Summary

Addresses #6575 by allowing the attested startup-health read used by both status and doctor to outlive the server's bounded cold service probe. The previous 1.5-second client deadline discarded a valid 1.65-second response and selected local diagnostics instead. The read is now bounded to 6.5 seconds on macOS/Linux and 16.5 seconds on Windows; cached reads remain immediate. Response proof, PID/port binding, schema validation and local fallback are preserved.

This reproduces one concrete false-fallback path. It does not establish the reporter's exact launchd flap root cause, so #6575 remains open pending affected-machine correlation.

Verification

Exact head: fc11fabb40e4b6f8bda555d51e1aa73751ef2e7a (documentation-only successor of validated runtime head a17a8a235b9d2baea34b53170cff72adf83b1c3f), rebased onto dev e53c8077e40b6f540bf1e5d5791a55282466bfac.

  • bun run test tests/cli/cli-status-startup-health.test.ts tests/config/settings-startup-health-seam.test.ts tests/service/service-desktop-startup-health.test.ts tests/server/startup-health-packaged-probe.test.ts tests/lab/core-lab-boundary.test.ts --parallel=1 --timeout 20000: 40 pass, 0 fail. The negative control previously rejected the delayed response.
  • Real loopback HTTP probe with verified per-request local management capability and response attestation: the same 1.65-second response falls back with the old 1.5-second deadline and is accepted with the new budget. This uses synthetic service health, not the reporter's service manager.
  • bun run typecheck, bun run privacy:scan, bun run structure:check, git diff --check: passed for the runtime head; final documentation-only changes also passed privacy, file-size ratchet and diff checks. Runtime source/tests are byte-identical to the validated head, so passing runtime checks were not repeated.
  • cd docs-site && bun run build: 561 pages built; 78,082 internal links checked. Rebuilt after synchronizing ja/ko/ru/zh-cn lifecycle guidance; final documentation build passed.
  • Broad validation: OCX_TEST_MAIN_TIMEOUT_MS=900000 bun run test --parallel=2 --timeout 15000 reached its 15-minute cap (exit 124), with 4 observed failures and 968 files unstarted. Continued exactly those files plus the 2 interrupted files using bun run test --parallel=4 --timeout 15000 <970 explicit paths>: 17,217 pass, 101 skip, 24 fail, 1 error across 970 files, exit 1. All 18 repository serial-lane files ran separately with --parallel=1 --timeout 20000: 654 pass, 0 fail. The union covers all 2,077 test files; this is completed file coverage across batches, not a green full suite.
  • Failure attribution on unmodified dev at the same base: Anthropic Fast (7 pass/1 fail; destination rejected because local DNS returns a non-global address), provider validation/verbosity (118 pass/20 fail), retained-root serialization plus plaintext server (42 pass/1 fail/1 error), and serving runtimes (34 pass/2 fail) reproduce the matching 24 failures. No production destination guard or machine DNS setting was relaxed.
  • The remaining 4 full-run failures are timing-sensitive cases: canonical non-stream buffering and two audio readiness cases pass in both clean-dev and PR-head focused runs (77 pass/0 fail); plaintext drip classification passes in both two-file retained-root runs. Their broad-run failures remain recorded; a successful rerun is not used to erase them.
  • No installed service restart/repair, provider-account traffic, affected-machine diagnosis or CI success is claimed. [Bug]: ocx status/doctor flap service=viable vs service=absent while launchd service is running (2.77.0 macOS) #6575 remains open for affected-machine correlation. Required hosted CI and maintainer review remain pending.

Checklist

  • Scope stays focused and avoids unrelated cleanup.
  • Docs or release notes were updated when needed.
  • Security-sensitive changes were reviewed for secrets, auth, and unsafe defaults.

Review readiness checklist

This PR stays in draft until every box below is ticked. Tick all four boxes once the requirements are met:

  • Required local validation passed; commands, results, and any full-suite exception are documented.

  • I pushed my PR to a recent dev commit (at most 10 behind; a maintainer may still ask for the exact tip before merge).

  • I resolved all correct Codex and CodeRabbit findings.

  • My PR is ready for review.

Readiness boxes ticked by maintainer @lidge-jun during the 2026-10-05 bug triage round 2 (lane B3). Per owner instruction no tests were run locally; local validation was limited to static checks and tests ran in hosted CI at the exact head.

📝 Walkthrough

Walkthrough

The live startup-health read now uses a platform-specific timeout that allows the bounded service probe to finish. The startup-health cache uses the same probe budget. Tests and documentation cover the budgets, delayed responses, and timeout fallback.

Changes

Startup-health reads

Layer / File(s) Summary
Define and apply platform budgets
src/codex/autostart-health.ts, src/server/startup-health-cache.ts
Exported helpers define probe budgets of 5,000 ms on non-Windows platforms and 15,000 ms on Windows. The cache uses the shared probe budget.
Use and validate the reader budget
src/cli/status.ts, tests/cli/cli-status-startup-health.test.ts, structure/ops/service-and-sidecars.md, docs-site/src/content/docs/*/reference/cli/lifecycle.md
The CLI read timeout adds 1,500 ms to the probe budget. Tests cover platform budgets, delayed attested responses, and a reader timeout. Documentation describes the bounded wait, cached reads, local fallback, and proof requirements.

Priority: ➖ Normal

Estimated code review effort: 2 (Simple) | ~10 minutes

Change: Bug fix

Suggested reviewers: lidge-jun

Merge Risk: ⚪ Minimal · up to fc11f

Live reads can now wait for the bounded startup probe, while invalid or timed-out results still fall back to local diagnostics. The translated lifecycle pages now describe the same limits; no change-specific product risk is established.

Security Architecture Review

Security architecture risk: 🔵 Low · up to fc11f

The longer reads preserve existing identity checks and local fallback. A concurrent service or settings change can, however, leave a delayed result describing older restart protection and recovery advice. No authorization bypass was identified.

Retained concerns

  • Low · reliability · inferred: A settings or startup-action mutation can invalidate an in-flight health probe, yet an already-awaiting request can still receive that probe's result without a stale marker. The race predates this PR, but responses beyond the former 1.5-second deadline can now be accepted by status and doctor instead of triggering local diagnostics. This may present obsolete restart-protection or repair guidance during concurrent changes. Generation checks protect subsequent cache publication and newer reads; they do not protect the result already being awaited. This is a recovery-state consistency concern, not an authorization bypass.
Security review details

Security Blast Radius

  • observed — The changed read targets the existing startup-health path for a runtime-bound PID and port. Its capability remains an exact allowlisted GET grant, not a reusable administrative credential. The runtime diff does not introduce additional tenant, infrastructure, secret, or tool authority.

Security Findings and Attack Paths

  • inferred — No new authorization-bypass path was identified in the inspected change. The retained concern affects health presentation and operator recovery guidance: obsolete restart-safe state can suppress advice, while obsolete unhealthy state can recommend unnecessary repair. These consumers display guidance rather than automatically executing the recommended command.

Trust Boundaries and Controls

  • observed — The client requires runtime identity and matching PID, port, and secret before creating a nonce-bound capability. The server validates and consumes that capability before awaiting route execution, rejects replay, and signs the response nonce afterward. Consequently, the 16.5-second Windows completion budget does not extend the capability's 10-second admission lifetime or grant access to another request.

Resilience and Maintainability Implications

  • observed — Transport failure returns unavailable, an unproven response has its body cancelled, and invalid payloads return null for local fallback. Centralizing timing preserves the existing server bounds while reducing deadline drift between producer and consumers.

Hardening Proposals

  • proposed — Associate awaited probe results with their generation and recheck that generation before returning them as current. On invalidation, mark the result stale or perform a bounded reread, explicitly defining freshness semantics for reads that overlap mutations.

Scoped intake at 5ebf5d3, not approval. The six-file diff shares the existing platform probe budget and gives the attested reader probe+1.5s; the original proof requirement and timeout fallback remain. The new synthetic 1.65s response case targets a concrete false fallback, not the reporter's launchd flap root cause. Keep #6575 open until affected-machine evidence connects the reported symptom to this path.

Cross-platform CI 37211873736 and React Doctor 37211873739 are action_required, not passing. Your failed/interrupted full run and incompletely attributed failures remain separate from the reported focused successes. Please retain Draft until the head-bound readiness requirements and actually executed exact-head CI are satisfied. I did not authorize fork workflows, run a scan, restart services or reproduce on the reporter's Mac.

Validation follow-up at exact head a17a8a235b9d2baea34b53170cff72adf83b1c3f (dev base e53c8077e4). The PR Verification section now records all 2,077 test files executed across the bounded initial run, the 970-file continuation and all 18 isolated serial lanes. The full suite remains red: 24 matching failures reproduced on clean dev; four timing failures pass in focused controls but remain recorded. Focused startup-health tests are 40/40, and a real loopback capability/attestation probe accepts the delayed response that the old deadline rejects. Typecheck, privacy, structure and the 561-page documentation build passed.

Please authorize/execute required CI for this current SHA and review the scoped timeout change. I am retaining Draft and leaving #6575 open; this does not reproduce or diagnose the reporter's launchd service state. @coderabbitai review

@Yuxin-Qiao, I will trigger a review of the scoped timeout change.

Your validation report distinguishes completed test-file coverage from a green full suite. The 24 failures reproduced on clean dev and the four timing-sensitive failures remain recorded. The focused startup-health tests and loopback probe support the bounded-read fix; they do not establish the cause of the reporter’s launchd symptom.

I cannot authorize or execute GitHub Actions through the available read-only tools. A maintainer must authorize the required workflows for a17a8a235b9d2baea34b53170cff72adf83b1c3f. This review request does not authorize those workflows or establish CI success.

Keep the PR in Draft and #6575 open until the stated readiness and affected-machine evidence requirements are met.

Addressed the translated-lifecycle finding in fc11fabb40e4b6f8bda555d51e1aa73751ef2e7a: ja, ko, ru and zh-cn now state both read budgets, prompt cached reads, timeout fallback and the /healthz limitation. Final documentation build passed (561 pages, 78,082 links); privacy, file-size ratchet and diff checks passed. Runtime source/tests are unchanged from the previously validated head. Full-suite failures remain documented and Draft is retained.

@Yuxin-Qiao, I verified that fc11fabb40e4b6f8bda555d51e1aa73751ef2e7a is the current PR head and that Draft is retained.

The translated-lifecycle finding is addressed. Each updated paragraph includes both read budgets, prompt cached reads, local fallback on timeout, and the /healthz limitation:

  • docs-site/src/content/docs/ja/reference/cli/lifecycle.md — Line 130.
  • docs-site/src/content/docs/ko/reference/cli/lifecycle.md — Line 200.
  • docs-site/src/content/docs/ru/reference/cli/lifecycle.md — Line 197.
  • docs-site/src/content/docs/zh-cn/reference/cli/lifecycle.md — Line 129.

The final commit changes only these four documentation files. It does not change runtime source or tests.

Hosted CI is not passing on this SHA. The exact-head workflows show:

Workflow Result
Cross-platform CI action_required
React Doctor action_required

I cannot authorize or execute hosted workflows through the available read-only tools. A maintainer must authorize these exact-head runs. Results for the prior SHA do not establish CI success for this SHA.

I will trigger the requested review. Your reported documentation checks remain separate from hosted CI and the documented full-suite failures. This reply does not establish merge readiness or diagnose #6575.

Follow-up at fc11fabb40e4b6f8bda555d51e1aa73751ef2e7a, base dev e53c8077e40b6f540bf1e5d5791a55282466bfac.

I compared the successor with a17a8a235b9d2baea34b53170cff72adf83b1c3f: only the four ja/ko/ru/zh-cn lifecycle pages changed; runtime source and tests are identical. The added paragraphs now carry both read budgets, cached-read behavior, timeout fallback and the /healthz limitation. The translation thread is already resolved. This is a scoped documentation follow-up, not a new runtime or security approval.

The updated Verification appropriately distinguishes completed coverage of 2,077 test files from a passing suite, retains the 24 reproduced base failures and four timing-sensitive failures, and binds the final documentation receipt to this successor. These are author-reported results; I did not execute the tests or documentation build.

Current-head Cross-platform CI 37256147179 and React Doctor 37256147118 still report action_required, not success. CI authorization requires a separate execution-trust decision; it is not granted by this COMMENTED review. Keep Draft and the outstanding required review/validation conditions. #6575 remains open: the synthetic delayed response does not diagnose the reporter's launchd state. No workflow authorization/rerun, local execution, scan, approval or merge was performed.

…ransport

The attested reader asked for 16.5 seconds on Windows, but the direct
transport kept its own 10-second exchange bound, so a cold 10-15 second
service probe still fell back to local diagnostics. The read client now
forwards the caller deadline to the default transport. Capability, proof,
and PID/port binding are unchanged.

  • Merge refresh: fc11fabb40..5ea7f2a054 changes none of the explicitly listed runtime/authentication files. Incoming dev changes supply the inference port to management handlers, add shim diagnostic guidance, and strengthen the test-only atomic-write guard. They leave local-read admission and response signing intact. (src/server/index/serve-options.ts:302–306,668–674,707–714; src/server/management/context.ts:196–199; src/codex/shim-diagnostics.ts:71–75; src/config/atomic-write.ts:127; src/lib/test-home-guard.ts:148–150)

  • Fix delta: Capability creation, fresh nonce, credential headers, runtime PID/port checks, proof verification, and error mapping are unchanged. fetchImpl retains its two-argument invocation; only the default direct-transport branch receives { timeoutMs }. Startup health still forces response proof, retains schema validation, and falls back to local diagnostics on failure. (src/server/local-management-read-client.ts:57–111; src/cli/status.ts:253–294; src/cli/doctor.ts:1381–1382)

  • Effective deadlines: Memory remains 2s, OAuth accounts 4s, and startup health 6.5s POSIX / 16.5s Windows. The previous Windows 10s transport limitation is resolved. The transport uses an absolute timer and destroys the socket on timeout; trickling bytes cannot reset that deadline. The unchanged 10s capability TTL governs request admission, not response completion. (src/cli/doctor.ts:747,768–770; src/oauth/health.ts:445–448; src/codex/autostart-health.ts:14–20; src/server/local-management-read-client.ts:89–102; src/server/direct-local-http.ts:300–321,366–371; src/lib/local-management-capability.ts:8,103–108)

  • Availability and seam: A hostile listener can now consume an additional 6.5s on Windows, but the wait remains bounded and cannot produce a trusted startup verdict without proof. Each command performs one startup read. Doctor’s startup/memory/model/account reads can accumulate 20.5s POSIX / 30.5s Windows, excluding other diagnostics; this is acceptable for the bounded diagnostic flow. directFetch is a programmatic dependency seam used only by the new test; production callers do not set it, and either transport branch still reaches the mandatory startup proof check. (src/cli/status.ts:755; src/cli/doctor.ts:1381,1494,1561,1633,1099; src/server/local-management-read-client.ts:29,100–107; tests/cli/cli-status-startup-health.test.ts:80–93)

  • Final commit: 46f5fac883..1865d284 changes only the French, Turkish, and Traditional Chinese lifecycle documentation. (docs-site/src/content/docs/fr/reference/cli/lifecycle.md:136; docs-site/src/content/docs/tr/reference/cli/lifecycle.md:192; docs-site/src/content/docs/zh-tw/reference/cli/lifecycle.md:129)

Maintainer refresh (lane B3) at 1865d284d068c893b68198bce73d2c933767aacc. No local tests were run for this refresh, per the owner's instruction for this round; test evidence comes from the hosted CI linked below.

Three commits were added on top of fc11fabb40, with a normal push (no rewrite):

  • 5ea7f2a054 merges current dev (37189ca3c6). There were no conflicts, and none of the PR's runtime files changed on dev since the old base.
  • 46f5fac883 fixes a gap in the Windows budget. fetchBoundLocalManagementRead called the direct local transport without passing a deadline, so the transport's own 10-second exchange bound (src/server/direct-local-http.ts) cut the 16.5-second Windows read short, and a 10–15 s cold probe still fell back. The read client now forwards the caller deadline to the default transport, through an optional directFetch seam. An injected fetchImpl is still called with two arguments, and capability, nonce, PID/port binding and proof checks are unchanged. A contract case in tests/cli/cli-status-startup-health.test.ts asserts that the transport receives startupHealthReadBudgetMs() and that fetchImpl keeps precedence. The structure note says the same. Deadlines for the other callers stay the same: 2 s for memory and 4 s for OAuth accounts.
  • 1865d284d0 adds the read-budget paragraph to the fr, tr and zh-tw lifecycle pages, so all eight locales carry it.

Static checks run in a clean worktree at this head: bun run typecheck (also type-checked the changed test file), bun run structure:check, bun run privacy:scan, bun scripts/file-size-ratchet.ts, and git diff --check. All exited 0.

Hosted CI at this head: PR Cross-platform CI, React Doctor, and a full-matrix lane=all dispatch on ci-mirror/pr-6587 at the same SHA, because status/service behavior is platform-sensitive.

Still unverified: a real 10–15 s cold Windows service probe end to end, and the reporter's launchd flap. #6575 stays open. @Yuxin-Qiao, the push reset the readiness checklist. Please re-check the boxes against this head when you are satisfied.

Maintainer integration (lidge-jun, dev only) — 2026-10-05 bug triage round 2, lane B3.

  • Head: 1865d28
  • CI at this head: PR CI 37320590159, 37320590554; full-matrix dispatch 37320627456 (windows 1-9, macos 1-2); every executed job passed.
  • Review: status/doctor startup-health read deadline aligned with the server cold probe, deadline now passed to the local HTTP transport (Windows 10s cap fix by the lane); independent security review PASS at this head; addresses one false-fallback path of [Bug]: ocx status/doctor flap service=viable vs service=absent while launchd service is running (2.77.0 macOS) #6575 (issue stays open); readiness ticked by maintainer with hosted-CI-only note.
  • No local tests were run, per owner instruction; hosted CI is the evidence.
  • scripts/ci/assert-mergeable-review.sh --maintainer-integration: OK.
