Summary

Git/filesystem part of the consolidated audit. Separate from release/installers in #163 and from the pending CLI, hook, workflow, bead/parser, client and security parts.

  • Make Git mutations asynchronous and bounded, preserve descendant cleanup, and handle filenames without lossy quoting or trimming.
  • Protect branch deletion with the expected commit and align merge cleanliness with the generated-file exclusions used by bead commits.
  • Preserve complete file generations during interrupted writes; block startup only when an in-progress recovery cannot safely establish ownership or completeness.
  • Handle short appends, contained paths, Manual QA file operations and cross-process locking without following unsafe links or discarding unknown data.

Relevant application documentation, status descriptions/details, changelog and scoped audit evidence are included. Published website documentation was updated separately on its required main branch.

Verification

Local checks passed on Node 26.8.2 / npm 12.0.2:

  • Full unit/client/server integration suite: 422 files, 6,056 passed and 12 platform skips.
  • Lint, application/test type checks and production build.
  • Package contents, version consistency, no-native-addon scan, script type stripping, installer metadata and redistributed-license checks.
  • Shell syntax, pinned actionlint with ShellCheck at the repository warning policy, and diff/artifact hygiene.

The isolated suite exposed an unrelated test fixture that assumed /tmp/test-project existed; it now uses the repository's canonical test-owned temporary-directory helper. No application workaround or dependency change was needed.

Fresh aggregate review accepted the complete 70-file part. Independent verification passed 419 tests across 21 files, full lint, both type checks and the aggregate diff check. The accepted source boundaries and the standalone part's caller adaptations were checked against the original base.

No E2E or full lifecycle tests were run. Native Windows/macOS behavior and physical power-loss durability were not verified locally. Local fixture tests exercise interrupted operations and real Git child-process cleanup where supported.

Review boundaries

This PR does not contain the newer CLI/process identity work, execution-hook recovery, strict bead contracts or remote-session recovery. Shared workflow changes here adapt callers to the Git/IO contracts only.

No version bump, migration, release, merge or CI waiting is part of this delivery. The owner will inspect CI and perform manual lifecycle checks.

Summary by CodeRabbit

  • Reliability

    • Improved Git operations with safer input validation, bounded timeouts, process cleanup, and protected remote-branch deletion.
    • Preserved unusual filenames, including whitespace, control characters, backslashes, and non-ASCII characters.
    • Ensured cleanup, ticket deletion, resets, and retry actions complete before reporting success.
    • Improved handling of invalid project paths with clear client-facing errors.
  • Data Safety

    • Strengthened interrupted-write recovery with durable proofs, symlink protection, containment checks, and blocked-recovery diagnostics.
    • Improved Manual QA quarantine handling, evidence durability, and cross-process locking.
    • Preserved user files during worktree cleanup and prevented unsafe overwrites.
  • Documentation

    • Updated workflow, recovery, security, and contribution guidance to reflect these safeguards.

…part validation

Select only the accepted Git42 and durable-IO21 source snapshots plus their reviewed documentation from c0ed98c. Preserve async mutation budgets, process-tree cleanup, opaque Git paths, safe refs and deletion leases; YAML completion proofs, interrupted recovery ownership, exact appends and native SQLite evidence locking.

This branch intentionally excludes newer CLI/process, hook, bead-contract, parser and client implementations. Its copied integration documentation will be scoped to this part before the final aggregate checks and PR. This is an assembly checkpoint, not a claim of finished-part acceptance.
Scope application guidance, changelog and audit dispositions to the accepted Git and durable-IO implementation, keeping recovery uncertainty distinct from ordinary orphan warnings and excluding unrelated release, parser, routing and process work. Move this part to its own audit ledger so it does not compete with the release ledger; the earlier seed retains the copied record.

Repair the canonical OpenCode log test fixture to use a real test-owned temporary project directory through the existing helper, rather than assuming a shared /tmp/test-project exists. No application workaround or dependency change is introduced. Full local verification passed422 files with6056 tests and12 platform skips, plus lint, both typechecks, build and package/install/license checks; fresh aggregate review independently passed419 tests and accepted the complete70file part.
…xture

Complete the accepted part checkpoint after the preceding commit recorded only the already-staged ledger relocation. Scope README, contributor guidance and changelog to the Git/filesystem changes, preserving original released history and excluding unrelated audit parts.

Use the existing canonical test-owned temporary-directory helper in the OpenCode log test rather than a nonexistent shared path. This is the exact fixture and documentation reviewed with the green6056-test isolated suite and419 independent tests; no production or dependency changes. All four paths are explicitly staged, and the part remains subject to a final clean-worktree check before PR creation.

📝 Walkthrough

Walkthrough

The pull request hardens Git execution and path handling, adds durable atomic-write recovery and SQLite locking, updates Manual QA and startup behavior, converts Git mutations and cleanup to awaited operations, and documents the resulting workflow contracts.

Changes

Git and durable I/O safety

Layer / File(s) Summary
Git execution and input validation
server/git/*
Git mutations now use bounded asynchronous execution, process-tree cleanup, safe ref validation, NUL-delimited path parsing, non-interactive SSH handling, and expected-head protection for remote branch deletion.
Workflow and cleanup integration
server/phases/*, server/storage/*, server/workflow/*, server/routes/*
Git resets, commits, squash operations, worktree removal, ticket cleanup, initialization, and retry flows now await asynchronous operations.
Durable writes and startup recovery
server/io/*, server/startup.ts, server/__tests__/*
Atomic writes create proof sidecars for selected artifacts. Recovery validates JSONL and YAML artifacts, preserves changed generations, and blocks unverifiable states.
Locking, containment, and Manual QA
server/io/fileLock.ts, server/phases/manualQa/*, server/log/*
File locks use persistent SQLite transactions. Manual QA and log writes use containment checks, no-follow symlink handling, opaque Git paths, quarantine preservation, torn-event repair, and diagnostics.
Paths, metadata, and documentation
server/storage/paths.ts, server/lib/containedPath.ts, shared/workflowMeta.ts, README.md, CHANGELOG.md, CONTRIBUTING.md, .github/*, .gitignore
Cross-platform path rules, workflow metadata, recovery guidance, audit ledgers, review dispositions, and ignore rules reflect the updated behavior.

Change: Bug fix

Merge Risk: 🟠 High · up to f4b42

This change significantly hardens Git execution, worktree cleanup, and crash-recovery behavior, but several of the new safety mechanisms still have real gaps: a prior installation's lock file could permanently block the new SQLite-based locking after upgrade, a crash during Manual QA quarantine can still lose both the source edit and its backup copy, worktree cleanup's new identity checks can theoretically mis-handle a directory replacement, and the daemon can still leave a Git/GitHub child process running past shutdown in a narrow timing window. None of these require exotic conditions to matter in production, and none have a full fix yet, so this should not be merged as fully hardened without addressing at least the lock-file upgrade path and the quarantine durability gap.

P2 Badge Migrate legacy lock files before opening SQLite

When upgrading after the previous implementation crashed while holding this lock, index.json.lock contains its JSON owner record. Opening that file as SQLite succeeds initially, but BEGIN IMMEDIATE raises ERR_SQLITE_ERROR: file is not a database; because only busy errors are handled, every subsequent evidence upload or removal for that Manual QA version fails permanently. Detect and safely migrate/remove the legacy JSON lock, or use a new database filename.

P2 Badge Preserve retries when a quarantine entry already exists

If a discard operation fails after copying one file but before writing its receipt, retrying encounters that existing quarantine entry and now throws instead of completing; the same happens when the same path drifts and is discarded again later in one Manual QA version. Since quarantine paths are keyed only by version and repository path, safely recognize/version an existing entry rather than making all later discard attempts unrecoverable.

CI failed: Test suite failures in atomicIO and manualQA integration tests due to path casing/normalization differences and test timeouts.

Overview

Across the CI runs, 2 jobs failed during the Vitest integration test suite. The failures are centered around path comparison and normalization issues in atomicIO.test.ts (affecting recoverOrphanTmpFiles) and a test timeout in manual QA operations.

Failures

atomicIO and manualQA Integration Test Failures (confidence: high)

  • Type: test
  • Affected jobs: 105021647322, 105021657649
  • Related to change: yes
  • Root cause: Path comparison mismatches (e.g., Windows short paths/case mismatch vs. full path or relative/absolute path variations) caused assertions on recoverOrphanTmpFiles to fail, and an operation test timed out.
  • Suggested fix: Normalize paths consistently using fs.realpathSync.native (or equivalent) in assertions and adjust timeouts or optimize performance-heavy manual QA operation tests.

Summary

  • Change-related failures: 2 test suite failures involving path handling and test timeouts.
  • Infrastructure/flaky failures: None.
  • Recommended action: Update the test assertions and path normalization in atomicIO.test.ts to be fully cross-platform and review the execution time of the failing manual QA operations test.

AI Description

• Bounds Git mutations, validates refs, and preserves opaque paths through delivery workflows.
• Makes interrupted-write recovery ownership-aware and fail-closed only for unresolved generations.

High-Level Assessment

The following are alternative approaches to this PR:

1. Retain tokenized lock files
  • ➕ Avoids a persistent SQLite sidecar
  • ➕ Uses only basic filesystem primitives
  • ➖ Requires unsafe stale-lock reclamation heuristics
  • ➖ Cannot reliably distinguish dead owners from slow live processes
  • ➖ Has replacement-lock races during release
2. Use a full write-ahead artifact journal
  • ➕ Could provide a uniform recovery protocol for every artifact type
  • ➕ Could centralize generation ordering and recovery diagnostics
  • ➖ Requires a broader storage architecture and migration
  • ➖ Adds transaction-log lifecycle and compaction complexity
  • ➖ Is disproportionate to the scoped durability fixes

Recommendation: Keep the PR's approach. SQLite transactions provide crash-released cross-process exclusion without stale-owner guessing, while proof and ownership sidecars add resumable recovery without replacing existing artifact formats. A full artifact journal should be considered only as a separate storage redesign.

The PR is not safe to merge until legacy container republishing is made compatible with the mandatory lockfile build and the remaining unresolved prior runtime and lock-format failures are addressed.

  3. P1 Ignored Files Still Deleted ▶

  5. P1 Windows Cleanup Never Recovers ▶
  6. P1 Runtime Shutdown Closes Other Streams ▶

Summary

The PR hardens Git subprocess handling, worktree cleanup, filesystem recovery, Manual QA persistence, path validation, and runtime shutdown. Changes made after the previous review also introduce broad release, installer, executable-resolution, and container-publication updates.

  • Git mutations use bounded asynchronous process-tree cleanup and shutdown tracking.
  • Recovery uses completeness and ownership evidence before promoting interrupted writes.
  • Automatic worktree cleanup protects ignored user files and replacement generations.
  • Installer and executable resolution paths apply conservative ownership and liveness checks.
  • The legacy container-republish path is currently incompatible with the Dockerfile’s mandatory lockfile contract.

P1 Legacy Locks Break Evidence

If an upgrade encounters an index.json.lock JSON file left by a crash under the previous locking implementation, this code opens it as SQLite and receives SQLITE_NOTADB. Only busy errors are retried, so every later Manual QA evidence add or removal for that ticket and version fails instead of reclaiming the old lock. Handle the legacy lock format before opening the path as a SQLite database.

1. Recovery can write outside its root ✓ Resolved 🐞 Bug ⛨ Security
Description
recoverOrphanTmpFiles accepts targetPath through the lexical isKnownArtifactTarget check and
later opens and promotes string paths without revalidating their ancestor directories. If an
artifact subdirectory is replaced by a symlink after scanning, recovery can read a temp file and
link or create its target through that ancestor outside the configured root.
Code

server/io/recovery.ts[798]

+        if (!isKnownArtifactTarget(canonicalRoot, targetPath, rootKind)) {
Relevance

●●● Strong

Symlink ancestor replacement undermines recovery containment and aligns with recent accepted
filesystem security hardening.

PR-#153

ⓘ Recommendations generated based on similar findings in past PRs

Evidence
The PR canonicalizes the root only once, recursively constructs ordinary pathname strings, and
accepts derived targets by a lexical relative-path allowlist. The no-follow helper explicitly
documents that it cannot pin replaceable ancestors, while promotion subsequently publishes using
those pathnames.

server/io/recovery.ts[767-800]
server/io/recovery.ts[638-704]
server/io/readFile.ts[4-24]

Agent prompt
The issue below was found during a code review. Follow the provided context and guidance below and implement a solution

Issue description
Recovery only proves that the derived target path is lexically below the startup root. A symlink substitution of a scanned ancestor can redirect the later open, link, or target creation outside that root.

Fix Focus Areas
- server/io/recovery.ts[774-807]
- server/io/recovery.ts[638-704]
- server/io/readFile.ts[4-30]

Recommended Fix
Make recovery operate only on paths whose full ancestor chain is still rooted in the canonical recovery root immediately before each destructive or publishing operation. Use descriptor-anchored/no-follow traversal where available; otherwise reject any changed, symlinked, or non-canonical ancestor before opening the temp, creating the target, linking, renaming, or deleting sidecars, and preserve the artifact when that proof cannot be made.

2. Old lock files block evidence updates 🐞 Bug ☼ Reliability
Description
tryAcquire now opens the pre-existing index.json.lock pathname as a SQLite database and rethrows
any error other than a busy-database condition. An installation that retained a JSON lock file from
the former lock implementation receives a non-database error on every Manual QA evidence update
instead of acquiring or recovering the lock.
Code

server/io/fileLock.ts[R44-53]

+    database = new Database(lockPath)
+ // A native busy wait would block this process's event loop. Failed
+ // attempts return immediately and the caller yields between retries.
+    database.pragma('busy_timeout = 0')
+    database.exec('BEGIN IMMEDIATE')
+    return database
+  } catch (error) {
+    try { database?.close() } catch { /* preserve the acquisition error */ }
+    if (isBusy(error)) return null
+    throw error
Relevance

●● Moderate

Legacy lock compatibility is plausible, but migration expectations and historical team behavior are
unclear.

ⓘ Recommendations generated based on similar findings in past PRs

Evidence
The new implementation uses new Database(lockPath) on the persistent lock filename and only
retries SQLite busy errors. Manual QA continues to derive that filename as index.json.lock, so a
legacy JSON payload at the same path is treated as a malformed SQLite database and escapes
acquisition.

server/io/fileLock.ts[34-54]
server/phases/manualQa/storage.ts[432-453]
server/io/tests/fileLock.test.ts[73-83]

Agent prompt
The issue below was found during a code review. Follow the provided context and guidance below and implement a solution

Issue description
The lock implementation changed the existing `index.json.lock` pathname from a JSON token file to a SQLite database without handling old contents. A stale lock left by the previous implementation is not a valid SQLite header, so every acquisition throws before the retry loop can help.

Fix Focus Areas
- server/io/fileLock.ts[40-54]
- server/phases/manualQa/storage.ts[432-453]
- server/io/__tests__/fileLock.test.ts[73-164]

Recommended Fix
Before opening the SQLite database, detect a legacy non-SQLite lock file and safely replace or migrate it only when it is conclusively an old lock artifact. Add an upgrade test that seeds the former JSON lock content at the evidence lock path and verifies that a new evidence operation succeeds.

3. Some Macs skip mixed-case files ✓ Resolved 🐞 Bug ≡ Correctness
Description
isWithinRoot lowercases every path on Darwin, so an untracked path such as Build/source.ts
matches the generated-output root build even on a case-sensitive volume.
summarizeWorktreeChanges consequently removes it from committable, and commitBeadChanges
places it among skipped files instead of staging it.
Code

server/git/worktreeChanges.ts[R152-154]

+  const compare = (value: string) => process.platform === 'win32' || process.platform === 'darwin'
+    ? value.toLowerCase()
+    : value
Relevance

●● Moderate

Plausible platform-specific correctness issue, but no closely matching historical acceptance or
rejection precedent.

ⓘ Recommendations generated based on similar findings in past PRs

Evidence
The new Darwin branch lowercases both operands before root comparison, while generated-root
classification uses that comparison and marks matching untracked paths as generated noise. The bead
commit path then stages only summary.committable and explicitly places generated-noise entries in
skippedFiles, proving that a case-sensitive macOS repository can omit a valid mixed-case source
path.

server/git/worktreeChanges.ts[149-157]
server/git/worktreeChanges.ts[228-283]
server/phases/execution/gitOps.ts[114-147]

Agent prompt
The issue below was found during a code review. Follow the provided context and guidance below and implement a solution

## Issue description
Path classification assumes every macOS filesystem is case-insensitive. On case-sensitive macOS volumes, legitimate mixed-case paths can therefore be classified as generated or control files and omitted from commits.

## Fix Focus Areas
- server/git/worktreeChanges.ts[149-157]
- server/git/worktreeChanges.ts[228-283]
- server/git/repoScopedPath.ts[22-26]
- server/phases/execution/gitOps.ts[114-147]

## Recommended Fix
Do not infer case sensitivity solely from `process.platform === 'darwin'`. Preserve exact-case comparison on macOS, or determine the repository filesystem's actual case behavior and pass that policy into path classification and repository-scoped validation; add coverage for a mixed-case path such as `Build/source.ts` on a case-sensitive policy.

4. Rename diagnostics mark changes staged ✓ Resolved 🐞 Bug ≡ Correctness
Description
parseGitStatusPorcelainZ emits every rename source as an index deletion, and the newly changed
parseStatusLines consumes that synthetic record to populate status sets. When Git reports a
worktree-only rename as R, the old path is placed in stagedFiles instead of unstagedFiles, so
recovery receipts and worktree diagnostics describe the wrong state.
Code

server/git/github.ts[636]

+  const records = parseGitStatusPorcelainZ(statusOutput)
Relevance

●●● Strong

Direct Git status misclassification causes incorrect diagnostics; recent Git boundary hardening
precedents favor fixing it.

PR-#143

ⓘ Recommendations generated based on similar findings in past PRs

Evidence
The parser's own test supplies a R new.ts\0old.ts\0 record but expects a source record with index
status D. GitHub status classification treats any non-space index status as staged and any
non-space worktree status as unstaged, so the incorrect synthetic column directly reaches its
diagnostics.

server/git/statusPorcelain.ts[53-68]
server/git/tests/statusPorcelain.test.ts[32-39]
server/git/github.ts[636-653]

Agent prompt
The issue below was found during a code review. Follow the provided context and guidance below and implement a solution

Issue description
A synthetic deletion for a rename always has index status `D`. For a rename reported only in Git's worktree status column, this incorrectly changes an unstaged deletion into a staged one for all consumers of the parsed status records.

Fix Focus Areas
- server/git/statusPorcelain.ts[53-68]
- server/git/github.ts[636-653]
- server/git/__tests__/statusPorcelain.test.ts[32-39]

Recommended Fix
Emit the synthetic source deletion in the same status column as the rename: use index `D` and worktree space for an index rename, and index space and worktree `D` for a worktree-only rename. Add assertions for the status columns of both variants.

5. Interrupted writes leave proof files ✓ Resolved 🐞 Bug ⚙ Maintainability
Description
atomicWrite creates a YAML proof sidecar and removes it only in the same process after the target
rename completes. If that process stops after the rename and before the unlink, startup scans only
names ending in .tmp, so the remaining .tmp.proof file is never cleaned.
Code

server/io/atomicWrite.ts[R298-303]

+    if (proofCreated) {
+      try {
+        assertContained?.(proofPath)
+        unlinkSync(proofPath)
+      } catch { /* recovery cleans a proof left after a completed rename */ }
+      proofCreated = false
Relevance

●●● Strong

Crash-window cleanup gap matches the team’s recent durability focus and accepted interrupted-write
fixes.

PR-#133

ⓘ Recommendations generated based on similar findings in past PRs

Evidence
Proof paths are newly defined by appending .proof to the atomic temp name, and the normal write
path has a post-rename cleanup window. Recovery filters entries to .tmp and legacy temp names,
neither of which matches a .tmp.proof sidecar.

server/io/atomicWrite.ts[63-65]
server/io/atomicWrite.ts[278-303]
server/io/recovery.ts[774-790]

Agent prompt
The issue below was found during a code review. Follow the provided context and guidance below and implement a solution

Issue description
A crash after publishing a YAML target but before deleting its proof sidecar leaves a `.tmp.proof` file. Recovery ignores that suffix because it only processes temp-file names, despite the write path claiming recovery will clean it.

Fix Focus Areas
- server/io/atomicWrite.ts[63-65]
- server/io/atomicWrite.ts[291-303]
- server/io/recovery.ts[774-790]

Recommended Fix
Extend recovery to recognize proof sidecars whose sibling temp file is absent. Derive and validate the intended target using the same artifact and containment checks, then remove only a verified orphan proof sidecar; add a crash-window recovery test.

Migrate legacy lock files before opening them as SQLite.

withEvidenceIndexLock still uses index.json.lock. The previous implementation stored a JSON token at that path. If that process crashed, the token remains. DatabaseSync then reports ERR_SQLITE_ERROR with file is not a database, and isBusy rethrows it. No startup cleanup removes the file, so Manual QA index operations remain unavailable.

Do not delete every non-database file automatically. An old process may still hold a valid JSON token, and an arbitrary or corrupt file produces the same SQLite error. Deleting it could allow old and new processes to update the index concurrently.

Run a one-time migration while all old writers are stopped. Validate the exact legacy token format, then quarantine or remove that file before SQLite creates the replacement database. Otherwise keep the error and report that manual migration is required.

Review by Claude Opus 5

1. [Important] buildEnv default GIT_SSH_COMMAND silently overrides the user's core.sshCommand (G24)

server/git/runCommand.ts buildEnv (~L475): the new default GIT_SSH_COMMAND='ssh -o BatchMode=yes' is added whenever the environment lacks GIT_SSH_COMMAND/GIT_SSH. Git gives the environment variable precedence over core.sshCommand in any config scope. Verified locally: with core.sshCommand="echo CONFIGCMD" and GIT_SSH_COMMAND="echo ENVCMD", git ls-remote ssh://… runs ENVCMD.
Failure: a user who picks a key or agent through git config (core.sshCommand = ssh -i ~/.ssh/work_key, Windows OpenSSH/1Password ssh.exe, sshCommand in an includeIf for a work directory) gets plain ssh on every LoopTroop fetch and push. That means the wrong key or no agent, and the result is "Permission denied (publickey)". Nothing in LoopTroop mentions the override.
Fix: leave core.sshCommand in charge. Two options: pass BatchMode through GIT_CONFIG_COUNT/GIT_CONFIG_KEY_n=core.sshCommand only when git config --get core.sshCommand in the project is empty, or skip the default when that lookup returns a value. Also add a test that has core.sshCommand configured.
The CHANGELOG line added in this PR ("SSH setup stays non-interactive without overwriting a caller's real configuration") is wrong for the same reason.

2. [Medium] G-01 is incomplete: Manual QA checkpoint mutations still use the sync 30 s SIGKILL runner

server/phases/manualQa/checkpoint.ts:74-98, 317-362. This PR touches the file, but discardExactFiles (restore --staged --worktree, clean -fd) and commitExactFiles (add -f -A, commit --only) still go through runGitSync. That path uses the 30 s default timeout and kills with killSignal: 'SIGKILL'. The ledger says G01 moved "local Git mutations" to the async SIGTERM→SIGKILL runner with GIT_MUTATION_TIMEOUT_MS.
Failure: Manual QA checkpoints a large or LFS-tracked tree, and add -f -A runs clean/smudge filters for more than 30 s. The add is SIGKILLed and leaves .git/worktrees/<id>/index.lock behind. withGitIndexRollback then cannot restore the index, and every later git call in that worktree fails with "Another git process seems to be running". This is the same scenario G-01 describes for bead commits. hookValidation.ts:110-116 (read-tree/add -A against a temporary index) is also still sync.
Fix: make commitExactFiles/discardExactFiles async on runGitMutation under withGitIndexRollbackAsync, as gitOps.commitBeadChanges now is. Add the hook/filter timeout test G-01 asks for on this path.

3. [Low] detached: true for every async git/gh child cuts it off from terminal SIGINT

server/git/runCommand.ts:~465 puts every async child in its own process group on POSIX so that the timeout can signal the whole tree. A side effect is that Ctrl+C on a foreground looptroop start no longer reaches in-flight git children, and the daemon does not track them to kill on exit.
Failure: Ctrl+C during a 5-minute worktree add, reset --hard or merge --ff-only stops the daemon while git keeps running without a parent. The next start then races that git process for index.lock, or finds a half-built worktree.
Fix: keep a set of live detached children in runAsyncRaw and send terminateProcessTree(child, 'SIGTERM') to each from the daemon's shutdown/SIGINT handler. Alternatively, go back to non-detached children and only signal the group when a timeout fires.

4. [Medium] G-14 regression: /api/projects/check-git and /api/projects/ls now return HTTP 500 for untrimmed or relative input

server/storage/paths.ts normalizeFolderPath now throws ContainedPathError('Project path must be absolute') for relative input, as intended. But server/routes/projects.ts:109,133,204,213,274 call it with no catch, and projectRouter has no onError mapping for ContainedPathError (unlike tickets.ts/files.ts/beads.ts). Nothing in the client was changed to trim either: ProjectForm.tsx:145 sends the raw folder text.
Verified: a Hono request against projectRouter returned " /tmp" → 500 Internal Server Error, "repo" → 500, and "/tmp " → 200 invalid.
Failure: a user pastes a path with a leading space, or the 300 ms debounce fires while they are still typing ~/proj or proj. The form shows "Git check failed" (a server error) where it used to show a "not a valid folder" verdict. The report's fix direction says to trim in the UI text field, and that part was not done.
Fix: catch ContainedPathError in these routes and return the existing status: 'invalid' payload (check-git) or 400 (ls), for example with a projectRouter.onError like tickets.ts:63. Trim the folder text in ProjectForm/FolderPicker before sending it. Add route tests for ' /x' and 'rel'.

5. [Low] G-03's new test still passes with the fix reverted

server/log/__tests__/executionLog.test.ts "canonicalizes a symlinked project root…". Mutation check: I reverted executionLog.ts to safeAtomicAppendWithin(projectRoot, relative(projectRoot, logPath), …), and the file still passed 16/16. The test replaces the append with mockAppend, so the containment check that threw Path escapes root never runs.
Fix: use the real safeAtomicAppendWithin with a temp symlinked root, then assert that the line landed in the real log file, or assert the root/relative arguments passed to the spy (calls[0][0] equals the realpath and calls[0][1] does not start with ..).

Coverage of this PR's findings

I checked each fix in the diff. Where marked "mutation", I reverted the fix site and confirmed that the named test file goes red:

  • G-01: partial, see item 2.
  • G-03: fixed, but the test is inert (item 5).
  • G-05: mutation red.
  • G-06: fix present.
  • G-08, G-09, G-10, G-30.1–4, G-32: fixed (read). A test fails with the G-11 revert.
  • G-11: mutation red.
  • G-12, G-13 (squash, operations, executionSetup/storage, status -z): fixed (read). I also ran a directory→symlink squash scratch test, which passed.
  • G-14: regression, see item 4.
  • G-15: fixed (read).
  • G-16: mutation red.
  • G-22: fix present.
  • G-23: fixed (read).
  • G-24: mutation red, but see item 1.
  • G-25: replaced by a SQLite BEGIN IMMEDIATE lock (read).
  • G-31.2 and G-31.3: fixed (read).
  • R-23: mutation red.
  • W-17: mutation red.
  • W-29.1: fix present.
  • S-12(2): fixed (read).

The PR does not claim G-30.5–7, G-31.1, G-31.4 or W-29.2; confirm another part owns them. The website (operations.md, system-architecture.md, diagnostics.md, database-schema.md) already documents the recovery/proof/RECOVERY_BLOCKED behavior. Nothing there needs to change for these items apart from the CHANGELOG SSH sentence in item 1.

Antigravity Review

1. Critical Bug: Mid-Copy Fallback Crash Permanently Wedges Daemon Boot (G-08)

  • Location: server/io/recovery.ts, server/io/recovery.ts

  • Problem:
    In promoteTmpFile, before starting the fallback copy, the marker is written without target:

    writeRecoveryMarkerWithDeps(markerPath, { version: 1, targetPath, source }, false, deps)

    The target identity is written to the marker only after copyValidatedSource has already fully completed (recovery.ts:703).
    If the daemon crashes or loses power during the chunked copy (the exact scenario finding G-08 is about), on the next boot:

    1. pathIsTaken(targetPath) is true (the partially written target exists).
    2. resumeMarkedCopy runs and reads the marker. Because the crash happened during the copy, marker.target is undefined.
    3. Lines 556–569 execute:
      complete = targetContainsSource(fd, source, targetFd, target)
        && sameEntryIdentity(lstatSync(targetPath), target)
      ...
      if (!complete) {
        return blockRecovery(
          targetPath,
          tmpPath,
          'its prepared fallback target is incomplete or changed',
        )
      }
    4. targetContainsSource returns false because sourceSize !== targetSize.
    5. blockRecovery throws RecoveryBlockedError.
    6. In server/startup.ts:64, RecoveryBlockedError is rethrown, aborting startupSequence().

    The daemon refuses to boot on every restart until an operator manually intervenes. Instead of recovering the complete temporary file over the partial target, a mid-copy crash becomes a permanent daemon outage.

  • Remediation / Better Path:
    There are two ways forward:

    1. Recommended (Stateless Atomic Fallback): Avoid .recovery markers entirely. Do not copy directly to targetPath. Copy chunk-by-chunk from tmpPath to a second temporary file (${targetPath}.copy-${randomUUID()}.tmp), fsync the second temporary file, and perform an atomic rename(secondTemp, targetPath). A mid-copy crash leaves targetPath completely absent and the partial second temp is discarded.
    2. If Retaining Markers: Capture fstatSync(targetFd) immediately after openSync(..., O_CREAT | O_EXCL) and write { version: 1, targetPath, source, target } to the marker before starting copyValidatedSource. When resumeMarkedCopy runs on an incomplete target matching marker.target, truncate and resume the copy with copyValidatedSource(fd, source, targetFd, true) rather than calling blockRecovery.

2. Severe Regression: Unconditional 4-Second Timeout Delay & Immediate SIGKILL Race (G-01)

  • Location: server/git/runCommand.ts

  • Problem:
    In runAsyncRaw:

    const timer = setTimeout(() => {
      if (overranBuffer) return
      timedOut = true
      terminateProcessTree(child, 'SIGTERM')
      killTimer = setTimeout(forceKill, TIMEOUT_KILL_GRACE_MS)
      ...
      abandonTimer = setTimeout(() => {
        timeoutCleanupComplete = true
        child.unref()
        settle(...)
      }, TIMEOUT_KILL_GRACE_MS + TIMEOUT_ABANDON_GRACE_MS)
    }, timeoutMs)

    When Git times out, it is sent SIGTERM. Git handles SIGTERM gracefully, cleans up .git/index.lock, and exits in tens of milliseconds. When Git exits:

    1. child.on('close') fires and calls settle(outcome).
    2. In settle:
      if (timedOut && !timeoutCleanupComplete) {
        pendingTimeoutOutcome ??= outcome
        forceKill()
        return
      }
    3. timedOut is true, and timeoutCleanupComplete is false (it is only set 4,000 ms later by abandonTimer).
    4. forceKill() is called immediately, sending SIGKILL to the entire process tree without any grace period. If a pre-commit hook was in the middle of executing a cleanup trap on SIGTERM, it is abruptly killed by SIGKILL.
    5. settle returns without resolving or rejecting the promise.
    6. Because child has closed, no further process events fire. The promise remains pending until abandonTimer fires 4 full seconds later (TIMEOUT_KILL_GRACE_MS + TIMEOUT_ABANDON_GRACE_MS).

    Every timed-out Git operation in LoopTroop—even when Git exits cleanly within 20 ms and has no child processes—hangs the caller and the event loop for an extra 4 seconds. On Windows, where taskkill /T /F kills the tree immediately, every timeout is also penalized with this 4-second wait.

  • Remediation / Better Path:
    Do not unconditionally wait for abandonTimer when Git exits. Check if any descendant processes actually remain in the captured process group (using processTree.ts's hasCapturedGroupMember(snapshot) on Linux). If no descendants exist, settle immediately. Only if descendants survive should killTimer remain active to send SIGKILL after TIMEOUT_KILL_GRACE_MS.

    const settle = (outcome: RawOutcome<string>) => {
      if (settled) return
      if (timedOut && !timeoutCleanupComplete) {
        if (snapshot && hasCapturedGroupMember(snapshot)) {
          pendingTimeoutOutcome ??= outcome
          // Let killTimer fire after grace period instead of sending SIGKILL instantly
          return
        }
      }
      settled = true
      clearTimeout(timer)
      if (killTimer) clearTimeout(killTimer)
      if (abandonTimer) clearTimeout(abandonTimer)
      if (overrunTimer) clearTimeout(overrunTimer)
      settleWith(pendingTimeoutOutcome ?? outcome)
    }

3. Missing Requirements: Internal Recovery Artifacts Omitted from Allowlist (G-30.1, G-32)

  • Location: server/io/recovery.ts, server/io/recovery.ts

  • Problem:
    isKnownArtifactTarget was introduced to prevent recovery from promoting arbitrary .tmp files. However, the allowlist missed internal LoopTroop restore artifacts written via safeAtomicWriteWithin:

    • opencode-steps-restore.json: Written at server/phases/execution/opencodeStepsConfig.ts:312. Contains the original content and SHA of the user's opencode.json (often containing API credentials) while steps are capped.
    • hook-validation-restore.json: Written at server/phases/executionSetup/hookValidation.ts:249. Contains the worktree snapshot and tree state required to recover from a crash during hook execution.

    If a crash occurs while either sidecar is being saved, recoverOrphanTmpFiles encounters opencode-steps-restore.json.<pid>.<hash>.tmp, logs:
    [recovery] Leaving ...: target is not a known LoopTroop artifact
    and refuses to recover it. The sidecar is lost, leaving the capped opencode.json configuration or hook restore permanently orphaned (directly violating G-04, G-29, and G-30.5).

  • Remediation / Better Path:
    Update KNOWN_TICKET_ROOT_ARTIFACTS in server/io/recovery.ts:

    const KNOWN_TICKET_ROOT_ARTIFACTS = new Set([
      '.gitignore',
      'interview.yaml',
      'prd.yaml',
      'relevant-files.yaml',
      'opencode-steps-restore.json',
      'hook-validation-restore.json',
    ])

4. Leaked Sidecars: Orphaned .proof Files Accumulate Indefinitely (G-09)

  • Location: server/io/atomicWrite.ts, server/io/recovery.ts
  • Problem:
    server/io/atomicWrite.ts:302 states:
    /* recovery cleans a proof left after a completed rename */
    However, in server/io/recovery.ts:785:
    if (!name.endsWith('.tmp') && !LEGACY_TMP_NAME.test(name)) continue
    A .proof sidecar is named <target>.<pid>.<hash>.tmp.proof. Because its name ends with .proof rather than .tmp, scanDir skips it. If a crash occurs after rename(tmpPath, filePath) succeeds but before unlinkSync(proofPath) executes, the .proof file is never scanned and remains orphaned on disk indefinitely.
  • Remediation / Better Path:
    Teach scanDir in server/io/recovery.ts to recognize .tmp.proof files and unlink them if the sibling .tmp file is absent and the target file already exists.

5. Incomplete Staged Deletion Filter: Misses Renamed (RD) and Copied (CD) Paths (G-05)

  • Location: server/phases/execution/gitOps.ts
  • Problem:
    In commitBeadChanges:
    const commitEntries = committableEntries.filter(entry => !(entry.indexStatus === 'A' && entry.worktreeStatus === 'D'))
    const committableFiles = commitEntries.map(entry => entry.path)
    The filter only excludes files with status AD (added in index, deleted in worktree).
    If a file was renamed in the index (indexStatus === 'R') or copied (indexStatus === 'C') and the new path is subsequently deleted from disk before the bead commit, the new path has status RD or CD.
    1. git add -- <newPath> removes <newPath> from the index.
    2. Because <newPath> is a new destination that never existed in HEAD, and is no longer in the index or worktree, git commit -m ... -- <newPath> fails with:
      error: pathspec ':(literal)<newPath>' did not match any file(s) known to git
    3. withGitIndexRollbackAsync catches the failure, restores the index, and the error repeats indefinitely on retry.
  • Remediation / Better Path:
    Generalize the exclusion so that any deleted worktree path that does not exist in HEAD (including additions AD, renames RD, and copies CD) is excluded from committableFiles:
    const commitEntries = committableEntries.filter(
      entry => !(entry.worktreeStatus === 'D' && (entry.indexStatus === 'A' || entry.indexStatus === 'R' || entry.indexStatus === 'C'))
    )

6. Security Gap: Unchecked Alternate Data Streams (ADS) on Windows in repoScopedPath (G-14, G-23)

  • Location: server/git/repoScopedPath.ts
  • Problem:
    While server/lib/containedPath.ts:24 was correctly updated to reject colons on Windows to block Alternate Data Streams (value.slice(drivePrefix ? 2 : 0).includes(':')), normalizeRepoScopedPath only checks /^[A-Za-z]:/.
    A path such as sub/file.txt:stream or foo.ts::$DATA is accepted by normalizeRepoScopedPath and passed into git add or git checkout. On Windows NTFS, this accesses Alternate Data Streams outside the intended file boundaries.
  • Remediation / Better Path:
    Reject colons in normalizeRepoScopedPath when running on Windows:
    if (process.platform === 'win32' && platformPath.slice(drivePrefix ? 2 : 0).includes(':')) {
      return null
    }

7. Divergent Parsing of Baseline Signatures Between Checkpoint and Operations (G-13)

  • Location: server/phases/manualQa/checkpoint.ts, server/phases/manualQa/operations.ts
  • Problem:
    When capturing tracked signatures in checkpoint.ts:179:
    const path = normalizeProjectPath(match[2])
    if (path) signatures[path] = match[1]
    Paths are normalized with normalizeRepoScopedPath.
    However, in operations.ts:143:
    const match = entry.match(/^\d+ ([0-9a-f]+) \d+\t([\s\S]+)$/)
    if (match?.[1] && match[2]) currentSignatures[match[2]] = match[1]
    currentSignatures stores match[2] raw, without normalizeProjectPath.
    When drift comparison is performed in operations.ts:150-154:
    for (const path of signaturePaths) {
      if ((baseline.trackedSignatures ?? {})[path] !== currentSignatures[path]) committedPaths.add(path)
    }
    Any discrepancy between the normalized key in baseline.trackedSignatures and the raw key in currentSignatures (e.g. leading ./, backslashes on Windows, or paths filtered by control directory rules) will cause the check to mismatch and falsely report untouched files as having committed workspace drift.
  • Remediation / Better Path:
    Use the same path normalization function for both signature generation in checkpoint.ts and drift detection in operations.ts.

8. Missing Fsync on Truncation in fixTrailingLineCorruption (G-10, G-11)

  • Location: server/io/recovery.ts, server/io/recovery.ts
  • Problem:
    In fixTrailingLineCorruption and fixCorruptionLarge:
    ftruncateSync(fd, retainedBytes)
    return true
    and
    ftruncateSync(fd, lineStart)
    return true
    closeSync(fd) is called in the finally block, but neither branch calls fsyncSync(fd).
    If power is lost or the OS crashes shortly after startup repair, the directory/file truncation metadata may not have flushed to disk, leaving the corrupt trailing bytes intact upon the next boot.
  • Remediation / Better Path:
    Call fsyncSync(fd) immediately after ftruncateSync(fd, ...).

9. Symlink Traversal Vulnerability in Manual QA safeAtomicWriteWithin (G-03)

  • Location: server/phases/manualQa/storage.ts, server/phases/manualQa/storage.ts, server/phases/manualQa/checkpoint.ts
  • Problem:
    Finding G-03 showed that calling safeAtomicWriteWithin(root, relative(root, path), ...) throws ContainedPathError when root contains a symlink and path is canonicalized, because relative() produces leading ../ escape sequences.
    While executionLog.ts was fixed:
    const canonicalRoot = resolveContainedPath(projectRoot, '.')
    const range = safeAtomicAppendWithin(canonicalRoot, relative(canonicalRoot, logPath), serialized)
    Multiple Manual QA call sites in storage.ts and checkpoint.ts still call:
    safeAtomicWriteWithin(ticketDir, relative(ticketDir, path), ...)
    Because path originates from validateManualQaDirectory (which resolves the realpath), if ticketDir resides in a symlinked folder, relative(ticketDir, path) generates ../ segments, causing Manual QA writes to fail with ContainedPathError: Path escapes root.
  • Remediation / Better Path:
    Canonicalize ticketDir with resolveContainedPath(ticketDir, '.') prior to relative() and safeAtomicWriteWithin.

10. Architectural Omissions Missed by the Initial Plan

  1. Process Group Lifetime vs. Leader Close:
    The plan assumed that escalating from SIGTERM to SIGKILL on the child process was sufficient for local Git mutations. It missed the fact that Git hooks and filter processes frequently run in their own child subtrees with redirected stdio. A runner that simply watches the leader process will return while hooks are still actively mutating the worktree. As seen in Issue 2, attempting to patch this without tracking group members leads to unneeded 4-second hangs.
  2. Stateful Markers vs. Pure Atomic Swaps:
    For fallback copies without hardlinks (G-08), the plan permitted a stateful marker approach. Stateful markers create edge cases across crashes, reboots, and permission errors. Mandating a second-temp write + atomic rename (temp -> temp2 -> rename(temp2, target)) is clean, stateless, and impossible to leave in an unrecoverable state.
  3. Fragmented Git Readers:
    The plan called for a -z reader, but failed to specify a single unified parser. This resulted in fragmented ad-hoc parsers in squash.ts, statusPorcelain.ts, operations.ts, and checkpoint.ts, each handling edge cases (spaces, tabs, renames) inconsistently.

Union Alpha — P2: Give each discard action its own quarantine destination

At HEAD 5295ccdc7cb4622d52fb95e0579494f07c09d68f, server/phases/manualQa/checkpoint.ts:289-300 now refuses existing quarantine entries, but quarantineFiles still names its root only by QA version (:266) and source path, not action ID. After successfully discarding a modification to README.md, editing and discarding README.md again during the same QA version fails because the first backup remains. Retrying an action interrupted after its copy has the same problem. applyManualQaDriftDecision invokes quarantine before restoring files (:465-466), so the second action cannot resolve drift; the existing tests exercise only one discard per path/version.

Keep no-overwrite protection, but put backups under an action-ID-derived directory and make retries of that action verify/reuse their own matching backup without overwriting a different generation. Add coverage for two successful discards of the same tracked path in one version, and retry after partial quarantine.

Union Alpha — P2: Require whole-write proof for JSONL recovery too

At HEAD 5295ccdc7cb4622d52fb95e0579494f07c09d68f, server/io/recovery.ts:128-132,151-168 promotes an empty JSONL temp or any newline-terminated sequence of valid records without proving the intended file was completely written. server/io/atomicWrite.ts:244,263-280 creates the empty temp before writing content and emits completion proofs only for YAML. When the target is missing (for example, an interrupted first write), a crash before the first content write therefore turns a nonempty intended bead tracker into a recovered empty collection; interruption at a record boundary similarly promotes a valid prefix. Newline termination proves record completeness, not whole-file completeness. Existing targets take a separate recovery branch; this finding concerns missing targets.

This is the same data-loss gap G09 fixes for YAML, still present in G10: writeTicketBeads (server/workflow/phases/beadsPhase.ts:867-871) serializes the entire authoritative tracker as independent newline-terminated records, and recovery explicitly allows its issues.jsonl path (recovery.ts:325). The empty-temp test (server/io/__tests__/atomicIO.test.ts:569-575) currently blesses an indistinguishable interrupted write.

Extend length/hash completion proofs to whole-file JSONL writes, including intentional zero-byte collections, and require a matching proof before promotion. Preserve unproven empty/prefix temps rather than treating them as completed trackers. Cover both crash-before-write and complete-record-prefix cases.

Union Alpha — P2: Finish the async mutation conversion in Manual QA

At HEAD 5295ccdc7cb4622d52fb95e0579494f07c09d68f, server/phases/manualQa/checkpoint.ts:74-78 still sends every command through runGitSync, including add and commit (:337-358) and restore/clean (:317-326,471-478). These execute in normal production flows: checklist generation calls prepareManualQaCheckpoint (server/phases/manualQa/generator.ts:247), and the workspace-decision handler calls include/discard (server/routes/ticketHandlers/manualQaHandlers.ts:590-591). Thus G01 remains unfixed for these callers despite the new mutation runner.

A project clean/smudge filter, LFS operation, or large restore taking over 30 seconds still blocks the daemon and receives immediate SIGKILL (server/git/runCommand.ts:239-243), potentially leaving Git locks and stranded QA actions. --no-verify on commit does not disable clean/smudge filters used by staging/restoring files. Current checkpoint tests use fast local operations and do not cover this boundary.

Use runGitMutation/runGitMutationOrThrow for the mutation commands and withGitIndexRollbackAsync for their transaction; propagate and await the resulting promises through checkpoint generation and the workspace-decision handler. Keep read-only probes synchronous if needed.

Claude Code Review — PR #164 (Part 2: Git operations and filesystem recovery)

1. Missing findings from Part 2 scope

The consolidated work plan (lines 117-119) assigns G-01 through G-36 to Part 2. The audit ledger at .github/audit-dispositions/git-filesystem.md covers a subset, leaving 13 findings unaddressed — including 2 High and 3 Medium:

Finding Severity Why it matters
G-02 High Board/public payload reads beads raw, skipping the alias table (qa_origin→qaOrigin, etc.). A malformed line silently empties every bead on the board and can cause ALL_BEADS_DONE with a planned bead never run.
G-04 Medium After a mid-run edit to opencode.json, the step-cap exclusion is keyed on "applied this bead" instead of "a record exists", so the cap gets committed and stays off for the rest of the ticket.
G-07 Medium ensureLocalGitExclude writes the ignore rule to the main checkout's shared info/exclude. Neither restore nor boot recovery removes it. After any capped run, the user's main checkout silently hides the file from git status and git add ..
G-17 Medium clean --apply doesn't re-check a worktree's dirtiness between plan and force-remove. An editor, hook, or late workflow write during the window loses its work.
G-18 Low stop's stale-state path uses unconditional rmSync instead of instance-matched clearDaemonState.
G-19 Low clean's first SIGTERM to OpenCode fires without re-checking process identity.
G-20 Low Concurrent looptroop start can report another invocation's daemon as its own.
G-21 Low The SIGKILL escalation timer is never cancelled when the child exits on SIGTERM.
G-26 Low The raw-fs lint guard covers only server/workflow and server/storage.
G-27 Low CLI builds invalid URLs for a daemon bound to IPv6 loopback.
G-28 Low looptroop logs --follow loses lines between the tail read and follow start.
G-29 Low Hook-validation snapshot restore doesn't survive daemon death mid-validation.
G-36 Low The skip-receipt unique index fails on a DB that already has duplicates.

G-02 (High, data loss) and G-07 (Medium, silent data hiding) deserve priority consideration for a follow-up or inclusion in this PR.

2. squash.ts mutation-command heuristic

squash.ts:16-18 routes git calls to the async mutation runner via:

const GIT_MUTATION_COMMANDS = new Set(['add', 'checkout', 'commit', 'merge', 'reset', 'rm'])
async function runSquashGit(worktreePath: string, args: string[]): Promise<string> {
  if (args.some((arg) => GIT_MUTATION_COMMANDS.has(arg))) {

This matches any argument, not just the subcommand. While git flags don't collide with these words today, checking args[0] (or the first non-flag argument) would be more robust against future flag additions or caller errors.

3. normalizeRepoPath trailing-slash removal

worktreeChanges.ts:121-124 removes .replace(/\/+$/, '') from normalizeRepoPath, moving it only to normalizeSetupRoot. The change is correct — Git output should keep its bytes intact — but any caller of normalizeRepoPath that previously relied on trailing-slash stripping will now get different paths. Verify that all current callers (especially in isWithinRoot, collectPathStrings, and the file-effects audit) handle trailing slashes correctly, since a trailing / makes startsWith checks behave differently (e.g. foo/ vs foo).

4. ensureWorktreeClean generated-noise allowlist

github.ts:791-793 now allows untracked files classified as generatedNoise to pass the clean-tree check. The classification comes from classifyWorktreePath. If the generated-noise rules are too broad (e.g. a user creates a dist/ directory with hand-written code), those changes would silently pass the clean-tree gate. Consider logging a warning when generated noise is allowed, so the delivery audit trail is not silent about it.

5. Recovery .recovery marker sidecar cleanup on normal write path

atomicWrite.ts:295-302 cleans up the .proof sidecar after a successful rename. However, the .recovery marker written during copy-fallback promotion (recovery.ts) is separate and cleaned up by the recovery code itself. If a crash occurs between writing the .recovery marker and the final cleanup, the marker persists. The next boot handles this correctly (resume or block), but the marker files will accumulate if recovery completes successfully but cleanup fails silently. The existing catch { /* best-effort */ } pattern means these sidecars are left intentionally, but document that .recovery, .proof, and .recovery.write-* files are expected cleanup artifacts (which the CONTRIBUTING.md addition does — good).

6. fileLock.test.ts portability

The startLockHolder helper spawns node --input-type=module -e '...' with node:sqlite import. This requires Node ≥22.5 for the built-in SQLite module. Since engines.node is >=24.18.1, this is fine, but the test silently skips on Windows via .runIf(process.platform !== 'win32'). The SQLite lock itself works on Windows, so consider adding a Windows-compatible test or documenting why the cross-process test is skipped.

7. runGitMutationOrThrow error message

runCommand.ts:458-461:

export async function runGitMutationOrThrow(...): Promise<string> {
  const result = await runGitMutation(projectPath, args, options)
  if (!result.ok) throw new Error(result.errorDetail)
  return result.stdout
}

The thrown Error carries the errorDetail string, which now includes the timeoutDetail (index.lock path) when applicable. This is good for diagnostics, but the error type is a plain Error. Callers that catch and re-wrap (e.g. squash.ts) should preserve the detail chain so it reaches the user-facing error.

8. Test for G-05 (AD staged-then-deleted)

The test commitBeadChanges → clears a staged-then-deleted scratch file is well-constructed: it verifies the commit succeeds with only feature.ts, that a second call is a no-op, and that the worktree is clean afterwards. One gap: it doesn't verify the git add was still called for the AD path (to clear the stale index entry). Consider asserting the git add args include the AD file's pathspec.

9. recovery.ts RecoveryBlockedError propagation

The RecoveryBlockedError is carefully propagated through scanDir and the startup sequence. The tests verify that startWalCheckpoint, rebuildTicketRuntimeProjections, and hydrateAllTickets are never called. However, the tests mock listTickets and getTicketPaths — consider adding a test that verifies the error message includes both the target path and the temp path, since that's the diagnostic an operator will see.

10. assertSafeRefName validation coverage

ref.ts validates refs against leading -, .., control characters, @{, dot-components, and .lock suffixes. It does not reject:

  • Refs containing \0 (NUL): already caught by runWorkingDirectory and spawnSync, but an explicit check in assertSafeRefName would be defense-in-depth.
  • Refs longer than 255 bytes (some filesystems truncate).
  • The empty-string check (ref.length === 0) is present — good.

11. CHANGELOG update accuracy

The CHANGELOG changes correctly describe the new behavior but use somewhat vague phrasing for the recovery model. The line "Git mutations and delivery now preserve opaque paths" could be clearer — it's specifically NUL-delimited readers that keep unusual bytes opaque, not all path operations. The existing phrasing is acceptable for a changelog but could mislead someone looking for exact guarantees.

Summary: The PR makes substantial, well-tested progress on the scoped findings. The async mutation runner with SIGTERM→SIGKILL escalation and process-tree cleanup (G-01), the SQLite-based file lock (G-25), the recovery marker system (G-08/G-09/G-10), and the NUL-delimited path readers (G-13) are the highlights. The main gap is the set of 13 unaddressed findings from Part 2's assigned range, particularly G-02 (High) and G-07 (Medium). The implementation is clean and the test coverage is strong for the addressed items.

Union Alpha — P2: Preserve core.sshCommand when adding the noninteractive SSH default

At HEAD 5295ccdc7cb4622d52fb95e0579494f07c09d68f, server/git/runCommand.ts:138-139 sets GIT_SSH_COMMAND whenever the two SSH environment variables are absent, without considering Git configuration. Git explicitly gives that environment variable precedence over core.sshCommand (https://git-scm.com/docs/git-config#Documentation/git-config.txt-coresshCommand). Consequently a repository whose core.sshCommand selects a nondefault identity, proxy, or SSH wrapper works with ordinary Git but loses that configuration for LoopTroop operations after this change. This reaches normal origin fetch (server/git/repository.ts:92-95) and bead pushes (server/git/push.ts:145-147), not just a hypothetical direct runner call.

Resolve and preserve the effective configured SSH command before supplying the fallback; only choose plain ssh with BatchMode when no explicit SSH command is configured. Apply any noninteractive option in a way compatible with the selected transport rather than replacing it. Add a runner/remote-options regression test for configured core.sshCommand with no GIT_SSH/GIT_SSH_COMMAND environment override; current tests cover only environment overrides.

Muse Spark review (PR 164, isolated diff main...fix/audit-git-filesystem vs report G-01..G-36):

[High] G-02 bead alias table + fail-closed execution write still missing

Where: server/storage/ticketQueries.ts:1222, server/phases/beads/beadsFile.ts:249
Evidence: readRuntimeBeads uses raw readJsonl + camelCase-only projection, no canonicalizeBeadAliases; readBeadsFile defaults malformedEntries: 'skip', only adapter.ts:904 passes 'fail', so execution writers still rewrite from lossy read and can delete one malformed line into ALL_BEADS_DONE.
Suggestion: route runtime read through readBeadsFile + canonical projection; make execution path 'fail' or refuse rewrite when malformedLines non-empty.

[Medium] G-04 step-cap still committed after mid-run opencode.json edit

Where: server/workflow/phases/executionPhase.ts:695
Evidence: excludePaths: stepsConfig ? [OPENCODE_CONFIG_FILENAME] : [] keyed on "applied this bead", not "restore record exists"; next bead commits working file carrying agent.build.steps.
Suggestion: key exclusion on any restore record for ticket, or strip agent.build.steps before staging model edit.

[Medium] G-07 exclude rule still in shared info/exclude, never removed

Where: server/git/repository.ts:130, server/workflow/phases/executionPhase.ts:350
Evidence: git rev-parse --git-path info/exclude shared with main checkout in linked worktrees; no removal on restore/boot.
Suggestion: drop rule (commit already excludes by path) or remove exactly that line on restore + boot.

[Low] G-14 follow-on: project-route errors regress 400 → 500

Where: server/routes/projects.ts:213,274
Evidence: normalizeFolderPath now throws ContainedPathError but /projects/check-git + /projects/ls call it outside try/catch with no global onError mapping (other routes map explicitly).
Suggestion: wrap both, return 400 with message.

[Low] G-30.6 exclusion case-sensitive on case-insensitive FS

Where: server/phases/execution/gitOps.ts:114
Evidence: normalizeRepoPath without case folding here; Opencode.JSON on win32/darwin misses exclusion.
Suggestion: compare case-insensitively on win32/darwin (same helper as worktreeChanges.ts:149).

[Third pass review]

Full independent review of the 8,338-line diff and 12 recent commits (f18297f..c5d1a1d) against the consolidated report G-01 through G-36.

Prior findings now resolved

The following G-findings are confirmed addressed with tests:

  • G-01: GIT_MUTATION_TIMEOUT_MS = 5min + runGitMutation async runner with SIGTERM→SIGKILL escalation, process-tree cleanup, descendant settlement. Two real-git tests verify no index.lock and no surviving hook descendant.
  • G-03: resolveContainedPath(projectRoot, '.') canonicalises the root before safeAtomicAppendWithin. Symlink-alias test added.
  • G-05: AD entries filtered from commit pathspec; staged additions are still cleared. Integration test covers the full AD→commit→no-op cycle.
  • G-06: GIT_FETCH_TIMEOUT_MS = 120_000 passed to remoteGitOptions and tryFetchOrigin.
  • G-08: Recovery markers (.recovery sidecar) with source+target identity, resumeMarkedCopy for interrupted copy-fallback, blockRecovery for unresolved ownership. Multiple containment tests.
  • G-09/G-10: Atomic proof (.proof sidecar with sha256+byteLength) required before YAML/JSONL temp promotion. YAML mappings without proof are left unpromoted.
  • G-11: safeAtomicAppend write loop with zero-progress detection. Short-write and zero-progress tests added.
  • G-12: assertContainedEntry inspects final entry via lstat without following it; outward and dangling symlinks are quarantined and removed. Integration test verifies both.
  • G-13: -z (NUL-delimited) readers in squash, integration candidate audit, setup storage, manual QA drift detection, statusPorcelain, and github.ts status. parseGitPathListZ exported. Opaque-path tests for tabs, newlines, spaces, backslashes, non-ASCII.
  • G-14: normalizeFolderPath now rejects relative/empty input, preserves / root, preserves POSIX backslashes, and only maps WSL drives when the mount exists.
  • G-15: assertSafeRefName in ref.ts rejecting leading -, .., control chars, @{, .lock components, duplicate dots, etc. Applied at all branch/ref entry points (push, github, repository, initialize, ticketMutations, focusedDiff).
  • G-16: readManualQaEventsWithDiagnostics uses safeParse, skips invalid shapes with a warning, preserves raw lines.
  • G-22: spawnSync wrapped in try/catch in runSyncRaw; NUL argv returns failure outcome. Test covers it.
  • G-23: Case-folding comparison in normalizeRepoScopedPath and isWithinRoot on darwin/win32; traversesSymlink rejects symlink ancestors while allowing legal final symlinks.
  • G-24: GIT_SSH_COMMAND='ssh -o BatchMode=yes' added when no SSH override is set, with core.sshCommand respected. Tests for both.
  • G-25: SQLite-backed withFileLock/withFileLockSync eliminates the stale-file-lock class entirely. Cross-process test with real child processes.
  • G-30.2: Symlink .tmp entries logged and skipped.
  • G-30.3: fsyncDirectory called after promotion/rename.
  • G-30.4: O_NOFOLLOW on copy-fallback target open.
  • G-31.2: SSH hostname cache no longer poisoned on transient failure.
  • G-31.3: isCommandAvailable('gh') shared helper replaces per-module probes.
  • G-32: Config-dir recovery added at startup (recoverOrphanTmpFiles(resolveAppConfigDir(), 'config')).
  • R-23/W-29.1: GIT_CONFIG_COUNT validation (rejects invalid, guards overflow); generated-file allowlist shared for cleanliness and bead commits.
  • W-17: tryDeleteRemoteBranch requires a 40-64 hex expected head and uses --force-with-lease with explicit refspec.
  • S-12(2): containedPath.ts rejects : after drive prefix on win32 while preserving POSIX colons.

New findings

1. [Medium] gitInvocation spawns an extra git config --get core.sshCommand on every git call — performance regression for multi-command sequences

Where: server/git/runCommand.ts:334-356 (gitInvocation)

The function calls runSyncRaw('git', ['config', '--get', 'core.sshCommand'], ...) on every invocation when no explicit SSH override is set. This spawns an additional git process per command.

During a squash operation (squash.ts), the command sequence is roughly rev-parse, merge-base, rev-list, diff, reset --mixed, N×add, diff --cached, commit, rev-parse — typically 9-16+ git commands. Each now doubles to 18-32+ process spawns. The bead commit path (gitOps.ts) and worktree initialization (initialize.ts) similarly run multiple sequential git commands.

Each probe has COMMAND_AVAILABILITY_TIMEOUT_MS (5 s) ceiling and typically completes in milliseconds, so this is a throughput concern rather than a correctness one. On high-latency or network-mounted filesystems the extra stat+open+read+close per git config probe adds measurable overhead.

Fix direction: Cache the core.sshCommand result per repository directory (or process-wide when absent). A Map<string, boolean> populated once per directory, invalidated only if GIT_SSH_COMMAND or GIT_SSH changes, would eliminate every subsequent probe. The ghIsInstalled helper already had this shape before it was replaced by isCommandAvailable.

2. [Low] withFileLockSync blocks the event loop under contention

Where: server/io/fileLock.ts:73-85 (sleepSync using Atomics.wait)

The synchronous lock path busy-waits via sleepSync(ms) which calls Atomics.wait(new Int32Array(new SharedArrayBuffer(4)), 0, 0, ms). This blocks the calling thread for each retry interval (default 25 ms) up to the timeout (default 5 s).

The sync path is used only from atomicAppend.ts for log/event appends. Under normal operation, contention is near-zero because SQLite BEGIN IMMEDIATE succeeds immediately on a free database. But if a slow holder (e.g., a large log rotation or a cross-process append during a GC pause) keeps the transaction open, the blocked thread cannot process timers, network I/O, or other pending callbacks until the lock is acquired or the timeout expires.

Mitigation already in place: The 5-second timeout bounds the block, and the sync path is used only for the append byte-range calculation (typically microseconds). This is acceptable for the current usage, but any future caller of withFileLockSync in a request handler should be aware of the blocking behavior.

3. [Low] assertSafeRefName test coverage gap: @{ pattern not tested

Where: server/git/__tests__/push.test.ts:713

The push test covers -bad, bad name, bad..name, bad//name, bad/.name, bad/name., bad.lock, bad.LOCK/child, @, and bad\0name. The @{ rejection (which prevents reflog-style injection) is not tested. A one-liner it.each addition covering 'bad@{now}' would close this gap.

4. [Low] normalizeFolderPath behavioral change: relative paths now throw instead of resolving against cwd

Where: server/storage/paths.ts:36-38

Previously normalizeFolderPath('relative/path') resolved against process.cwd(). Now it throws ContainedPathError('Project path must be absolute'). The route handlers (projects.ts) were updated with try/catch, and resolveGitRepoRoot/resolveGitRepoRootAsync handle the throw. The test suite covers the new behavior.

Any caller outside the routes (e.g., scripts, tests, or plugins) that passed a relative path will now see an exception instead of a silently resolved path. This is the correct security boundary behavior but is worth noting as a semantic change.

5. [Informational] isKnownArtifactTarget regex allowlist is a maintenance burden

Where: server/io/recovery.ts:150-174

Recovery only promotes temps whose target matches a known artifact name. Adding a new artifact type (e.g., a new manual-qa sub-artifact or a new runtime file) requires updating the regex list here, or the temp will be silently left behind. A brief comment in the allowlist explaining this contract — and a grep instruction for future artifact authors — would reduce the risk.

6. [Informational] promoteTmpFile writes the recovery marker twice with identical content

Where: server/io/recovery.ts:729-800

The hardlink-success path and the copy-fallback path both write the marker before the operation, then again after completion with replace: true. Both writes carry the same JSON payload (version, targetPath, source, target identity). The second write's purpose is atomic publication, but the marker itself carries no completion flag — boot-time recovery re-verifies the target content byte-by-byte via targetContainsSource. This is correct and safe; noting only for reviewer clarity.

Observations

  • The SQLite file lock is a significant improvement over the O_EXCL stale-file approach. The staleMs option and its TOCTOU window are completely eliminated.
  • The recovery marker/proof system correctly addresses G-08/G-09/G-10: interrupted YAML/JSONL writes are no longer promoted on a valid-prefix parse alone.
  • The async migration (sync→runGitMutation) for add, commit, checkout, reset, merge, and worktree remove is a substantial and well-tested change. The withGitIndexRollbackAsync counterpart correctly preserves the index snapshot semantics.
  • The removeWorktree revalidation (directory identity check after git worktree remove --force returns) is good TOCTOU mitigation against a parent-directory swap during the await.
  • Workflow metadata descriptions are updated consistently with the code changes. The test describes bounded Git, scoped recovery, and durable Manual QA behavior pins the key phrases.

Codex:

  • High — the runtime bead projection still bypasses the canonical reader. server/storage/ticketQueries.ts:1222-1255 reads raw JSONL rows and projects only camel-case keys, so snake_case aliases disappear; one malformed row also falls into the catch and returns an empty projection. Reuse the canonical bead reader with its diagnostics and skip only malformed entries, so a damaged row cannot hide every bead.

  • High — cleanup uses a stale plan when removing worktrees. server/cli/cleanCommand.ts:381-399 computes removable candidates once and passes them to removeWorktree without rechecking ownership, Git registration, closed-ticket state, or dirtiness. A worktree can become active or receive user data while planning is in progress. Revalidate those conditions immediately before each removal, or hold a cleanup lease.

  • High — an abandoned Git timeout can still be followed by recursive deletion. server/git/runCommand.ts:581 settles after the abandon grace period without confirming the child is gone, while server/git/worktreeRemoval.ts:82-113 falls back to recursive removal after Git fails. A stuck Git process or hook can still operate on the worktree while it is deleted. Use the fallback only after termination is confirmed; otherwise leave the path for recovery.

  • High — an unresolved OpenCode step-cap conflict can put opencode.json into a bead commit. When applyOpencodeStepsConfig returns a conflict, server/phases/executionPhase.ts:327-352,691-695 leaves stepsConfig unset, so reset preservation and commit exclusion no longer include the config. The sidecar deliberately leaves the file in place, and the next commit can capture LoopTroop’s cap settings. Keep the path excluded while the sidecar is unresolved, or stop the run until the conflict is resolved.

  • Medium — stored Git revisions are accepted as arbitrary strings and passed before --. server/phases/beads/beadsFile.ts:128 only checks that beadStartCommit is a string; server/phases/execution/gitOps.ts:255-260 and server/phases/manualQa/checkpoint.ts:248-255 then build Git revision arguments from it. A corrupted value beginning with a Git option can change what diff reads or writes. Accept only verified commit revisions, reject option-shaped values, and use --end-of-options where supported.

  • Medium — Manual QA event deduplication is outside the append lock. server/phases/manualQa/storage.ts:206-220 reads the existing events and checks the ID before appendJsonlWithin takes its file lock. Two processes can both observe a missing ID and append duplicates. Put repair, read, duplicate checking, and append under one lock.

  • Medium — synchronous log locking can block the server event loop. server/io/atomicAppend.ts:45-47 calls withFileLockSync, whose retry uses Atomics.wait in server/io/fileLock.ts:93-112; server/log/executionLog.ts:203 reaches it from request/SSE work. Another process holding the lock can freeze unrelated requests for the lock timeout. Use the async lock path or queue log writes.

  • Medium — index rollback can erase unrelated staged changes. server/git/indexSnapshot.ts:26-104 snapshots the whole index, awaits Git work in withGitIndexRollbackAsync, and restores the old copy on failure. If another actor stages a file during that await, the restore silently removes the newer staging. Hold an index lock across the operation or compare the index identity before restoring.

  • Medium — case handling disagrees on macOS. server/git/worktreeChanges.ts:150-158 folds path case only on Windows, while server/git/repoScopedPath.ts:23-27 also folds it on Darwin. On a normal case-insensitive macOS volume, a differently cased control directory can pass worktree classification and commit filtering. Share one volume-aware classifier and use it from server/phases/execution/gitOps.ts:114.

  • Medium — generated OpenCode ignore rules are never removed. server/phases/executionPhase.ts:352 adds a root opencode.json rule through server/git/repository.ts:130, but the restore path does not remove that LoopTroop-owned rule. After the generated file is removed, a future user-created root config can remain silently ignored. Track the rule’s ownership and remove only that rule after cleanup, or avoid persisting it in shared repository metadata.

  • Low — asynchronous Git paths still perform blocking availability probes. server/git/runCommand.ts:347 synchronously runs git config before an async Git child starts, and server/git/push.ts:30-43 synchronously probes gh through isCommandAvailable. A slow local executable or filesystem can block the daemon for the probe timeout. Cache the result or perform these probes asynchronously.

  • Low — recovery does not recognize the nested OpenRouter routing config target. scripts/dev.ts:47-52 writes opencode/openrouter-routing.json below the app config directory, but server/io/recovery.ts:39-43,368 allows only known config files at the root. A crash during that atomic write leaves the nested temp unrecovered. Register the nested target explicitly or give config recovery the known path.

Reuse the native canonical temporary-directory helper instead of the duplicate non-native resolver. Windows short aliases otherwise open a second project database connection, bypassing read-only and failure-injection fixtures.

Move real Git/database and module-mocking suites into isolated integration workers so Windows process probes and shared module state cannot contaminate the pure test pool. This changes test scheduling and fixture paths only; application behavior is unchanged. Six focused suites (38 tests) and lint passed on the shared fix.
Revalidate the original worktree entry after awaited Git removal, including birth time for filesystems that report weak inode values, so fallback cleanup cannot delete a replacement generation. Add an opt-in NUL-safe ignored-file inspection guard that fails closed and permits only LoopTroop-owned roots, covering both the Git removal and fallback paths while keeping explicit deletion behavior unchanged. Add regressions for pre-removal and post-yield ignored files, missing Git inspection, replacement targets, and successful conservative removal. The clean-command caller will opt into the guard for real Git worktrees.

Impact: conservative callers preserve user-owned ignored files; explicit ticket/project deletion remains destructive. Root-owned CLI preview/apply wiring is intentionally separate.

P1 Ignored Files Still Deleted

The new ignored-file protection defaults to off, and no production caller enables it. In particular, clean --apply uses ordinary git status --porcelain, so a worktree containing only ignored user files such as .env files is considered removable. It then calls removeWorktree without preserveIgnoredFiles: true, allowing those files to be recursively and irreversibly deleted.

Track detached Git and GitHub children so runtime close terminates and awaits them after closing HTTP admission, preventing mutations from outliving the daemon. Mark fallback targets incomplete until copied and fsynced, and refuse to truncate an edited same-or-smaller target or overwrite a completed target that advanced with newer bytes. Preserve contained final quarantine symlinks by validating only their parents and write collisions beside them. Record the confirmed regressions, shared ignored-file cleanup guard, native temporary fixture note, and deferred review findings in the user-facing docs and disposition ledgers.

Make terminal-ticket Free Disk Space cleanup use the shared ignored-file guard while leaving explicit project and ticket deletion destructive. Add a real linked-worktree regression proving an ignored blocked.env is preserved and LoopTroop-owned .ticket/runtime data is removable. Document the conservative app behavior and leave the frontend confirmation copy for its owning packet.

P1 Cleanup Can Partially Succeed

When several terminal worktrees are eligible, this checks ignored files only as each worktree is removed. If a later worktree contains a protected ignored file, earlier worktrees have already been irreversibly deleted before the operation returns an error. The error response omits the bytes already freed, so users cannot tell that cleanup partially succeeded. Validate all eligible worktrees before starting destructive removal.

Avoid running Git from a pre-start ticket skeleton, where Git would inherit the enclosing repository's ignored files and block safe cleanup or inspect the wrong scope. Verify real worktree roots with untrimmed rev-parse output, stripping only Git's final terminator so trailing-space paths remain valid. Permit only a direct .ticket skeleton without other entries and fail closed for unknown files. Add parent-ignored, skeleton-owned, and trailing-space regressions, and document the conservative behavior.

Review by Claude Opus 5 — round 3

Round-2 status: (1) detached-child shutdown — fixed (stopActiveCommands + awaited in createRuntime.close, covered by tests/createRuntime.test.ts); (3) hookValidation, (4) G-03 mock, (5) symlink-ancestor callers, (6) append-lock/dead sync path — deferred or withdrawn per the ledger, no new evidence against those calls; (2) SSH probe — not fixed, see H-2.

C-1 (Critical) — preserveIgnoredFiles makes "Free disk space" fail for essentially every real project

server/git/worktreeRemoval.ts:114-124 + server/storage/projects.ts:616 (commit b6abebc0)

assertNoIgnoredWorktreeFiles refuses removal when any ignored entry classifies as something other than looptroopExcluded. But classifyWorktreePath has four categories, and the two that matter here — generatedNoise (node_modules, dist, build, coverage, .venv, target, *.log, …) and setupExcluded (.cache/project-tooling and every execution-setup temp root LoopTroop itself created) — are not looptroopExcluded, so they trip the refusal.

deleteProjectWorktrees passes preserveIgnoredFiles: true and does not catch, so one such worktree aborts the whole loop; DELETE /projects/:id/worktrees (server/routes/projects.ts:433-438) returns 500 and frees nothing — including the worktrees that were fine.

Reproduced on this branch:

$ git -C wt ls-files --others --ignored --exclude-standard --directory -z
node_modules/
$ npx tsx -e "import {assertNoIgnoredWorktreeFiles} from './server/git/worktreeRemoval'; assertNoIgnoredWorktreeFiles('…/wt')"
Error: Refusing to remove a worktree with ignored files outside LoopTroop roots: node_modules/

A completed ticket whose execution setup ran npm install is the normal state, so the feature is broken for the common case, not an edge one. The new test only covers blocked.env (category committable), which is why it passes.

Fix: the guard should refuse only on committable ignored entries — generatedNoise and setupExcluded are exactly the disposable bytes this endpoint exists to reclaim:

.filter((path) => classifyWorktreePath(path, { untracked: true }).category === 'committable')

and deleteProjectWorktrees should skip-and-continue (reporting the skipped worktree) rather than abort the loop, so one protected checkout doesn't block cleanup of all the others. Please add a regression with an ignored node_modules/ and a .cache/project-tooling/ entry asserting the worktree is removed.

H-2 — core.sshCommand probe still runs a blocking spawnSync inside the async runners

server/git/runCommand.ts:387-415, reached from runGit (:735), runGitMutation, runGitSync, runGitBinarySync — uncached, once per git call.

The ledger defers this as "performance", but the cost is not only latency: gitInvocation is called from runGit/runGitMutation, whose entire stated contract in this file's header is "these are the calls that can block for the entire timeout, so they must not sit on the event loop." Every async fetch/push/ls-remote/commit now synchronously blocks the event loop — and with it HTTP and SSE for every ticket — on a child spawn before it starts.

Measured on this host, 50 iterations: git config --get core.sshCommand 412 ms vs git rev-parse HEAD 394 ms — the probe costs as much as the command, i.e. ~2x on every git call.

Fix is small: memoise per realpath(gitdir) (or per directory) in a Map, and invalidate never — core.sshCommand does not change under a running daemon; a single WeakMap/Map lookup removes both the latency and the event-loop block without changing the preserved-SSH semantics the regression test asserts.

Antigravity Review

Verification Summary of Recent Commits (e9e1dcb5, e1d3e4ae, b6abebc0, f4b422aa)

The following previous issues were verified as resolved:

  • Mid-Copy Fallback Recovery (G-08): In server/io/recovery.ts, fallback copy now writes { ..., complete: false } before the copy and { ..., complete: true } after fsync. Incomplete targets matching the source prefix are now safely resumed with copyValidatedSource(..., truncate=true) instead of blocking recovery.
  • Missing Fsync on Truncation (G-10, G-11): Both small-file and large-file truncation paths in fixTrailingLineCorruption now call fsyncSync(fd) immediately following ftruncateSync.
  • Orphaned .proof Sidecars (G-09): scanDir in server/io/recovery.ts now explicitly recognizes .tmp.proof files and unlinks them when the sibling .tmp file is absent.
  • Directory Substitution on Windows (ino === 0): server/git/worktreeRemoval.ts now captures birthtimeMs in EntryIdentity and validates entry identity before fallback directory removal.
  • Preservation of Ignored Files & Skeleton Cleanup: removeWorktree supports preserveIgnoredFiles using NUL-delimited ls-files inspection, invoked by terminal-ticket cleanup. Pre-start skeletons without .git are verified to contain only .ticket without inheriting enclosing repository ignore rules.
  • Manual QA Quarantine Links: server/phases/manualQa/checkpoint.ts now keeps existing final symlinks intact and writes retry copies (.attempt-${suffix}) beside them.
  • Process Teardown on Daemon Close: Detached child processes are now tracked and cleanly terminated/awaited during createRuntime.close().

Remaining Critical & High Severity Issues

1. Unconditional 4-Second Timeout Penalty and Immediate SIGKILL Race (G-01)

  • Location: server/git/runCommand.ts:640-687
  • Problem:
    In runAsyncRaw:
    const timer = setTimeout(() => {
      if (overranBuffer) return
      timedOut = true
      terminateProcessTree(child, 'SIGTERM')
      killTimer = setTimeout(forceKill, TIMEOUT_KILL_GRACE_MS)
      abandonTimer = setTimeout(() => {
        timeoutCleanupComplete = true
        child.unref()
        settle(...)
      }, TIMEOUT_KILL_GRACE_MS + TIMEOUT_ABANDON_GRACE_MS)
    }, timeoutMs)

    const settle = (outcome: RawOutcome<string>) => {
      if (settled) return
      if (timedOut && !timeoutCleanupComplete) {
        pendingTimeoutOutcome ??= outcome
        forceKill()
        return
      }
      settled = true
      ...
    }
    When an async Git/GH command times out, SIGTERM is sent. When Git terminates in response to SIGTERM (usually within 10–50ms), child.on('close') fires and calls settle(outcome).
    1. forceKill() is executed immediately on close, sending SIGKILL without waiting for the 2,000ms TIMEOUT_KILL_GRACE_MS grace period.
    2. settle returns without settling the promise.
    3. Because child has closed, no further process events fire. The promise remains pending unconditionally for 4,000ms until abandonTimer expires (TIMEOUT_KILL_GRACE_MS + TIMEOUT_ABANDON_GRACE_MS).
    4. On Windows, where taskkill /T /F kills the process tree immediately, every timed-out operation is unconditionally penalized with this 4-second hang.
  • Remediation:
    Do not stall until abandonTimer when no processes remain in the tree. Check whether child descendants still exist in the process group. If none remain, settle immediately. If descendants exist, allow killTimer to expire naturally before escalating to SIGKILL.

2. Restore Artifacts Omitted from Recovery Allowlist (G-30.1, G-32)

  • Location: server/io/recovery.ts:46-51, server/io/recovery.ts:364-385

  • Problem:
    KNOWN_TICKET_ROOT_ARTIFACTS remains restricted to:

    const KNOWN_TICKET_ROOT_ARTIFACTS = new Set([
      '.gitignore',
      'interview.yaml',
      'prd.yaml',
      'relevant-files.yaml',
    ])

    LoopTroop writes internal restore sidecars into ticketDir using safeAtomicWriteWithin:

    • opencode-steps-restore.json (server/phases/execution/opencodeStepsConfig.ts:29): Stores original user credentials and configuration while steps are capped.
    • hook-validation-restore.json: Stores worktree snapshot state during hook execution.

    If a crash occurs while writing either file, recoverOrphanTmpFiles encounters opencode-steps-restore.json.<pid>.<hash>.tmp and logs:
    [recovery] Leaving ...: target is not a known LoopTroop artifact
    The temporary file and its .proof sidecar are abandoned and never promoted, permanently losing the restore artifact and leaving the capped configuration unrecoverable.

  • Remediation:
    Add 'opencode-steps-restore.json' and 'hook-validation-restore.json' to KNOWN_TICKET_ROOT_ARTIFACTS.

3. Incomplete Staged Deletion Filter Breaks Commit on Renamed/Copied Files (G-05)

  • Location: server/phases/execution/gitOps.ts:119-122
  • Problem:
    In commitBeadChanges:
    const commitEntries = committableEntries.filter(entry => !(entry.indexStatus === 'A' && entry.worktreeStatus === 'D'))
    const committableFiles = commitEntries.map(entry => entry.path)
    The filter exclusively excludes additions deleted in worktree (indexStatus === 'A' && worktreeStatus === 'D').
    If a file was renamed in the index (indexStatus === 'R') or copied (indexStatus === 'C') and the destination path is subsequently deleted from disk before the bead commit, the entry has status RD or CD.
    1. committableFiles retains <newPath>.
    2. git add -- <newPath> deletes or unstages <newPath> from the index.
    3. git commit -m ... -- <newPath> fails with:
      error: pathspec ':(literal)<newPath>' did not match any file(s) known to git
      because <newPath> never existed in HEAD and is present in neither the index nor the worktree.
    4. withGitIndexRollbackAsync catches the error, rolls back the index, and the cycle repeats on every retry.
  • Remediation:
    Exclude all worktree-deleted entries that do not exist in HEAD (entry.worktreeStatus === 'D' && ['A', 'R', 'C'].includes(entry.indexStatus)).

4. Pre-Creation Torn Marker Permanently Wedges Daemon Boot (G-08 Edge Case)

  • Location: server/io/recovery.ts:776-785, server/io/recovery.ts:610-625
  • Problem:
    In promoteTmpFile:
    if (!existingMarker) {
      guard(markerPath, true)
      writeRecoveryMarkerWithDeps(markerPath, { version: 1, targetPath, source }, false, deps, guard)
    }
    retryWhileWindowsHoldsTheFile(() => {
      guard(targetPath, true)
      targetFd = openSync(
        targetPath,
        fsConstants.O_WRONLY | fsConstants.O_CREAT | fsConstants.O_EXCL | (fsConstants.O_NOFOLLOW ?? 0),
        source.mode & 0o777,
      )
    })
    The initial marker is published on disk at line 778 before targetPath is created by openSync.
    If a crash or power cut occurs between line 778 and line 796 (before the target identity is acquired and written into the marker):
    On the subsequent daemon startup:
    1. resumeMarkedCopy is invoked. Because marker.target is undefined, lines 610–625 execute:
      if (!marker.target) {
        ...
        complete = targetContainsSource(fd, source, targetFd, target)
          && sameEntryIdentity(lstatSync(targetPath), target)
        ...
        if (!complete) {
          return blockRecovery(
            targetPath,
            tmpPath,
            'its prepared fallback target is incomplete or changed',
          )
        }
      }
    2. Because targetPath was either not yet created or only partially opened, complete evaluates to false.
    3. blockRecovery throws RecoveryBlockedError.
    4. In server/startup.ts:64, RecoveryBlockedError is rethrown, terminating startupSequence().
      The daemon enters a permanent crash loop on every restart until the operator manually removes the .recovery marker file from disk.
  • Remediation:
    Do not write an initial marker without a target identity before openSync. Create targetFd first with O_CREAT | O_EXCL, read its fstatSync(targetFd), and write { version: 1, targetPath, source, target, complete: false } as the very first marker publication.

Codex:

Very important — a failed worktree enumeration is treated as an empty registration set. server/cli/cleanCommand.ts:59-81 maps any git worktree list failure to an empty set, then :194-210 can classify a still-registered clean worktree as unregistered and :425 removes it. Keep the result nullable and refuse candidate classification/removal whenever registration cannot be read.

Very important — clean --apply can delete ignored user files. server/cli/cleanCommand.ts:313-323 does not enable preserveIgnoredFiles; server/git/worktreeRemoval.ts:113-140 therefore never runs its ignored-file guard because the option defaults to false. An ignored file can leave git status --porcelain clean and then be removed by the recursive fallback. Pass the preservation guard for Git worktrees, or make the safe default fail closed when the ignored-file check cannot complete.

Very important — a timed-out Git operation can still reach recursive deletion. server/git/runCommand.ts:624-687 settles after SIGTERM/SIGKILL and an abandon grace without observing process termination. server/git/worktreeRemoval.ts:178-210 treats that timeout like an ordinary Git failure and recursively removes the worktree. A surviving hook, filter, or descendant can still mutate the path while it is being deleted. Do not run the recursive fallback on an unverified timeout; retain the path or require confirmed termination.

Very important — shutdown can return while detached Git descendants still run. server/git/runCommand.ts:60-102 treats a direct child’s exitCode or signalCode as a completed shutdown even when close is still blocked by a descendant holding stdio. server/createRuntime.ts:205-213 then lets runtime shutdown finish without escalating that process group. Keep the group in the survivor set until close or the full escalation window completes before resolving stopActiveCommands().

Muse Spark review (missed-items sweep, head f4b422aa):

[High] Shutdown drains run before the HTTP server closes, so post-drain children still outlive close()

Where: server/createRuntime.ts:193-213 (close(), commit e1d3e4ae)
Evidence: close() initiates handle.close() (~199) but awaits it last (213), after await stopActiveCommands() (208), await pollerStopped (209), and the second drain (212). Any handler admitted before the listener closed can spawn a Git/gh child while await serverClosed is pending — after both drains ran — and close() then resolves with that mutation still writing. That is the exact survival e1d3e4ae claims to prevent, and it cascades: a write landing after close races next boot's resumeMarkedCopy, which fails closed and turns an unclean shutdown into a startup-blocking operator incident. Both added tests start the child before close()/drain, so neither exercises the admitted-during-close window.
Suggestion: await serverClosed (and pollerStopped) before the drains, and drain once more after — close admission fully, then kill/await children, with no spawn window after the final drain.

Keep ignored-file protection for terminal-ticket housekeeping while continuing past individual refusals. Return skipped ticket IDs and reasons, count only successfully removed worktrees, and keep the dialog open with the partial result. Label the preview as total size because protected checkouts may remain.

Add storage, route, and dialog regressions for mixed eligible/protected worktrees and preserve explicit ticket/project deletion behavior. Focused storage/routes/UI suites and ESLint pass; aggregate verification follows with the shared cleanup changes.

Source changes and their focused regression coverage are committed separately by the filesystem review owner.
Preserve trusted repository SSH configuration with a fresh asynchronous probe for every Git invocation, and surface Windows taskkill completion so timed-out commands are retained only when cleanup remains unverified. Keep closed-child PIDs fail-closed, retry runtime drains after shutdown errors, and wait for both poller and server closure. Tighten BigInt worktree identity checks, skeleton-only cleanup guards, timeout handling, source-only marker recovery, and staged rename/copy deletion cleanup. Refresh the README and PR164 disposition evidence, including POSIX descendant and Windows cleanup regressions.

P1 Windows Cleanup Never Recovers

On Windows, cleanup is confirmed only when taskkill exits successfully. If taskkill returns a nonzero status because the child exited before its PID was inspected, the already-closed child remains permanently tracked. Later shutdown attempts skip signaling that child but can never remove it, so every close() attempt fails and the daemon exits with a shutdown error even though no process remains.

Stop accepting new SSE clients before closing the HTTP listener, then close every registered stream so Node server.close can finish even when browsers hold long-lived responses. Add an admission gate reset for each runtime generation and reject a stream that reaches registration after shutdown begins. Keep replay buffers intact, catch transport-close errors, document the shutdown behavior, and cover the broadcaster, route, late-admission, and real HTTP runtime paths.
Record that graceful runtime shutdown closes active browser streams before the HTTP listener waits and rejects late stream admission, preventing long-lived SSE connections from holding the daemon open.
Comment thread server/createRuntime.ts

P1 Runtime Shutdown Closes Other Streams

When an embedded process runs multiple runtimes, they all share the same global SSE broadcaster. Closing one runtime therefore closes SSE streams served by every other live runtime, while starting another runtime can reopen stream admission during the first runtime's shutdown. Scope SSE clients and admission state to each runtime so one runtime cannot disrupt another.

Preserve the release audit documentation and test isolation while carrying the Git/filesystem changes onto the squashed main history.
