…ith rename (#7426) Establish clear terminology: Middleware (lifecycle hooks), Skills (user-facing capabilities), Plugins (standalone packages). Migrate extensions/ → middleware/ with re-export shim to maintain backward compatibility during one-release deprecation period. Update 3 call sites in core modules and add decision document.
