BREAKING CHANGE(access): remove old commands

Summary

Predicated to #477; npm access should have a clear set of commands & functionality. Removing old subcommands should help provide better clarity.

Exit Criteria

• remove 2fa-required
• remove 2fa-not-required
• remove public
• remove restricted
• remove edit (note: docs already say this should exist but doesn't, let's just drop it)
• remove ls-packages
• remove ls-collaborators
