort question
fetches.
- `DashboardHeader` — the size probe and `open-path`.
- `FolderPicker` — both probes.
WHY IT MATTERS BEYOND CONSISTENCY
The old pattern was `payload.details || payload.error`, and several routes send
`details` as a Zod `flatten()` — an object. `new Error(object)` is
"[object Object]", which is what the question panel showed when an answer was
rejected and what the approval banners showed on a validation failure.
`readJsonDetail` quotes `details` only when it is a string, so those now read
`Could not send that answer (HTTP 400: Invalid question reply payload)`. Pinned
by a new test in `AIQuestionContext.test.tsx`.
`FolderPicker` is the other behavioural one: neither probe checked `ok` before
reading the body, so a 500 on the git check became a confident "not a git
repository" — the same wrong answer the generation counter was added to stop an
out-of-order response giving.
WHAT IS DELIBERATELY NOT CONVERTED
Four `!ok` checks remain, and none of them reads a failed body, so the
docstring's claim is now true as written:
- `bootstrapAuth` logs a fixed line — a refused nonce exchange is not a refused
session.
- `ProfileSetup`'s OpenCode health probe is a boolean, not a message.
- `useSavePrompt` returns the server's structured validation payload; it already
clones for the description.
- `AIQuestionContext.stopTimer` must release its stop receipt on failure, which
a throw past that line would skip.
CHANGELOG wording corrected: it claimed five readers were replaced, which was
true of the hooks layer alone.
Tests that stubbed `(!init || init.method === 'GET')` now check `!init?.method` —
passing `{ signal }` makes `init` truthy with no method.

… drew emptiness Review round 1, second half. §7.2 says "add the error branch to each consumer"; the fetchers were fixed and eight consumers were not, so a 500 still rendered as "nothing here yet". Five reviewers listed the same surfaces. - **A phase's version history.** `useTicketPhaseAttempts` throws now, but the four surfaces offering the selector destructured `data = []` and never read `isError`. A failed request therefore hid the selector *and* silently scoped artifacts and logs to the live attempt — the wrong version, with nothing saying so. `PhaseAttemptsUnavailable` (one sentence, one place, beside `PhaseAttemptSelector`) now fills that slot in `CouncilView`, `PhaseReviewView`, `ApprovalView` and `CodingView`. - **The setup-plan pane**, which was the worst of them: `generationFailed` is derived from "not loading and no plan", so a 500 rendered `FailedSetupPlanGenerationBanner` and offered Regenerate — a destructive action — for a plan that may well exist. A failed *request* is now its own state and is excluded from that condition. - **The beads approval editor**, the interview approval pane and the interview navigator: all three fell through to "No … artifact available yet". - **Both log panels.** `LogContext`'s catch says "the panel's own error surface is the historical-log query" — that surface did not exist. It does now, in `PhaseLogPanel` and `FullLogView`. - **The artifact list.** Its rows come from static phase definitions, so they render on a failed request and read as "not produced yet"; the only notice was inside a dialog nobody opens for an empty button. The notice now sits beside the list, and the dialog keeps its own — a test pins that the dialog notice and its retry still work, which is why this is an addition rather than a short-circuit. - **`PrdApprovalPane`** showed nothing when a refresh failed but older content was cached, leaving stale text editable and approvable. It now carries the "showing the last one that loaded" strip, matching `PhaseArtifactsPanel`. NOT CONVERTED, DELIBERATELY `WorkspacePhaseSummary` also reads phase attempts, but only to pick the active one for log scoping and a retry label — there is no selector and no natural slot in a one-line progress chip. A failure degrades the scope rather than making a false claim. Two tests pin the two that mattered most: a 500 on the attempts route says so instead of showing one attempt, and a 500 on the beads route says so instead of "No beads artifact available yet." Review round 1, third part. Ten findings across the ticket cache, the two
deletion paths and the normalisation boundary.
DELETION
- **The UI-state revision map outlived the ticket.** It only ever climbs — that
is what stops a stale read lowering a revision a save has moved past — so
carrying it across identities made every autosave on a reissued id claim to
replace a revision that never existed. Cleared with the other ticket-scoped
stores. Reported by three bots independently.
- **Cancel-with-delete ran none of the cleanup.** That option really deletes the
row but answers `{ success, ticketId }`, so `applyTicketActionResult` had
nothing to apply and the hook only invalidated three keys. It now takes the
same barrier and the same `clearTicketCaches` as the delete mutation.
- **The barrier was neither closed nor bounded.** Draining the queue does not
stop the debounced autosave enqueueing a *new* save while the drain awaits the
old one, and waiting on a raw fetch promise means one stalled PUT holds the
mutation open and the DELETE is never sent. A tombstone now refuses saves for
a closing ticket — checked when a save reaches the front of its queue, not
when it is enqueued — and the drain gives up after 5s. A failed DELETE
releases the tombstone.
- **Project deletion drained nothing** and found tickets only by their cached
`projectId`, so a detail query still in flight was missed — and its response
could land after the delete and reinstall the ticket. A ticket id is
`<projectId>:<externalId>` (`buildTicketRef`), so every cached key is now
searched for an id this project owns, and each is drained before the DELETE.
- **The key predicate only matched flat parts.** A future
`['artifact', { ticketId }]` would escape it silently — the rot this predicate
replaced an enumerated list to end. Object parts are searched one level down.
NORMALISATION
- **`createTicket` / `updateTicket` returned `res.json()`** typed as `Ticket`.
Both go through `normalizeTicketResponse` now.
- **A partial `runtime` in a patch replaced the whole cached runtime.**
`getTicketRuntime` always answers complete, which is right for a read and
wrong for a patch: a reply carrying a bead count alone blanked the bead list,
the PR state and the ETA. `normalizeRuntimePatch` keeps only the keys the
response named, and `mergeTicket` merges `runtime` one level down. Safe today
only because `respondWithState` sends the enriched ticket — a contract, not a
guarantee.
- **Required scalars are validated** (§7.5.7, owner's call): `externalId`,
`projectId`, `title`, `priority`, `createdAt`, `updatedAt` and
`implementationTiming` are coerced to a safe value rather than trusted. They
do not throw — only `id` and `status` do, because neither has a usable
substitute and a boundary that rejects a payload wholesale turns one drifted
field into a blank board.
- **`previousStatus`** falls back to any ticket list when the detail entry is
absent, which is the case for a board action.
- **`DraftView`** was the third and last surface parsing the stored council
roster with a cast.
Nine new tests, including the two runtime-patch cases the previous test could
not distinguish (it asserted only the field it set).
Co-Authored-By: Claude Opus 5 <noreply@anthropic.com>
Review round 1, final part. Five findings, one of them a regression this PR caused. **A historical error rendered with no logs at all.** `getTicketErrorOccurrences` answers newest-first, but `ErrorView` read the "previous" occurrence as `index - 1` — the *newer* one — so `startTime` landed after `endTime` and nothing could fall inside the window. Undated rows used to pass that filter and mask it; §7.16's exclusion of them turned it into an empty log panel. Three reviewers found it independently, and it is the clearest argument for the round: correcting one thing exposed what the old leniency was hiding. **The error view fell back to the oldest unresolved occurrence.** `[...occurrences].reverse().find(…)` on a newest-first list, with `occurrences.at(-1)` behind it — also the oldest. Pre-existing, but this PR made that path reachable: an `activeErrorOccurrenceId` naming a resolved occurrence now falls through to it while blocked, so Retry and Continue would have been offered beside the wrong failure. **The session probe fired on every reconnect.** `sessionProbedRef` was cleared per `EventSource`, and `scheduleReconnect` builds a new one every three seconds, so a daemon that was merely down was asked about the session indefinitely — the plan said "on the first reconnect failure". The latch now belongs to the subscription: cleared when the ticket changes, and when a stream actually opens, because an open stream proves the session is good and the next failure deserves a fresh question. **Aggregate question polls could apply out of order.** `refreshTicket` was fenced; the 30-second poll was not, and its snapshot prunes. A slow poll landing after a faster later one — or after a per-ticket refresh — deleted questions that were still live. Both now draw from one token sequence, so they can be ordered against each other: a refresh issued after a poll started wins for its ticket. The per-ticket token is monotonic too, because pruning the map when a ticket left the active set would otherwise let the counter restart at 1 and revive a stale response. **A rejected timer frame still moved the shared clock offset**, so the countdown jumped on every open ticket. The freshness decision is made from a small ref mirror rather than inside the `setTimers` updater — an updater has to stay pure, which is the same rule §7.6 fixed for `select`. All five are pinned, and each was checked by reverting the fix and watching the named assertion fail — ten mutations across this round's three commits, ten caught. Do not mark failed Git checks as invalid repositories.
Line 90 converts every request failure into an invalid Git verdict. A 500 response or network failure then shows the “not a git repository” state and disables selection for a path that may be valid. Keep the status neutral or add a distinct error state. Set invalid only after a successful response with d.isGit === false.

Do not fall back to live content when attempt history is unavailable.
When the initial phase-attempt request fails, each view receives attempts = []. It then derives no archived attempt and renders artifacts and logs in live mode. The notice makes the failure visible, but it still displays content from a possibly wrong phase version.
When isAttemptsError is true and no cached attempt data exists, suppress version-dependent artifact and log content until retry succeeds. Preserve rendering only when a previously loaded attempt selection remains available.

CRITICAL: useDeleteProject missing error handler for closingTickets
settleTicketUiStateSavesForDelete adds every project ticket to closingTickets, but if the DELETE request fails there is no releaseClosingTicket fallback. All those tickets remain permanently blocked from saving UI state until the page is refreshed. Compare with useDeleteTicket (line 744–754), which wraps the delete in try/catch and releases the ticket on error.

SUGGESTION: Bead-commit metadata query errors are silent
This useQuery does not destructure isError, error, or refetch, so a failed fetch silently degrades labels to bare ids. The surrounding file now uses QueryErrorNotice for every other artifact surface; adding it here would keep the error-boundary contract consistent.

Review round 2 (three offline reviews + five bots). Most of it is round 1's own
fixes applied to only half of what they were written for — the same shape as
round 1's finding about the original PR.
MIRROR DEFECTS OF ROUND 1
- **`useDeleteProject` never released the write barrier.** Five reviewers.
`settleTicketUiStateSaves` tombstones each ticket *before* the DELETE, and the
two ticket paths release it in a `catch`. This one had no catch, so a single
refused project delete left every ticket in that project unable to persist
panel state for the life of the tab.
- **The bounded drain was a hole as well as a fix.** Timing out and then
clearing the tombstone on success let a save queued behind the stalled one
find the door open again. Saves are now stamped when enqueued and refused if
the stamp predates the deletion — which survives the tombstone being lifted,
while a save made *after* one (a reissued id, or a failed delete) carries a
higher stamp and still goes through.
- **Snapshot ordering was one-directional.** Round 1 stopped a poll clobbering a
newer refresh; the reverse was open, and neither was ordered against SSE at
all. One never-restarting sequence and one "last applied per ticket" map now
express the whole rule: a snapshot applies to a ticket only when nothing newer
already has. A live event claims the sequence, so a response that read the
server before it arrived cannot prune what it just told us.
- **`normalizeTicketPatch` still trusted malformed fields.** The raw spread
copies the payload wholesale and the conditional keys only ever *add*, so a
numeric `status` (which reaches `startsWith`) and a non-object `runtime`
survived. Invalid keys are removed after the spread.
- **`collectCachedProjectTicketIds`** still searched only flat key parts while
`clearTicketCaches` searched nested ones — one shared `collectQueryKeyStrings`.
FAILED QUERIES NOW GATE, NOT JUST ANNOTATE (owner's call)
- Coverage gaps live in the artifacts, so a failed artifact request made
`coverageWarning` absent — indistinguishable from "no gaps". Both approval
panes now say so and refuse to approve until it is known.
- The council and phase-review screens stop rendering version-scoped artifacts
and logs when the attempt history failed with nothing cached; the notice above
them says why. `ApprovalView` deliberately keeps rendering: its default is the
live attempt, which is correct, and blanking an approval screen because its
*history* failed would block work for an unrelated reason.
- Stale-refresh strips ("showing the last one that loaded") extended from the
PRD pane to the beads, interview and setup-plan panes.
DEDUPLICATION (owner's call)
SonarCloud's gate moved to ERROR at 3.7% new-code duplication, and it is this
PR's doing: routing the panes through one error helper made previously-divergent
blocks identical, and round 1's `isError` destructure enlarged a fourth. Both
groups are now extracted — `useSelectedPhaseAttempt` (the query, the manual
selection, the fallback and the log scope, shared by three surfaces) and
`approveArtifact` / `fixCoverageGaps` in `approvalHooks`. Codacy asked for the
second one in round 1 and I deferred it; it is the same finding.
CONSISTENCY
`ProjectForm`'s git check reads a failed body as a verdict (as `FolderPicker`
did); `refreshOpenCodeModelsQuery` forwards the query signal;
`clearTicketArtifactsCache` takes the caller's client rather than the module
singleton; phase-attempt records are validated instead of cast.
Six mutations run; two were not caught and both were my tests, not the fixes —
the approve-gating assertions passed for the wrong reason (an empty bead list
and a missing content hash also disable the button) and nothing covered the
drain race at all. Both are fixed and re-verified: eight of eight now caught.

Gate approval content when the phase version is unknown.
When the attempt query fails with no cached attempts, this code still renders an editable live approval pane. PhaseAttemptsUnavailable only adds a notice. It does not prevent saves or approvals against an unknown phase version.
Read isPhaseVersionUnknown from useSelectedPhaseAttempt. When it is true, render the unavailable state instead of the approval pane. This preserves read-only and approval gating until the attempt query succeeds.

Preserve the archived attempt number for snapshot logs.
When the only returned attempt is archived, useSelectedPhaseAttempt sets logMode to 'snapshot' but omits phaseAttempt. PhaseLogPanel then sends an unscoped historical-log request, and queryLogPage applies no phase_attempt filter. The snapshot may include logs from other attempts.

Reject saves that were queued while deletion was active.
A save queued after settleTicketUiStateSaves stamps the ticket has a higher sequence. If it waits behind a stalled save until the drain times out, successful deletion clears closingTickets before it reaches line 504. The save then sends a UI-state PUT for the deleted ticket. If the ID has been reissued, it can write old UI state into the new ticket.
Capture whether the ticket was closing when this save was enqueued. Reject that save when it reaches the queue front. Add a regression test for a save enqueued after deletion starts and before the stalled predecessor settles.

…rth copy Review round 3, plus my own re-check of round 2. Two of the three are defects in round 2's own work. A COMMENT THAT RENDERED CodeRabbit caught it: round 2 put a `//` comment among JSX children in `CouncilView`. That is *text*, not a comment, and React paints it — four lines of internal note above the artifact panel. Nothing caught it because every assertion in that suite looks for text that should be present and none for text that should not. There is now a test that fails if any `// ` appears in the rendered output, and a sweep confirmed the other file I edited the same way put its comment in a JS block, not JSX. AN OVER-CORRECTED GUARD Greptile: round 2 made a live SSE event claim the snapshot sequence so an in-flight snapshot could not prune what the event had just delivered. That was too strong. `opencode_question` carries *one* request, not the step's whole set, so rejecting the snapshot outright also discarded every other question it had learned about — until the next 30-second poll. The two kinds of supersession are now kept apart. A newer *snapshot* replaces an older one outright: it read the server later and its pending set is the better one. A live *event* only withdraws the older snapshot's right to prune; its upserts still apply. Pinned by a test that holds a refresh open, delivers a different question over SSE, and then releases the refresh — both questions have to survive. THE FOURTH COPY My own re-check. Round 2 extracted `useSelectedPhaseAttempt` and adopted it in three surfaces; `CodingView` kept its own copy — the same "fixed the rule in one place, not every place" failure that round 2 was mostly about. It is on the hook now. Doing that surfaced two real differences the copies had accumulated: `CodingView` reset the manual selection when the phase changed and the others did not (it is the one whose phase changes within a mount, so it is the one that noticed), and `ApprovalView` derived `logPhaseAttempt` from a different variable than its siblings. Both are settled one way in the hook. `WorkspacePhaseSummary` keeps its own two-line helper: it wants the active attempt and has no selector, which is a different need rather than a copy. Also: the edit toolbar — mode tabs, autosave indicator, Save — was a fourth duplicated block SonarCloud named, carried by all four approval panes. One `ApprovalEditToolbar`, 150 lines removed. And `InterviewApprovalNavigator`'s outline is now its own component, which settles Sonar's one remaining code smell (cognitive complexity 20 against a limit of 15) that round 1's error branch had pushed over. SONARCLOUD Its last analysis is `a3f09c6f`, before round 2's deduplication, so the 3.7% figure the API still reports predates the fix and its checks did not run on the newer commits. The four block groups it named by file and line are all gone now. Three mutations run; all three caught, including the rendered comment. Co-Authored-By: Claude Opus 5 <noreply@anthropic.com>

Late callbacks restore deleted revisions
If a UI-state fetch or save remains in flight after the bounded deletion drain completes, its callback unconditionally records the deleted ticket's revision after clearTicketUiStateRevisions has run. When that ticket ID is reused, the first autosave sends the restored revision and is rejected as a conflict, leaving the new draft unsaved.

Stale snapshots resurrect resolved questions
If a snapshot reads a pending request before an SSE resolved or authoritative updated event removes it, resolveSnapshotApplication still permits the older snapshot's upserts with prune: false. The stale response then recreates the removed request, making a resolved question answerable again and returning the ticket to a stale waiting-for-input state until another authoritative refresh.

Reject snapshots from a previous ticket lifecycle.
appliedSnapshotRef records a generation only after a snapshot response resolves. If a refresh starts, the ticket becomes inactive, and the ticket becomes active again before that response resolves, Line 399 accepts the old snapshot because no prior generation was applied. The snapshot can then restore obsolete requests or overwrite the new lifecycle state.
Capture a per-ticket lifecycle epoch when refreshTicket starts, or advance a ticket-specific snapshot barrier during inactive-ticket cleanup. Reject the response when that value no longer matches. Add a deactivate/reactivate delayed-refresh regression test.
