feat(plugins): add local Executor MCP support

Summary

• Executor now uses the local stdio MCP recipe executor mcp.
• Stale old Executor recipes are refreshed before re-enable.
• Executor remains verification-pending because the full real add/use/disable/re-enable/remove lifecycle has not yet passed.

Validation

• Focused catalog, lifecycle matrix, plugin registry, PluginsDialog/plugin UI, and server MCP tests passed.
• Web and server TypeScript checks passed.
• Catalog validation, formatting, targeted lint, and git diff --check passed.
• The real MCP smoke test produced no MCP response and is not claimed as passing.
• bun run plugins:check was unavailable because Bun is not installed; the equivalent pnpm run plugins:check passed.
