feat(markdown,usage): adopt five remaining upstream corrections

Five remaining clean-applying sources from the upstream catch-up range: the
GitHub mark on github.com markdown links, Claude usage limits on retried turns,
email-bearing account labels hidden in usage limits, and two review-agent label
additions to the existing .macroscope config.

Pylon adaptations

None. Every source applied unchanged.

Verification

• Tests: 191 passing across three suites.
• vp check: 0 errors across 3362 files.
• Unused exports and files: knip:check clean.

The markdown link module gains a ./links export, so a reinstall is needed
before its tests resolve; that is a local workflow note rather than a change.

Model and harness: Claude Opus 5 via Claude Code.

Source traceability verified during cycle #414 closure: the source list now uses frozen upstream SHAs. The previous list used Pylon port commit SHAs. All listed port commits are merged ancestors; source/port changes match (some patch IDs differ only because surrounding context changed).
