Workstream 5: AI review refresh

Part of EPIC #1560 (Design refresh — Phase 2). Workstream 5.

Goal

Redesign AI review to feel more polished and legible.

Scope

• Improve how feedback, reasoning, and review state are presented to the user.
• Make review output easier to scan and act on.

Conventions

• Branch off the Phase 2 integration branch (epic/1560-design-refresh-phase-2) and target it as the PR base.
• Build on the Phase 1 foundation: pdd/cli_theme.py (color system) and pdd/cli_status.py (status/progress vocabulary) — no ad-hoc visuals.
• Reference EPIC Epic: Design refresh — Phase 2 (adaptive theming, pdd connect, AI review, sync/checkup motion) #1560 and "Workstream 5" in the PR description.
• Update the tracking table in Epic: Design refresh — Phase 2 (adaptive theming, pdd connect, AI review, sync/checkup motion) #1560 when the PR merges.
