Summary

This PR replaces #6695 by @ismell0992-afk with the design the maintainer said was acceptable on that PR: a preflight of the selected runtime that refuses with guidance and never discovers an alternate runtime.

Under a Windows app-sandbox or WDAC-style policy, the same Bun bytes can write the profile from one path and get EPERM from another. A durable launcher that bakes in the denied path crash-loops (#6695 observed about 4,700 Task Scheduler restarts).

  • Preflight. New src/lib/bun-runtime-preflight.ts. Before a Windows service install or repair, or a Codex shim install or refresh, changes anything, the selected runtime itself runs with argv and no shell, under a 5 s timeout. It creates and removes a uniquely named probe entry in the config directory and acknowledges a nonce. There is no memoization. A standalone selection is probed in-process only when it is exactly process.execPath; any other standalone selection refuses.
  • Refusal. OCX_RUNTIME_PREFLIGHT_FAILED (reason spawn / timeout / create / remove / protocol) with guidance: reinstall through a policy-permitted npm global install (npm install -g @bitkyc08/opencodex), or select a trusted Bun with OPENCODEX_BUN_PATH before running ocx. Selection and provenance in bun-runtime.ts are unchanged, and no runtime is discovered from PATH or install locations.
  • Ordering. An orchestrated install admits the runtime once, before the existing service is stopped. The frozen selection is passed to internal commit functions, the service script and WinSW rendering, and the install record, so no second check can refuse after a stop. Direct installer calls keep their own admission. On a first-time scheduler install, the check runs after the task is registered and the config root is claimed, immediately before anything is stopped; a refusal rolls back that new registration. (Probing earlier would create the root before ownership initialization and break fresh ownership claiming.) The preflight never deletes the config root.
  • Startup. Shim auto-restore on a refused preflight returns deferred with guidance, and startup continues. Other platforms make zero spawns and zero filesystem changes.
  • Docs. structure/runtime.md and structure/ops/service-and-sidecars.md are updated, along with the Windows troubleshooting page in English and seven translations.

Verification

  • bun scripts/test.ts on the three new files (tests/lib/bun-runtime-preflight.test.ts, tests/service/service-runtime-preflight.test.ts, tests/codex-integration/codex-shim-runtime-preflight.test.ts) plus tests/service/service.test.ts, tests/service/winsw.test.ts, tests/ci-workflows/bun-runtime.test.ts, all tests/codex-integration/codex-shim*.test.ts, and the layout and file-size ratchet tests: 564 pass / 24 skip / 0 fail. The skips are the existing pwsh shim cases.
  • bun run typecheck, bun run structure:check, and bun run privacy:scan all pass.
  • Tests cover denial before any stop, staging, removal, download, token, XML, or state write; a composed install where the stop happens before a would-be second preflight; selection freezing; spawn, timeout, nonce, leftover, and absent-root cases; standalone identity mismatch; and non-Windows zero side effects. They reuse the blocker-file fixture from fix(service): write-probe bundled Bun before baking it into durable launchers #6695.
  • The full local suite was not run (several worktrees were sharing the machine). Hosted CI covers the Windows shards. Native WDAC behavior is not reproducible in CI.
  • Independent gpt-6.1-sol review ran two rounds; both findings were fixed, and the final verdict is PASS.

Checklist

  • Scope stays focused and avoids unrelated cleanup.
  • Docs or release notes were updated when needed.
  • Security-sensitive changes were reviewed for secrets, auth, and unsafe defaults. (The runtime trust contract is unchanged: there is no new discovery or persistence path.)

Summary by CodeRabbit

  • Documentation
    • Updated Windows guidance on selecting a runtime and applying it to an installed service.
    • Explained that service and Codex shim operations may be blocked when the selected runtime cannot create and remove a directory in the configuration folder. Automatic shim repair is deferred with guidance while startup continues.
    • Clarified that a failed first-time scheduled service setup is rolled back, while the configuration folder is retained for retry.
  • Bug Fixes
    • Service and shim operations now consistently use the selected runtime and do not switch to another runtime if checks fail.

📝 Walkthrough

Walkthrough

Windows service and Codex shim operations now check whether the selected Bun runtime can create and remove a directory under the configuration root. The selected runtime is passed through service artifacts, shim wrappers, and install state. Documentation and tests describe admission, refusal, and deferred shim restoration.

Changes

Windows runtime write preflight

Layer / File(s) Summary
Runtime admission probe
src/lib/bun-runtime-preflight.ts, tests/lib/bun-runtime-preflight.test.ts, scripts/test-layout/layout.json, tests/fixtures/test-layout-expected.json
Adds a Windows-only probe that classifies spawn, timeout, create, remove, and protocol failures. It validates the configuration root and limits cleanup to the probe’s matching empty directory. Tests cover refusal cases, non-Windows no-I/O behavior, and filesystem changes during the probe.
Service install and repair admission
src/service/orchestration.ts, src/service/windows-ops.ts, src/service/repair.ts, src/service/state.ts, src/lib/winsw.ts, tests/service/service-runtime-preflight.test.ts, tests/service/service-desktop-runtime-preflight.test.ts, tests/service/service-ownership-handover.test.ts, tests/service/service.test.ts, structure/runtime.md
Service installation and repair select and freeze a runtime before their relevant side effects. Windows service assets and install state receive that runtime. Tests check refusal ordering, runtime reuse, and persisted selection.
Codex shim admission and runtime reuse
src/codex/shim.ts, tests/codex-integration/codex-shim-runtime-preflight.test.ts
Shim installation and refresh pass the selected runtime into wrapper generation. Healthy and disabled shims skip selection and probing. Automatic restoration defers on RuntimePreflightError; tests check refusal, guidance, and runtime reuse.
Runtime selection and preflight guidance
docs-site/src/content/docs/*/troubleshooting/windows-memory.md, structure/ops/service-and-sidecars.md
Updates Windows troubleshooting guidance in eight locales. The pages describe when to set OPENCODEX_BUN_PATH, how ocx service repair records the selection, when operations refuse, and the stated recovery options. Operational documentation describes the probe contract and runtime-selection handling.

🎯 Functional Correctness | 🟡 Minor | ⚡ Quick win

Spawn errors other than ETIMEDOUT are all reported as spawn, including ENOBUFS.

maxBuffer: 1024 is set at Line 113. Suppose the selected runtime prints more than 1024 bytes to stdout. For example, a Bun banner or a deprecation notice could do this. spawnSync then sets child.error.code to ENOBUFS. Line 117 maps that to spawn. That is a different failure kind: the executable started, but its acknowledgment was oversized. The retrieved learning says to keep host spawn errors separate from a child that started but returned an untrustworthy result.

Map ENOBUFS to protocol so the diagnostic is correct.

Proposed fix
   if (child.error) {
-    throw new RuntimePreflightError((child.error as NodeJS.ErrnoException).code === "ETIMEDOUT" ? "timeout" : "spawn");
+    const code = (child.error as NodeJS.ErrnoException).code;
+    throw new RuntimePreflightError(code === "ETIMEDOUT" ? "timeout" : code === "ENOBUFS" ? "protocol" : "spawn");
   }

Based on learnings: "host-side spawn/pipe/write/read/wait errors should be classified as a 'host error', while 'child process crash' should be reserved for a child that started successfully but exited without producing a trustworthy result."

Preserve the preflight error code after scheduler rollback.

If this call throws RuntimePreflightError, the catch block at Lines 380-389 rolls back the registration and throws a plain Error. The fresh-install caller therefore loses OCX_RUNTIME_PREFLIGHT_FAILED and reason, although other admission paths expose them. Keep the rollback detail in the message, but preserve the preflight code and reason on the reported error. Assert both properties in the fresh-install refusal tests. The PR objective requires preflight refusal to report OCX_RUNTIME_PREFLIGHT_FAILED.

387-387: 🎯 Functional Correctness | 🟡 Minor | ⚡ Quick win

Keep OCX_RUNTIME_PREFLIGHT_FAILED when a fresh scheduler install is rolled back.

assertRuntimeWritable at Line 387 can throw a RuntimePreflightError. The catch block at Lines 397-407 then rolls back the registration. After the rollback, it throws a new plain Error. The new error drops code and reason. The other admission paths keep both properties: installServiceSafely, repairService and the direct installers. The PR objective says every refusal reports OCX_RUNTIME_PREFLIGHT_FAILED. The tests at tests/service/service-runtime-preflight.test.ts Lines 62 and 224 check only the message text, so they do not catch this.

…s launchers

Under a Windows application-control policy the same Bun bytes can write
the profile from one path and get EPERM from another. A durable launcher
that bakes a denied path crash-loops. Before a Windows service install,
repair, or Codex shim install/refresh changes anything, run the already
selected runtime itself to create and remove a probe entry in the config
directory. On failure refuse with OCX_RUNTIME_PREFLIGHT_FAILED and
guidance (reinstall through a permitted npm global install, or select a
trusted Bun with OPENCODEX_BUN_PATH). No alternate runtime is discovered
or persisted.

An orchestrated install admits the runtime once before the existing
service is stopped and passes that frozen selection to rendering and the
install record. Direct installer calls keep their own admission. The
preflight never deletes the config root. Shim auto-restore defers with
guidance and startup continues. Other platforms are untouched.

On Windows the runtime preflight runs before staging and may create the absent config root. It never deletes it and leaves it empty, so ownership initialization still claims it; assert emptiness instead of absence.
