Malaria - Enhance EPI form

Problem Description

After the Disease enum has been modified in (#13796) as part of epic (#13797), the case data form needs to be updated with Malaria-specific details.

Acceptance Criteria

• A new section as "Exposure period to consider" should be displayed.
  • This section will displays the start and end dates of Investigation. They are automatically calculated fields, based on Symptom onset dates.
  • Exposure start date = Onset date - MaxIncubation
  • Exposure end date = Onset date - MinIncubation.
• In the exposure table, duration column will be included after the date column, which will inferred from start and end dates of exposure dates.
• "Name of source case" column should display with empty value for Malaria disease.

Exposure form

• "Exposure details will be hidden".
  • contact to source case will be hidden
  • Role will be hidden
• Location has no impact
• Below two new Activity types will be added to the existing "Type of Activity" list
  • Blood transfusion recipient
  • Bone marrow transplant recipient.
• If activity type is 'Travel', Prophylaxis details need to capture with the below values
  • Prophylaxis prescribed and course completed
  • Prophylaxis prescribed but course not completed
  • Prophylaxis prescribed and not taken
  • No prophylaxis prescribed
• Selected prophylaxis details needs to be displayed in the conclusion section as read-only, after transmission(Suspected main mode of transmission).
• For Travel activity, reason for it needs to captured as,
  • Business
  • Visiting family or friends
  • Migration
  • Military
  • Humanitarian mission
  • Traveler from an endemic country
  • Student
  • Tourism
  • Other (with text field)
• New activities will be included for the existing activity list as
  • Blood donation
  • Bone marrow donation
• Residing or working/ traveling YesNoUnknown details will be hidden along with its heading.
• Since Malaria is not a contagious, we should hide the "Contacts with the source case" section.
• Conclusion section is updated with the new "Transmission" details as,
  • Contracted from infected mosquitoes in an endemic country
  • Contracted from infected mosquitoes transported by air from an endemic country to a non-endemic country
  • Contracted while receiving medical care
  • Acquired locally through mosquito transmission with strong epidemiological evidence directly linking it to a known imported case
  • Acquired locally through mosquito transmission without evidence of importation and without a direct link to transmission from an imported case
  • Mother-to-child transmission
  • Laboratory transmission due to occupational exposure
  • Transfusion/transplant recipient
  • Other (with text field)
• There should be an option to capture the comments.
