Restricts the lookup of associated ways and relations for a feature to situations where an `osm` object is available. Adds indexes on single layers for WOF, shapefiles and NE data, although currently hard-coded.
This is to support the ne_10m_urban_areas, which should not be visible at z10+. Moved some code around to where it felt more natural.

Added support for extra data tables, including indexing them. Made indexing configurable so that it is (in theory, at least) not hard-coded to our schema. Updated to match changes in tilezen/raw_tiles#17.
