Custom icon not working

I tried to upload this custom icon to match the other icons on the toolbar:

But instead, I get this:

Any idea why?

EDIT: Also, great extension btw...is there anyway to turn off the CTRL SHIFT V short cut?

Thanks. This feature is broken after switching to manifest V3. Plan to fix in next version.
