fix(server): Stop no longer leaves a killed background command showing as running - #18163

…g as running Stop's settle skipped the roster of every provider thread with a live session, trusting the process to report the clear. When a later run's start failed it took the provider thread's lastRunOrdinal, so the earlier run's ownership-gated roster clear was dropped and the thread kept listing the killed command. The provider thread Stop just ended now gets its roster cleared like its turn items already are. Co-Authored-By: Claude Opus 5.5 <noreply@anthropic.com>
ApprovabilityVerdict: Approved at Macroscope's review found this PR approvable — This is a small, focused server bug fix: Stop now clears stale background-task state for the provider thread it actually interrupted, while leaving other live provider threads unchanged. A targeted integration test covers the regression, and no defaults, schemas, infrastructure, sensitive packages, or diagnostic suppressions are modified. Notes:

979-982: 🎯 Functional Correctness | 🔵 Trivial | ⚡ Quick winExercise preservation of another live thread's roster.
The fixture gives the other live-session provider thread an empty roster and asserts only that the stopped thread is cleared. A settlement regression that clears every live-session provider thread would therefore pass this test. Give the other thread a nonempty roster when
staleRosteris enabled, then assert that Stop preserves it.Suggested fix

Independent confirmation: I hit the same bug and reached the same diagnosis and the same one-condition fix. My case was a A regression case with a Monitor on the interrupted provider thread and a failed newest run fails on |
Problem
A Claude thread can keep showing a background command as running after Stop has killed it, and pressing Stop again changes nothing.
How I hit it: a turn started
sleep 1800withrun_in_background. I then sent a follow-up at a different effort. The Claude adapter correctly refused it with "Claude is still running background agents or commands, and this model or setting change would end them... press Stop". I pressed Stop. The CLI process closed and the shell was killed (its task output reads[killed]). The thread still lists the command, though, and its provider thread projection keepspendingBackgroundTasks: [{ taskId: "boul7ifcc", ... }]. Twelve more presses of Stop each ranClaudeAdapterV2.interruptTurnandthread.background-work.settlesuccessfully, and none of them cleared it.Mechanism, from the server trace:
startTurn, but by then it had already taken the provider thread'slastRunOrdinal.provider_thread.updatedwith an empty roster. Only run 19's event stream routes it, and run 19 writes it throughwriteIfProviderThreadOwnerwithexpectedLastRunOrdinal: 19. The row holds 20, so the clear was dropped.settleBackgroundWorkthen skips any provider thread with a live session ("A live process owns its roster and reports clearing it"). The session is still registered, so nothing clears the projection.Change
In
settleBackgroundWork, the provider-thread roster loop now treats the provider thread that Stop just ended the same way the turn-item loop above it already does. Its roster is cleared even when the session is live. Other provider threads with live sessions still own their rosters. This matches the function's doc comment, which says it ends work "on the provider thread whose interrupt just returned (stoppedProviderThreadId)".I left the ownership gate alone. It correctly stops stale runs from writing over newer state. The settle step runs after the adapter's interrupt has returned, and the item loop already treats that point as authoritative.
Scope and approval
There is no prior issue. I think this qualifies as a small, focused fix for an obvious bug: a three-line condition change plus one regression test, with no change to the product's behaviour beyond making Stop clear what it has already stopped.
Verification
BackgroundWorkStop.integration.test.ts. The live Codex provider thread gets a projected roster that its process never clears, and Stop must empty it. Without the fix the test fails withexpected [ { taskId: 'sleep', …(2) } ] to deeply equal []. With the fix all 13 tests in the file pass.vp test runon the 20 other server test files that referencependingBackgroundTasks,background-work.settleor the Claude background-task fixtures: all pass.src/storageCleanup.test.tswas also matched and fails onmainwithout this change, so it is unrelated.tsc --noEmitforapps/server,vp fmt --checkandvp linton the changed files all pass.Written by claude-opus-5-5 in Claude Code (T3 Code).
