For hover (and go to def), clarify when using the `Query` fallback of approximately resolve symbol

Path resolves to https://github.com/rust-analyzer/rust-analyzer/blob/904438e993b4cc3c1d9269a44436c1b112de16c0/crates/ra_hir/src/path.rs#L3-L7, which it clearly shouldn't. We can make this more clear somehow.

Note that in discord, @matklad and I discussed adding a DefPresentation for presentation details of a declaration. This could be used to give this information when implemented.
