Malaria - Enhance Sample form

Problem Description

After the Disease enum has been modified in (#13796) as part of epic (#13797), the sample and pathogen test form needs to be updated with Malaria-specific details

Acceptance criteria

• Update the sample list Malaria specific as follow
  • Blood
  • Serum
  • Dried blood spot
  • EDTA whole blood
  • Plasma
  • Clinical Sample (Other)
• Update the pathogen tests Malaria specific as below,
  • Thick blood smear
  • Thin blood smear
  • Antigen RDT
  • Other antigen detection test
  • RT-PCR
  • qPCR
  • LAMP
  • Other molecular assay
  • ELISA (antibody)
  • Other serological test
• For all the tests, expect "Thick blood smear", plasmodium spices need to display. Spices values as below,
  • Plasmodium spp
  • Plasmodium falciparum
  • Plasmodium vivax
  • Plasmodium malariae
  • Plasmodium ovale
  • Plasmodium knowlesi
  • Plasmodium cynomolgi
  • Plasmodium species, not specified
  • Coinfection
• Selected Plasmodium value should display in samples pathogen test list as is it for IPI Serotype and Measles Genotype.
• For all the quantitative tests(Thin blood smear/PCR /qPCR /ELISA/IFAT), result details should display.
  • Thick blood smear
    • Only Test result, no species, and result details.
  • Thin blood smear
    • Test Result + Species + Result details
  • RDT
    • Test Result + Species + No result details.
  • PCR
    • Test Result + Species + No result details
  • qPCR
    • Test Result + Species + Result details + CT Value
  • LAMP
    • Test Result + Species + No result details
  • ELISA
    • Test Result + No species + No result details
  • IFAT
    • Test Result + Species + No result details
• YesNo checkbox for storing the reference laboratory as "performed by reference laboratory".
• ReTest checkbox with YesNo values. Default it to "No".
• ‘Result verified by supervisor’ is not mandatory
