Content marked with first issued date?

Perhaps each article should be dated with a `First published: YYYY-MMM-DD` line or whatever date format makes sense.

I don't have a strong opinion whether revisions should be dated. Revision date and changes can be determined from the revision history, but I don't know if people will find it easier just to look at a `Last revised: <date>` instead of dealing with Git.
