Malaria - Enhance case data form

Problem Description

After the Disease enum has been modified in (#13796) as part of epic (#13797), the case data form needs to be updated with Malaria-specific details.

Acceptance criteria

• The "Notified" by card is enabled.
• Case definition as per ECDE should be displayed.
• Pre existing conditions should display.
• Additional Pre existing conditions displayed.
• The Immunization card should display.
• Quarantine details should be hidden.
• Luxembourg standard Case Person tabulation is displayed
• Occupation List is updated with Work in an airport
