Confirmed independently on macOS with

Invalid prompt: your prompt was flagged as potentially violating our usage policyerror:codex exec --ephemeral --ignore-user-config --ignore-rules --skip-git-repo-check \ -C /private/tmp -s read-only -m gpt-5.6-sol --json 'Reply only OK'
This rules out workspace instructions, repository context, and the user config in our case. GPT-6 Astra had already been affected; GPT-5.6 Sol was initially usable and then began failing with the identical error later the same day.
