Problem

On Claude Code 2.1.286, every Claude spec/quality review is rejected with no receipt (REJECTED, recovery uncorrelated-launch-result), so no HTML plan task can reach task-review or accept-scope. Found while resuming the SMARTS build (#893): T-02's re-armed spec review completed, but its decision could not be published.

Root cause (live hook captures on 2.1.286, not inferred)

  1. Tagged model id. From a 1M-context Opus parent, PostToolUse(Agent).tool_response.resolvedModel is claude-opus-5-5[1m]. It was validated with HOST_ID_RE (no brackets), which raised resolvedModel is malformed, and the launch was rejected. The background launch shape itself is unchanged and fine. The 2.1.281 fixtures were already async_launched.
  2. Handback reports. The child now reports through a SubagentHandback tool call (Pre/PostToolUse carrying the child's agent_id), and its SubagentStop has no last_assistant_message. Fixing (1) alone would still reject at the first stop.

Fix

  • resolvedModel gets a dedicated MODEL_ID_RE that admits one trailing [a-z0-9]{1,16} tag. HOST_ID_RE is untouched, so agentId, prompt_id, etc. still refuse brackets. The value is recorded verbatim; the Go engine already accepts any non-empty resolved_model.
  • New PostToolUse(SubagentHandback) seam (hooks.json matcher + hook routing). It counts only when all of these hold: the agent_id is the bound reviewer's, the session is the launching parent session, the agent_type is ca:authority-reviewer, the request is RUNNING, the message is within MAX_DECISION and success: true. The message is stored on the request (top level, under the existing lock and integrity hash, not in the engine-pinned launch object). A second handback rejects the review (repeated-handback). Ordinary handbacks from any other agent return silently and are never refused.
  • SubagentStop stays the single completion point. It takes the handback, otherwise last_assistant_message (the 2.1.281 path is unchanged). If both are present they must be identical, or the review is rejected.
  • Recovering an already-abandoned oversized legacy review now raises INVALID_RECOVERY: request was already abandoned instead of the misleading legacy review recovery binding failed.

Known limit, unchanged from before: a reviewer run synchronously binds its agent id only after it has stopped, so it fails closed. The qualified path is the background launch.

Evidence

  • Sanitized 2.1.286 fixtures (fixtures/claude-hooks/2.1.286/) recorded from one live ca:authority-reviewer background launch, beside the 2.1.281 set.
  • Regression tests went red for the production reason (resolvedModel is malformed, then review decision is absent, then the misleading recovery message) before each fix.
  • The real-engine end-to-end test now also runs the full 2.1.286 sequence through the shipped hook and CLI to task-review and accept-scope.
  • Mutants killed: widened global id regex, loose tag, handback overwrite, any agent type, any session, ignored disagreement, ignored delivery failure, dropped old-host path, dropped hook routing. One equivalent mutant (prefer the final message when both are present) survives by construction.
  • Local: adapter suite 165 passed / 1 skipped, ca hook tests 1819 passed, cold install 468/0, sync-core, codex static package, host descriptors, ci-impact, artifact package/workflow/approval all green. test_startup_emitters has 2 failures that reproduce identically on clean origin/main; they are pre-existing and environmental, not from this change.

Conflict-hierarchy level 1 (audit-trail correctness): the change widens what the review seam accepts only by host-observed shapes, and each widening is bound to the reviewer's identity and fails closed otherwise.

Ships ca 2.23.2, ca-codex 0.14.9, ca-pi 0.15.9 (shared authority library).

Summary by CodeRabbit

  • Bug Fixes
    • Improved compatibility with Claude Code 2.1.286, including support for model identifiers with a trailing context tag.
    • Review reports delivered through handbacks are accepted only when they come from the expected reviewer in the launching session. Duplicate or mismatched reports are rejected, and handbacks must agree with any other report provided.
    • Recovering an already-abandoned oversized review now reports that it was already abandoned rather than showing a misleading binding error.
    • Security-gate approvals no longer expire after 30 minutes. They remain valid for the reviewed sensitive lines in the reviewed files; new, changed, or moved sensitive lines require review.

Every Claude spec and quality review was rejected without a receipt on
Claude Code 2.1.286, blocking task-review and accept-scope for any HTML
plan. Two host changes, both confirmed by live hook captures:

- A 1M-context parent resolves the reviewer alias to
"claude-opus-5-5[1m]". resolvedModel went through HOST_ID_RE, which
refuses brackets, so the launch was rejected as
uncorrelated-launch-result. resolvedModel now has its own check that
admits exactly one trailing lowercase tag; agentId, prompt_id and the
other host identifiers keep the strict form.
- The child reports through a SubagentHandback tool call and its
SubagentStop no longer carries last_assistant_message. The hook now
records the bound reviewer's delivered handback (same agent, same
  launching session, pinned agent type, RUNNING state, bounded size);
a second handback rejects the review, and when a host supplies both
forms they must be identical. SubagentStop stays the only completion
point, and the 2.1.281 final-message path is unchanged.

Recovering an already-abandoned oversized legacy review also reported
a misleading "legacy review recovery binding failed"; it now reports
INVALID_RECOVERY "request was already abandoned".

Recorded 2.1.286 fixtures sit beside the 2.1.281 set, and the real-
engine end-to-end test runs on both. Every new rule is mutation-killed
(a widened global id regex, a loose tag, a handback overwrite, wrong
agent type, wrong session, ignored disagreement, ignored delivery,
dropped old-host path, dropped hook routing). Ships ca 2.23.2,
ca-codex 0.14.9, ca-pi 0.15.9 (shared library).

CHANGELOG: Claude Code reviews complete again on Claude Code 2.1.286 (tagged reviewer model, handback reports); a retried abandonment names the real state.

📝 Walkthrough

Walkthrough

Repeated recovery of an abandoned oversized review now reports request was already abandoned. Claude review requests accept tagged model IDs and use a bound reviewer’s delivered handback as a report. Security-gate passes no longer expire after 30 minutes and remain bound to reviewed lines and files.

The security gates currently honor recorded passes without a time limit. Strengthen the regression test to catch a future reintroduction of the expiry check; this gap does not block merging.

Security Architecture Review

Security architecture risk: 🟡 Moderate · up to c8be2

Binding approvals to file paths improves isolation, but removing expiration permits an old approval to authorize the same sensitive line after its surrounding code changes. The demonstrated exposure is local commit admission, not a confirmed production compromise.

Retained concerns

  • Medium · security · inferred: Persistent approvals do not bind ordinary sensitive lines to their reviewed execution context. A local actor able to modify and stage source can reuse an approved line at its original path after changing surrounding inputs, control flow, or configuration that the scanner does not classify as sensitive. If all detected digests remain covered and the marker remains present, both enforcement points accept that approval without another review. This condition was previously limited by the freshness window; path binding prevents cross-file reuse but does not prevent same-path contextual replay.
Security review details

Security Blast Radius

  • inferred — The evidenced approval-replay scope is commit admission within a repository and its linked worktrees. Marker sharing under the main checkout predates this PR; removing expiry extends the lifetime of that shared authority. Exploitation requires source-editing and staging capability plus an available matching approval, not newly granted cloud or service credentials.

Security Findings and Attack Paths

  • inferred — An old approved sensitive call can be reintroduced at the same path with different surrounding arguments or control flow while retaining its ordinary digest. If the other additions introduce no uncovered sensitive digest, both gates accept the old marker even after the former freshness window. This is an inferred authorization-replay path, not a verified production exploit.

Trust Boundaries and Controls

  • observed — The new handback boundary checks reviewer identity, parent session, running state, reviewer type, bounded schema, remaining state capacity, and successful delivery. Conflicting handbacks or disagreement with the stop message are rejected rather than replacing the authoritative report.
  • observed — Stop finalization still correlates by reviewer identity and background-task corroboration without explicitly comparing the stop session to the launching session. The exact base/head comparison shows this predates the PR. Host guarantees concerning identity uniqueness and replay resistance remain unverified; this is not retained as an introduced concern.

Resilience and Maintainability Implications

  • observed — Approval recording replaces the marker atomically. Both consumers block on unreadable scans and require coverage of every detected sensitive digest; unreadable marker contents yield no usable approvals. These controls contain partial-write and classification failures but do not establish that a persistent approval still matches its reviewed context.

Hardening Proposals

  • proposed — Preserve non-expiring approvals only when a security-relevant snapshot or explicit review subject remains unchanged. Extending ordinary approval identities beyond path and line text would address contextual replay without relying solely on elapsed time.

Included review availability: This review used your included allowance. Your plan provides up to 2 included reviews per hour; 1 remain after this review.

Security review of the handback seam found two fail-closed availability
gaps and a few hardening points; all are fixed here.

- State bound: the handback cap measured UTF-8 bytes, but state is
stored ASCII-escaped, so a valid report could push a large review
request past MAX_STATE. Every later load would then fail, the request
could never be recovered, and strict registry scans (SendMessage
denial, Codex spawn correlation) would fail for that user until a
manual delete. The handback now keeps room for its escaped form plus
the completion payload, and _save refuses any write past the bound
  except retaining a legacy abandonment.
- Late bind: when SubagentStart wins the launch race, the request sits
LAUNCHING with a pending start marker. The handback now joins that
marker the way SubagentStop already did, instead of rejecting a
  legitimate report.
- A doubled delivery of the same handback (same tool_use_id and text)
is a no-op rather than a rejection; any other second report still
rejects. The stored record is a closed {tool_use_id, message} that
_load validates (Claude review requests only, bounded size).
- security-controls.md names the new report source in the host hook
  evidence boundary row.

Each rule is mutation-killed. The legacy-state fixture now models the
unbounded writes of the adapters that produced those states.

CHANGELOG: the reviewer handback can no longer make a review request unreadable, and survives the launch/start race.

Both new end-to-end authority tests are kept; provenance is re-baselined.
The crypto/secret commit gate expired a recorded pass after 30 minutes,
so any pause between the review and the commit (or a later commit of the
same reviewed lines) forced the gate to be re-run for unchanged content.
The pass is already bound to the exact line digests it reviewed, which is
what stops it laundering a different diff; the time window added nothing
but friction. Require only that a pass exists and covers every sensitive
line, matching the migration gate (H-14). Maintainer directive 2026-10-03.

CHANGELOG: a crypto/secret gate pass no longer expires; it stays valid
for exactly the lines it reviewed.

Comment thread core/pysrc/git-enforce.py
…ed in

Review on #895: with the time window gone, a pass bound only to a line's
digest could admit that identical line after it moved to another file
(an intentionally-reviewed test payload pasted into production code, for
instance). line_digest now hashes the repository-relative path with the
line, and the diff scanner passes each section's file, so the recording
side (security-pass.py) and both gates (pre-bash, git-enforce) agree on
path-bound approvals. An unattributable section keeps the line-only form
on both sides.

The hook unit tests that pinned the removed window now assert the gate
has none, and read the marker at the expected root instead of mocking
marker_fresh. A new guard case proves a pass for one file does not cover
the same line in another (killed when the path is dropped).

CHANGELOG: a security-gate pass covers the lines it reviewed only in the
files it reviewed them in.

_marker_fresh_second_args ignores marker_fresh(marker, minutes=30), so adding that call to either gate can still pass this assertion. The two-hour aged-marker test exercises pre-bash.py, but does not cover git-enforce.py. Check for all marker_fresh calls. This is a coverage gap, not a current production failure.

Update the test helper and no-window assertion around
`_marker_fresh_second_args` to detect every `marker_fresh` call, including calls
that pass `minutes` by keyword, and assert that neither security gate contains
any such call. Add coverage confirming the two-hour aged-marker behavior in
`git-enforce.py`, matching the existing `pre-bash.py` test.

test_crypto_context pinned the line-only digest; a pass is now bound to
the file it reviewed, so expect the path-bound form (computed from the
staged line read back from the index). The site's context exhibit records
the exact helper bytes it observes, and the _hooklib contract-comment
update moved only that source hash; every observed case is unchanged.
