Made examples for dephasing, dephasing at constant rate and dynamic decoupling. Also added the python notebook for the calculation as a reference. They work fine on the qbead.
This pr also incorporates the state class pr and the gates example pr. The 'gates example' was added on top of the 'state class' and 'dephasing, dephasing at constant rate and dynamic decoupling' is added on top of 'gates example'. The 'entanglement example' can be seen separately since it required changes in the .h file.
