`NSMenu` needs `NSMenuItem` to be useful

Related to GHI #518

Custodial refactor of NSMenuItem in AppKit is needed.

Namely:

• fix syntax issues
• refactor for ARC
• refactor for nullability
• add TODO/FIXME comments for incomplete implementation
• Etc.
