INVALID_SESSION_ID: Session expired or invalid

I am stuck from very long time on that issue and it is bit annoyed.

It seems to me that this issue is related with the following things which I wanted to mention.

 1) I am not getting refresh token.
 2) Also every time the access token is also same.
