Problem

watch_pull_request lets an agent wait on a pull request without polling: the server checks, wakes the agent only when something happened, and the thread stays Waiting in Working instead of landing in the inbox. Nothing like it exists for anything else an agent waits on, such as a discussion or issue reply, another repository's CI, a deploy or a release. Today that means an external poller, or a scheduled task that starts an agent turn on every run even when nothing changed.

Discussion: #19879. #21338 asked for the GitHub issue and discussion case and was closed as a duplicate of #19879, with its details moved there. There is no explicit approval of the direction yet. I'm opening this so the working implementation can be judged next to the proposal, and I'm happy to close it if the direction isn't wanted.

Fix

watch_command { command, label, everyMinutes } and unwatch_command { watchId? }, an MCP tool pair modeled on the pull request watch lifecycle:

  • The server runs the command on the owning environment, in the thread's workspace. The first run only records a baseline, so a state that already holds (a closed discussion, an existing answer) never wakes the agent. After that, exit 0 with output that differs from what the agent last saw wakes it with that output, queued after any active turn. Empty output, and exit 1 with nothing on stderr, are quiet, so grep and jq -e gates work. Quiet runs write nothing and start no model.
  • Any other exit or a 60 second timeout is a failure. 8 in a row, output over 4 KB, 10 wakes or 7 days end the watch with a wake that says why.
  • Runs wait while the thread is working, except the baseline run, which happens right away so news between the agent's own check and the end of its turn isn't absorbed into the baseline.
  • While a watch is on, the thread is Waiting through the existing derivePendingBackgroundWork path, so every client shows it without new status logic. The thread details card lists each watch with a stop control. On mobile, the background work sheet opened from the Waiting status lists them, and its Stop ends them through the same thread Stop path.
  • Stop, settle, archive, leaving Full access, and switching to plan mode end a watch. Forks don't copy watches.
  • Only the thread's own agent can start one, through MCP, in Full access and default mode, since the command later runs outside any approval prompt. Subagent threads cannot. Limits: 5 watches per thread, 4 runs at once per environment.

Contracts carry the watch on the thread and two internal server-only commands (thread.command-watch.start, thread.command-watch.sync), so no client can make the server run a command.

Verification

  • Focused tests: vp test run src/orchestration-v2/commandWatch.test.ts src/orchestration-v2/CommandWatchReactor.test.ts src/mcp/toolkits/commandWatches src/orchestration-v2/ThreadForkService.test.ts src/orchestration-v2/ProviderTurnControlService.test.ts src/processRunner.test.ts src/orchestration-v2/testkit src/orchestration-v2/ProjectionStore.test.ts src/mcp/OrchestratorMcpToolkit.integration.test.ts src/orchestration-v2/runtimeLayer.test.ts src/orchestration-v2/ThreadFork.execution.test.ts in apps/server (all pass), plus orchestrationV2PendingBackgroundWork.test.ts in packages/shared and operations/commands.test.ts in packages/client-runtime. They cover the baseline (including during the starting turn), wakes, quiet gates, failures, limits, expiry, stop interrupting a running command, fairness between watches, forks, and a final wake that fails to write keeping the watch instead of losing the news.
  • Typecheck of contracts, shared, client-runtime, server, web and mobile. Lint on the changed files shows no new findings.
  • Live run in a dev server on fresh state (macOS, web client, Codex GPT-6-Luna at low effort). The agent was asked to watch cat status.txt every minute. The baseline run recorded the existing content with no wake. After I changed the file, the next run woke the agent once with the new output, and it called unwatch_command, returning the thread to the inbox. The same prompt on main gets "watch_command isn't available".

Agent asked to wait on a reply

The watched output changed: one wake, then the agent stops watching

  • Independent live run by @Lenoridev at d8224639be (report): real Codex sessions on Linux watching a discussion, an issue, another repository's CI, deployment statuses and releases. 16 expected wakes, none from unchanged or own-account activity, and correct baselines, Stop, settle, archive, permission changes and a server restart.

Not checked: mobile on a device or simulator, and desktop beyond the shared web UI. The branch no longer changes mobile code: upstream's background work sheet (#21395) lists command watches and its Stop ends them.

Known limit, shared with pull request watches: the Stop action picks what to stop from the client's last projection, so a wake that starts a turn in the same instant as a Stop click can survive it. A second Stop ends it.

Approvability

Verdict: Needs Human Approval

Macroscope's review found this PR requires human approval — This PR adds a substantial production workflow that periodically executes agent-supplied shell commands, persists watches, wakes threads, and changes orchestration and client behavior across 51 files. An unresolved high-severity finding reports that a truncation path can silently lose command output; the PR also adds a new default interval and a line-level lint suppression.

Needs human approval because:

Merge Risk: 🔵 Low · up to 55619

Command watches appear mergeable with owner awareness, but stop controls may still be offered without operate permission. Confirm those controls before release; the possible CI concern also remains open.

Security architecture risk: 🟡 Moderate · up to 10280

Background shell commands can continue after the initiating conversation turn and use the server account’s permissions. Permission checks, execution limits, and lifecycle cancellation constrain that authority. However, restarting resets failure-based stopping, and deployment isolation and downgrade behavior remain incompletely verified.

Retained concerns

  • Low · reliability · observed: Failure-based stopping does not survive restart. Consecutive failures are counted only in an in-memory map, so restarting before the eighth failure allows persisted watches to continue retrying shell commands beyond that failure sequence. This weakens recovery-time failure containment for the newly delegated execution path. Permission checks, the persisted seven-day lifetime, and wake limits still apply; this is not evidence of an authorization bypass.
Security review details

Security Blast Radius

  • inferred — The maximum execution scope is the authority available to the server’s shell process, not merely the selected workspace. Working-directory selection is not a sandbox: commands can potentially access filesystem resources, network destinations, and credentials available to that account. This PR extends that authority into persisted background work; the evidence does not establish a new cross-environment escape or greater OS privilege than existing Full-access execution.

Security Findings and Attack Paths

  • inferred — An adversary controlling content returned by a watched command could influence text delivered to a subsequent Full-access continuation. The message builder explicitly identifies that text as potentially written by others, labels it as output rather than user instructions, and uses a fence longer than embedded backtick runs. These are relevant trust cues, not proof that instruction-following attacks are impossible; no exploit or control bypass was established.

Trust Boundaries and Controls

  • observed — MCP provider credentials are issued with the server’s environment identity and stored in a local credential registry. Authentication resolves the bearer token to that stored scope, and watch creation derives its target thread from the live caller. Project lookup then uses persisted thread ownership and the configured local database rather than a tool-supplied environment or path.

Resilience and Maintainability Implications

  • observed — Server Stop handling clears pull-request and command watches in one thread payload, avoiding restoration by competing full-thread updates. Watch removal and lifecycle events interrupt active reactor fibers, with scanning as a backstop. Synchronization rejects results from ended watch identities, and failed terminal-state writes retain the watch for retry rather than silently losing the outcome.

Hardening Proposals

  • proposed — Make the intended recovery contract explicit. If eight consecutive failures is meant to bound execution across restarts, persist the failure streak with the watch and define its crash/retry semantics. Otherwise, document the limit as process-local so users do not rely on a durable failure-containment guarantee.

76-140: 📐 Maintainability & Code Quality | 🔵 Trivial | ⚖️ Poor tradeoff

Move the watch capability into a domain service and keep the MCP handlers thin.

The watch_command handler does several things itself. It checks capabilities, loads the caller, applies the Full access and subagent policy, generates the watch id, builds and dispatches the orchestration command, and reads the projection again. The unwatch_command handler follows the same pattern. The repository rules say that an MCP tool handler "decodes input, calls one service method, and maps errors". This logic is now reachable only through MCP, so a scheduled task, the CLI, or a WebSocket RPC cannot reuse it without copying it.

Move the start and stop flow, including the watchId lookup after a duplicate start, into a service method such as CommandWatchService.start / stop. Each handler then calls that one method. The refusal mapping can stay as the transport-level error mapping.

As per coding guidelines: "A transport handler does three things: decode the request, call one service method, and map the service's typed errors to the transport's error. Nothing else."

26-26: 📐 Maintainability & Code Quality | 🔵 Trivial | ⚡ Quick win

Import the ProcessRunner service module as a namespace.

The reactor imports the ProcessRunner service tag with a named import. The Effect services guideline says consumers import a service module as a namespace, for example import * as Foo from "./Foo.ts", and then yield* Foo.Foo. runtimeLayer.ts already uses import * as ProcessRunner from "../processRunner.ts". Use the same form here. CommandWatchReactor.test.ts uses the same named import, so update the test the same way.

On the two CodeRabbit nitpicks:

  • ProcessRunner namespace import: done in 9600354.
  • Moving the watch flow into a domain service: leaving it in the MCP handlers for now. Starting a watch is deliberately agent-only through MCP, since the command later runs outside any approval prompt, so there is no second transport to share it with. Clients stop watches through their own RPC command. It also matches watch_pull_request, which dispatches from its handler the same way. Happy to extract a service if maintainers prefer.

Command watcher verification for PR #22182

The discussion, issue, cross-repository CI, deployment-status, and release workflows passed this run. No watcher defect was found in the cases exercised. The run produced 16 expected monitor notifications, and the agent completed a response to each one. Unchanged and filtered activity produced no extra turns.

Tested PR #22182 at commit d8224639befec92f1bb2d0ecabe41e2e9fe32a35. The PR head still matched this commit when checked during testing. The checkout remained clean throughout verification.

Environment and method

  • Date: October 20, 2026. Evidence timestamps use UTC.
  • Runtime: Debian Linux x64, kernel 6.12.111, Node 24.22.0. The host has about 3.3 GB RAM.
  • Client: the web development client, exercised through T3 Code's collaborative Browser panel.
  • Provider: real Codex sessions using GPT-6.1-Sol with Low reasoning and Full access. These sessions called the real watch_command MCP tool. They acknowledged each wake and kept watching until explicitly stopped.
  • Isolation: a detached worktree at the tested commit, its own .t3/userdata/statev2.sqlite, and a small separate Git workspace for the test threads. The database was seeded through migrate-dev-db with stopped history. The live installation's database was never used as writable runtime state.
  • The test workspace's Git remote was pingdotgg/t3code. Every watched GitHub resource belonged to T-A-R-S-BOT/TestRepo, so the CI watch exercised another repository.
  • GitHub checks authenticated as T-A-R-S-BOT. Lenoridev supplied the external comments and replies. Tokens were selected per command without switching the shared active account.
  • Each watch used a one-minute interval. Five independent source-specific threads ran together. Two additional threads checked a blocked subprocess and a discussion already closed with an answer selected.

The five check commands made 48 server-run GitHub checks, including the extra closed-discussion baseline case. Initial manual query smoke checks are excluded from that count. Seven threads completed 23 provider turns, consisting of seven setup turns and 16 turns triggered by watch notifications.

Results were checked against persisted thread projections, monitor notification items, the thread event streams, actual command executions, and agent responses. Live waits used recorded execution or notification events. Automated orchestration tests used their existing event and test-clock mechanisms.

Live results

Case Action and observed result Result
Initial baseline Existing discussion and issue comments, a successful CI run, a pending deployment, and an existing release established baselines. All five watches had zero wakes and one setup turn each. Passed
Unchanged checks Repeated unchanged checks left the output hashes and wake counts unchanged. They started no additional provider turns. Passed
Own-account filtering Added a bot-authored issue comment, a discussion comment, and a nested discussion reply. Two subsequent checks per command left every wake count at zero. Passed
Discussion comment A new Lenoridev top-level comment produced one wake and an agent response. Passed
Nested discussion reply A Lenoridev reply under a bot-authored top-level comment produced the next wake. Filtering the parent author's text did not discard an external reply. Passed
Selected answer Selecting an existing bot-authored answer candidate produced one wake even though its comment body was filtered from ordinary comments. Passed
Discussion closure Closing the discussion produced one wake identifying the closed state. Passed
Discussion reopening Reopening it after a server restart produced the next wake and a completed agent response. Passed
Issue reply and lifecycle An external issue comment, closure, and reopening produced three wakes, one per observed change. Passed
Cross-repository CI The watcher observed a new run in progress, its intentional failure, and a later successful run. Each observed state produced one wake. Passed
Deployment status A GitHub deployment record changed from pending to in progress, then failure, then success. Three wakes matched the three changes. Passed
Release draft Creating an unpublished draft release left the published-release command output unchanged and produced no wake. Passed
Release publication Publishing that draft and later publishing another release produced two wakes. Passed
Already closed and answered A separate watch started after the discussion was closed with an answer selected. Its baseline and next scheduled check produced zero wakes and no additional provider turn. Passed
Stop a blocked command A baseline command wrote its PID and blocked in signal.pause(). Clicking the web composer's Stop button removed its watch and terminated the captured process before the command timeout. No monitor notification followed. Passed
Settle and archive Settling the issue thread and archiving the CI thread each removed its watch. Later checks did not execute. Passed
Permission and interaction changes Moving the deployment thread out of Full access and the release thread into plan mode each removed its watch. Later checks did not execute. Passed
Server restart The active discussion watch retained the same ID, output hash, and wake count. Two post-restart unchanged checks left the thread at five provider runs and four wakes. Reopening then produced run six and wake five. Passed
Per-watch Stop Clicking the discussion watch's Stop control in the thread details panel removed the final watch and cleared the Waiting presentation. Passed
Activity after Stop An external discussion comment after all watches stopped produced no further checks, notifications, or provider turns during the following 136-second observation window. Passed

Final persisted notification counts were discussion 5, issue 3, CI 3, deployment 3, and release 2. The blocked-command and closed-baseline threads each had zero notifications. All 23 provider turns were completed, and all seven test threads had no active command watches at the end.

GitHub fixtures

Focused automated verification

Both commands ran from the repository root with one worker. All eight files passed, with 146 tests in total. No repository-wide checks were run.

vp test run \
  apps/server/src/orchestration-v2/commandWatch.test.ts \
  apps/server/src/orchestration-v2/CommandWatchReactor.test.ts \
  apps/server/src/mcp/toolkits/commandWatches/handlers.test.ts \
  packages/shared/src/orchestrationV2PendingBackgroundWork.test.ts \
  --maxWorkers=1
# 4 files, 68 tests passed.

vp test run \
  apps/server/src/orchestration-v2/ThreadForkService.test.ts \
  apps/server/src/orchestration-v2/ThreadFork.execution.test.ts \
  apps/server/src/orchestration-v2/ThreadStop.test.ts \
  packages/client-runtime/src/operations/commands.test.ts \
  --maxWorkers=1
# 4 files, 78 tests passed.

The watcher tests cover the first baseline, changed and repeated output, empty-output gates, error exits, output limits, the wake limit, expiry, eligibility checks, duplicate commands, the per-thread watch limit, busy-thread behavior, failed wake persistence, interruption of a running check, and fairness between watches. The additional files cover fork, Stop, pending-work, and client command behavior. These are automated results, separate from the live cases above.

Commands and reproduction

The evidence archive contains check_github.py, fixtures.json, and blocking_check.py. The GitHub helper prints deterministic JSON. Discussion checks include reply IDs and bodies, the closed flag, and the selected answer ID. Discussion and issue checks filter the watched account's ordinary comments. The CI helper prints the latest run ID, status, conclusion, and URL. The release helper excludes drafts. The deployment helper prints the status of the fixed test deployment.

To reproduce with an isolated development server, authenticate its watched shell commands as the fixture account before starting that server:

export GH_TOKEN="$(gh auth token --user T-A-R-S-BOT)"
vp run dev --home-dir /path/to/isolated-t3-home

In a Full-access test thread, ask the agent to call the actual MCP tool with this input, changing the final argument to issue, ci, deployment, or release for the other cases:

{
  "command": "python3 /path/to/evidence/check_github.py discussion",
  "label": "QA discussion",
  "everyMinutes": 1
}

Keep the watch active after each wake, perform one fixture change, then await its persisted notification and completed agent response before testing the next change to that resource. Use another GitHub account for external replies. The archived fixture action journal records the mutations and accounts used in this run.

The helper checks pagination flags for discussion comments and replies and fails if its first 100 entries are incomplete. The small test issue had fewer than 100 comments. These fixture commands are examples for this controlled repository, not a complete GitHub monitoring client.

Verification limits

  • Live provider coverage was Codex on Linux. Other provider adapters were not exercised live.
  • Native Electron, iOS, and Android clients were not run. The integrated client pass used the web UI through the Browser panel. Responsive browser rendering was not counted as native mobile evidence.
  • GitHub Actions runs and release publications were real. Deployment transitions used controlled GitHub API records, with no hosting service behind them.
  • The client and server used direct development connections. T3 Connect relay, tunnel connections, and offline recovery beyond the deliberate server restart were not tested.
  • Seven-day expiry, eight consecutive failures, the ten-wake limit, fork behavior, and output-limit cases were checked by the focused automated tests rather than prolonged live runs.
  • The Stop-versus-new-turn race documented in the PR was not reproduced or claimed fixed by this run.
  • The host experienced slow initial development-client loading and event-loop stalls during cold startup. This run was a functional test, not a performance benchmark.

The isolated dev server and test state remain available for inspection. Every test watch was stopped. No implementation changes or pushes to the contributor's branch were made.

Tested and reported by GPT-6.1-Sol through the Codex harness in T3 Code, on behalf of Lenoridev.

@Lenoridev thank you, this is a thorough run. Covering five sources at once, the already closed and answered baseline, the blocked command on Stop, and the restart case goes well beyond what I could check locally. I linked your report from the PR description under Verification.

watch_command runs a shell check for a thread on an interval and wakes the
agent only when it prints something new, so waiting on a discussion reply,
another repository's CI or a deploy no longer needs an external poller or a
scheduled task that starts a turn on every run. The first run records a
baseline, so a state that already holds when the watch starts never wakes
the agent. The thread stays Waiting in Working while it watches. Stop,
settle, archive, leaving Full access, the thread details card and the mobile
Waiting pill end a watch.
