Form savior broken on manage subscriptions?

It's throwing an error to the js console -- something about "live"... Might be related to this: anandgorantala/Form-Savior#4

So this fixed the "live" problem, but now it is throwing the following, and I'm not savvy enough to figure this one out, so I'll need some help.

Uncaught TypeError: Object form .frequency_range has no method 'apply' jquery.js:3075
jQuery.event.dispatch jquery.js:3075
jQuery.event.add.elemData.handle jquery.js:2751

To reproduce, go to Screens > (select one to edit) > Click on Manage Subscriptions (in one of the field locations on the screen mockup) and then click on Add a new feed or move the frequency slider.
