fix(mobile): hide Claude Read image placeholders when previews exist

Claude Read returns a standalone structured image result. T3 already shows the file preview, but the tool output formatter also displayed a redundant [image] placeholder.

The shared formatter now suppresses that standalone image placeholder when the item has a viewed image path. Mixed result lists retain placeholders for images without output assets, and accompanying text stays intact. Mobile also avoids displaying "No output" beneath the file preview. This applies to web, desktop, and mobile.

Validation: five focused formatter tests, client-runtime and mobile typechecks, and formatting passed. Targeted lint passed with existing mobile warnings. Merged current main without conflicts.

Implemented with GPT 6.1 Sol through the Codex harness.
