feat: agent memory hints API for active ingest participation

Extend add() with optional MemoryHints: importance, topic, supersedes, entities, retention, type. Includes preference pattern extraction (16 regex), supersession handling, and async post-ingest similarity scan + entity extraction. Zero LLM cost.
