Introduction

Data, plots, timeline and journalling about my health management and battle with UC / PSC since January 2005. This grasping of data about myself, even if it gives just an illusion of control, it "works" nonetheless as coping mechanism because sometimes just the illusion is enough to calm down your monkey brain from taking over and the stress to worsen and even cause further malaise.

This is something I wanted to do for many years but never started. A recent double hospitalization in less than a month, and the advent of powerful LLM coding agents gave me the nudge I needed.

🩸 Liver & Biliary Support

Ursacol (Ursodeoxycholic Acid)

  • Mechanism: Hydrophilic bile acid → replaces toxic bile salts → reduces cholestasis & inflammation in PSC.
  • Dose: Currently 2 × 300 mg (AM) + 3 × 300 mg (bedtime) = 1.5 g/day (align with weight‑based guidance with your hepatologist).
  • Optimization: Maintain split dosing (AM + bedtime).

Silymarin — current: 800 mg/day

  • 2 × 200 mg at lunch + 2 × 200 mg at bedtime (= 800 mg/day). Hepatoprotection via glutathione up‑regulation and NF‑κB modulation.

N‑Acetylcysteine (NAC) — 3 g/day

  • 1.5 g lunch + 1.5 g bedtime → hepatic GSH support and mucosal protection.

MSM — 4 g/day

  • 2 g lunch + 2 g bedtime → thiol replenishment & connective‑tissue support.

TMG (Trimethylglycine) — 500 mg AM + 500 mg dinner

  • Supports BHMT remethylation and may aid bile flow; complements methionine/choline.

SAMe (S‑adenosyl‑L‑methionine) — 400 mg AM + 400 mg afternoon

  • Evidence for cholestasis symptom and enzyme support; avoid late‑evening dosing if stimulating.

Taurine — 1 g AM + 1 g dinner

  • Substrate for bile‑acid conjugation; supportive for bile flow.

Vitamin K2 (MK‑7) — 100–200 µg dinner

  • Fat‑soluble vitamin support in cholestasis (bone/INR considerations). Water‑miscible forms if advised.

🧬 Anti-inflammatory & Gut Support

Adalimumab + Mesalazine

  • Baseline for mucosal control — continue as prescribed.

Curcumin (Golden Milk) — 7 g powder with pepper

  • Lipid vehicle (milk) + piperine improves absorption.

Omega‑3 + Phosphatidylserine

  • At lunch per schedule; maintain split EPA+DHA to ~3 g/day total.

Psyllium husk — 10 g morning + 10 g dinner

  • Remission maintenance and butyrate support. Separate from meds/supps by ~2 h (iron, calcium, NAC, quercetin, mesalazine).

Probiotics

  • L. rhamnosus + NutrioneLife at dinner.
  • Step‑up options: De Simone formulation (VSL#3) or E. coli Nissle 1917 (Mutaflor) — in afternoon, ≥3–4 h after Bactrim.

Quercetin — 500 mg lunch + 500 mg bedtime

  • Anti‑inflammatory/anti‑fibrotic; keep ≥3 h away from iron (Noripurum).

Boswellia (AKBA‑standardized) — 300–500 mg lunch + dinner

  • Botanical adjunct for UC inflammation/fibrosis; use standardized extract.

🌙 4. Sleep Optimization

Melatonin — 1–3 mg (start 1 mg) at bedtime

  • Sleep onset/maintenance; titrate by 0.5–1 mg steps.

Glycine — 3 g bedtime

  • Improves slow‑wave sleep and thermoregulation.

Myo‑inositol — 3 g bedtime

  • Serotonergic/cholinergic modulation for calmer sleep.

L‑Theanine — 250 mg total

  • Keep 100–150 mg mid‑afternoon + remainder at bedtime if helpful for wind‑down.

Magnesium (bisglycinate/taurinate‑dominant)

  • Keep morning + pre‑bed; avoid high citrate late day if it loosens stools.

Dosing in schedule: 500 mg lunch + 500 mg bedtime.

  • Roles: anti‑inflammatory, mast‑cell stabilizer, antifibrotic.
  • Spacing: keep ≥3 h away from iron; avoid taking with large calcium/fiber boluses that impair absorption.

⚖️ Strategic Observations

Focus Recommendation
Iron absorption Maintain Noripurum away from quercetin, NAC, and calcium.
Bowel motility If stools become too loose, shift part of magnesium dose earlier or reduce citrate fraction.
Methylation balance You are using TMG 500 mg AM + 500 mg dinner for methyl‑cycle support.
Inflammatory control Curcumin + Omega-3 + Adalimumab + Quercetin + Silymarin create an excellent anti-NF-κB network; avoid unnecessary NSAIDs.
