fix cfn #160
Description
- CFN_INDEX_FILE doesn't work to be changed
- assumptions around TMP_S3_URL and S3_INFRA_BUCKET --- document them or do without
- aws-cfn2dot assumes there's lodash-firecloud
- can it be made to work without TMP_S3_URL ?!
