Support manually associating PRs associated with easy issues

Newcomers don't necessarily have to follow the issue template. At times, they may have an empty/unusable PR body. In such a case, a collaborator can tell highfive to associate the PR to an issue by dropping a comment in the issue/PR with the relevant number - something like @highfive: associate #<number> - this also replaces any existing associations.
