Fixed rtl_fm up/downsampling #32

I fixed arbitrary resampling to the point where acceptable sound can be heard:

Output buffer size is being set correctly (although ugly)
The output buffer is copied so that the upsampling algorithm can work correctly.

Two additional commits are included by accidend due to the dual git repository mess.
