Summary

On Windows, #6639 broke the Claude intercept local CA for any install that already had a CA directory, and made every CA load roughly 20 PowerShell process launches long. This PR keeps #6639's ownership, symlink, hard-link and ACL guarantees and fixes both problems.

Existing directories and files were refused. Releases before #6639 created <OPENCODEX_HOME>/claude-intercept with mkdirSync(…, 0o700). Windows ignores that mode, so the directory and its ca.key, ca.pem and lock database kept the DACL they inherited from the profile. #6639 required a pre-existing directory and each entry to already pass the strict protected-DACL inspection before any hardening, so upgraded Windows installs got local_ca_path_unsafe. The intercept guide says the main intercept stays available on Windows with ACL checks.

An existing directory or entry that fails the strict check now gets the existing inherited inspection first. That inspection requires the current user as owner and allows only current-user, SYSTEM and Administrators grants. Only after it passes is the entry hardened and then strictly re-verified. A foreign owner or any foreign Allow grant is still refused, with no ACL mutation. Directories that already pass the strict check are no longer re-hardened on every load.

Every check launched PowerShell. assertLocalCaWindowsAcl runs Get-Acl in a new PowerShell process, at about 0.5–1 s each on windows-latest. One load re-inspected the same unchanged files and the SQLite journal about 22 times, synchronously during server startup, because Claude intercept is on by default.

Verified ACL results are now remembered by file identity (bigint dev, ino and creation time), at the level each was verified, for the duration of one withLocalCaPublication call. Identities this code unlinks or replaces are retired. Every memo hit still runs the lstat and descriptor identity checks.

Entries this process just created with O_CREAT|O_EXCL, or with a successful mkdir, skip the redundant owner pre-check. They are narrowed while still empty, and the strict check after narrowing still enforces the current user as owner before any secret is written. By reading the code, a CA load drops from about 22 Get-Acl launches to 5, on both fresh and existing homes.

PowerShell cmdlet autoload stalled the check in real ocx start processes. Temporary timing instrumentation on a throwaway CI branch (run 37381976691) showed the cause of the remaining 30 s startup stalls. Inside ocx start children, the first Get-Acl call on the new claude-intercept directory ran until its 30 s budget expired, and the CA was then refused as local_ca_path_unsafe. The .NET-only principal lookup in the same process took about 200 ms, and each icacls harden about 50 ms.

Get-Acl, ForEach-Object and ConvertTo-Json are module-autoloaded cmdlets. PowerShell 5.1 resolves them through the per-profile module analysis cache under %LOCALAPPDATA%. A fresh or redirected profile makes PowerShell rebuild that cache by scanning every installed module, which exceeds the budget. The inspection script now uses only .NET types: DirectorySecurity or FileSecurity with Owner and Access sections, and JSON built with a [char]34 quote. It returns the same owner, protection and SID-rule data with no cmdlet autoload, and a module on PSModulePath can no longer shadow Get-Acl.

Picker records on Windows. picker-ca-persistence.ts relied on O_NOFOLLOW, which does not exist on Windows. A symlinked authority-init.json was therefore followed and failed closed as picker_ca_metadata_invalid instead of picker_ca_metadata_unsafe. It now checks with lstat first and refuses a symlink or a mismatched opened identity as unsafe. Picker mode is macOS-only in production.

The startup preparation diagnostics… test now compares only the picker diagnostics. It still checks every captured warning for private-key text. On Windows it had also captured an unrelated soft-fail icacls timeout warning from hardenSecretDirAsync(dir, { required: false }) in src/config/paths.ts.

These causes account for the red Windows shards on dev run 37360604391. #6639's PR CI skipped the Windows shards, so none of this ran on Windows before merge. The shard runner stops at the first failing batch, so the dev run showed only the first failure in each shard. Later batches surfaced on this branch.

  • windows 9/9: local CA ignores predictable predecessor temp names… pre-creates the directory and failed at withLocalCaPublication. The picker-store symlink test failed in a later batch.
  • windows 7/9: fresh production restarts retain fingerprint… loads the intercept CA and the picker CA in a child process with a 10 s limit and timed out. The diagnostics test failed on the stray warning above.
  • windows 5/9 and 7/9: the sibling-home and start-journal tests wait for runtime publication, which waits for startServer. startServer synchronously runs the CA load.

Verification

  • No local tests were run, per owner instruction for this landing round. Evidence is hosted CI at the exact head, including a full-matrix ci.yml run with lane=all (linked in a comment).
  • bun run typecheck passed.
  • bun scripts/file-size-ratchet.ts passed.
  • bun run structure:check passed.
  • bun run privacy:scan passed.
  • git diff --check is clean.
  • An independent security review is recorded in PR comments and is bound to the final head.

Checklist

  • Scope stays focused and avoids unrelated cleanup.
  • Docs or release notes were updated when needed. (structure/clients/claude-desktop.md describes the migration, the memo, and the picker record checks.)
  • Security-sensitive changes were reviewed for secrets, auth, and unsafe defaults. (Credential-file ACL policy; see the security review comments.)

Summary by CodeRabbit

  • Security
    • Windows certificate files and directories now receive stricter ownership and access checks. Existing files with trusted inherited permissions can be secured, while unexpected owners or access grants are rejected.
    • Picker certificate metadata is rejected if it points to a symbolic link or changes between inspection and opening.
  • Bug Fixes
    • Improved Windows permission handling for certificate files, directories, and related database files.
    • Picker setup avoids writing credentials when certificate metadata is unsafe.

📝 Walkthrough

Walkthrough

Windows local-CA publication validates ACL ownership and grants before hardening eligible inherited permissions. It caches ACL verification by file identity during each publication and checks identity around ACL inspection. Picker CA metadata reads reject symlinks and mismatches between path metadata and the opened file.

Changes

Local CA ACL handling

Layer / File(s) Summary
Directory and file ACL validation
src/claude/intercept/local-ca-files.ts, src/claude/intercept/local-ca-windows.ts, tests/claude-integration/claude-intercept-local-ca-windows.test.ts, structure/clients/claude-desktop.md
New entries are hardened and privately checked. Existing entries that fail private inspection must pass inherited-grant inspection before hardening and reinspection. The Windows ACL runner reads owner and access-rule data through .NET security objects. Tests cover trusted inherited ACLs and rejection of foreign owners or grants.
Publication-scoped ACL checks
src/claude/intercept/local-ca-files.ts, tests/claude-integration/claude-intercept-local-ca-windows.test.ts
The checker caches ACL levels by device, inode, and birth time and verifies identity around inspection. Reads, staging, locks, entry validation, and SQLite sidecars use the checker. Tests cover cache scope, staged identities, sidecars, and substituted key identities.

Picker CA metadata validation

Layer / File(s) Summary
Metadata path identity checks
src/claude/intercept/picker-ca-persistence.ts, tests/claude-integration/claude-picker-ca-store.test.ts, structure/clients/claude-desktop.md
readState checks path metadata before opening and rejects symlinks or device and inode mismatches. The race test verifies that replacement metadata fails before credential-store access.

Startup test adjustments

Layer / File(s) Summary
Startup warning and wait assertions
tests/claude-integration/claude-picker-startup.test.ts, tests/cli/sibling-home-client-sync.test.ts
The startup warning assertion filters for entries containing “Claude Desktop picker.” Runtime and client startup waits use 25-second deadlines instead of 15-second deadlines.

Priority: ➖ Normal

Estimated code review effort: 3 (Moderate) | ~25 minutes

Change: Bug fix

Merge Risk: 🔵 Low · up to 2606b

The sibling-home startup test can fail prematurely on slower runs, creating a localized CI reliability risk; align both waits to the test’s 30-second budget. This does not affect production behavior.

Security Architecture Review

Security architecture risk: 🔵 Low · up to 2606b

The changes preserve current-owner and trusted-principal restrictions while restoring eligible Windows installations and reducing repeated checks. No new unauthorized-access path was established. Risk remains low rather than minimal because authorization reuse depends on trusted principals not changing permissions during publication, and native Windows behavior was not exercised in this review.

Retained concerns
No architecture-level concerns identified.

Security review details

Security Blast Radius

  • inferred — The reviewed sensitive scope is local CA key/certificate publication and config-bound picker credential recovery. Compromise would affect the authority material associated with those local paths or configuration identities; the supplied relationships do not establish a new cross-service or environment-wide propagation path.

Security Findings and Attack Paths

  • observed — Foreign ownership or grants on existing inherited CA entries fail the migration gate before hardening. Changed file identity triggers fresh inspection rather than inheriting cached authorization. The corresponding fault-injection tests assert rejection before secret access or writes.

Trust Boundaries and Controls

  • inferred — Authorization reuse relies on the verified directory and file ACLs excluding untrusted writers. Identity pinning is not permission-change detection: an already trusted owner, SYSTEM, or administrator could change the ACL on the same object after memoization. No newly empowered untrusted principal was established, but this trust assumption bounds the cache guarantee.

Resilience and Maintainability Implications

  • observed — Local-CA reads retain descriptor identity, link-count, size, and post-read timestamp checks. Temporary cleanup checks identity before unlinking and preserves the original failure rather than deleting a substituted path. These controls continue to contain substitution and partial-publication failures despite reduced ACL inspection frequency.

Independent security review (gpt-6.1-sol, static review only, no local tests per owner instruction), bound to head 69a3291:

SECURITY REVIEW VERDICT: PASS at 69a3291, with no remaining findings.

Review history:

  • 6d245c5: PASS, two low findings. First, an exact dev/ino reuse could satisfy a stale per-publication memo. Second, the entry refusal tests did not assert that no hardening happened. Both were folded in f2914a7: inherited-level results are never memoized, identities this code unlinks or replaces are retired, and the refusal tests now assert no hardening.
  • f2914a7: PASS, two low findings. First, the temp identity was not retired on the ENOENT cleanup path. Second, the hardener caches could hide a hardening call in the refusal tests. Both were folded in 69a3291.
  • 69a3291: PASS. Migration runs the owner and allowed-principal check before any ACL mutation, and strict verification after it is still mandatory. Foreign owners and grants are refused without mutation. The memo is local to one publication, and identity checks run on every access. Error text is still sanitized, and POSIX behavior is unchanged.

Independent security review (gpt-6.1-sol, static only, no local tests per owner instruction), bound to head 788c3e1:

SECURITY REVIEW VERDICT: PASS at 788c3e1.

  • Skipping the owner pre-check for entries this process created with O_EXCL or mkdir is safe. The entry is still empty while it is narrowed, hardening does not change ownership, and the strict owner and ACL check runs before any secret write. A foreign or Administrators default owner fails closed.
  • The publication-local memo keyed by dev:ino:birthtimeNs is acceptable against the stated attacker, and identity checks run on every access.
  • Picker metadata reads now also reject symlinks and a mismatched opened identity. POSIX behavior is unchanged, and errors are still sanitized.
  • One low, nonblocking finding: the code comment overstates creation-time uniqueness, because NTFS tunneling can restore creation times and file IDs can be reused. The tuple adds discrimination; it does not guarantee replacement detection. This is wording only, and the memo's safety rests on the verified-private DACL.

Independent security review (gpt-6.1-sol, static only, no local tests per owner instruction), bound to head d223603:

SECURITY REVIEW VERDICT: PASS at d223603, with no new findings.

  • The .NET-only ACL read preserves owner, protection status, and explicit and inherited SID rules. Object ACEs were omitted by Get-Acl's GetAccessRules as well, so this is an existing limitation, not a regression.
  • No path or account name enters the JSON. Only canonical SIDs, cast integers and literal booleans are concatenated, and the TypeScript validator rejects anything unexpected.
  • Constructor and method exceptions terminate the script with a non-zero exit, and the caller rejects failures, timeouts, oversized output and malformed output.
  • The -Command argument contains no double quotes, and the path is still passed as base64 data.
  • The earlier low wording finding about creation-time uniqueness is fixed in this head.

Use one deadline that matches the test’s 30-second budget.

If runtime publication occurs in the first five seconds, the second helper’s fresh 25-second deadline can expire before the test timeout, even when startup could finish within the test budget. The cited late-publication case only delays the helper diagnostic: startup that needs more than five additional seconds cannot finish within the 30-second budget. A shared 25-second deadline could also reject startup that would finish between 25 and 30 seconds.
