Generalize NUnit.Extensibility for broader use

Enable use of the extensibility package outside of the NUnit engine and even outside of NUnit itself.

While the functionality needed by NUnit should remain, NUnit conventions should be optional. For example, the "NUnit.Extension." package name prefix should be available but not required and users should be able to set the desired prefix.
