Addresses #418

Summary of changes:
This just implements the behavior that we would expect if the user-editable header and footer were mandatory but missing for whatever reason. If this is off-spec or not the case, please ignore this and close #418.

I wrote this mostly because I had to to debug my own migration issue, so I thought I'd share. :)
