Closes #15663

Single-issue rationale: the rule and its worked instance are one change. A store-authority table nothing obeys is a document, and moving knowledge facts to a durable home without stating the rule fixes one datum and leaves the class open — which is how #12733 arrived after the same failure had already been described elsewhere.

Thinking Path

The issue's open decision resolved itself in the code. autobot-backend/user_management/config.py:173 sets postgres_enabled = True unconditionally with the comment "AutoBot always runs full, Postgres-backed user management (#10636)", so the single-user escape hatch that would have argued for a disk store is gone: Postgres is present in every deployment, autobot-backend/migrations/ is live, and user_management.database.get_async_session_factory() is already how llc/deps.py gets a session. ChromaDB was the tempting cheap answer and is the wrong one — _vectorize_fact_in_chromadb writes sanitize_metadata_for_chromadb(metadata), which is lossy, and a fact whose embedding fails never reaches Chroma at all, so the store that would have "already had the data" does not have all of it.

Three choices worth naming:

  • The trigger is "writes two stores", not "writes a store". A module writing one place cannot disagree with itself. A module durably writing two has a second copy by construction, and the only question left is which one wins — which is exactly what the table answers. The detector and the table therefore share one definition instead of approximating each other.
  • Both call shapes count, and this is not a detail. knowledge/facts.py — the module knowledge: browser empty + 'unvectorized on every reload' — facts (Redis) wiped 43→0 while 22 ChromaDB vectors remain; facts not durably persisted; vectorized status read from Redis flag not the vector store #12733 is about — never calls a Redis method directly; every write is asyncio.to_thread(self.redis_client.hset, ...), a bare attribute reference. A first version of the detector read only ast.Call, reported 24 dual-store modules, and silently omitted the one the issue was filed for. Reading ast.Attribute as well found 27, knowledge/facts.py among them.
  • "Don't know" is not "no". vectorized_ids() returns None rather than an empty set when ChromaDB is unreachable. A false "nothing is vectorized" makes the browser lie, and in the reconciler it would re-embed the entire knowledge base; the opposite mistake — skipping — loses a fact from search indefinitely, so the reconciler processes the whole batch on a "don't know".

The reconciler is where the real damage lived. background_vectorization._get_vectorization_status read vectorization_status off the Redis hash and _filter_pending_facts skipped anything stamped completed. That is a claim about ChromaDB written next to the fact, consulted by the one component whose job is to repair drift between them: a fact whose vector was lost but whose stamp survived was skipped forever. The status endpoint had already been fixed for #12733; the repair loop had not, and the two disagreed about what "vectorized" means.

What Changed

  • autobot_shared/store_authority.py (new) — Store, a frozen Concept(name, system_of_record, projections, write_sites, rebuilt_by, note), and the table: 13 concepts across 24 write sites. system_of_record("<name>") is the module-level lookup a write site calls, so the authority is one jump from the copy and a typo fails at import rather than rotting in a doc. Three concepts declare Redis as their system of record — auth_sessions, device_pairing_challenge, autoresearch_proposal — each with the justification the guard requires.
  • autobot-backend/models/knowledge_fact.py + migrations/versions/20260825_085_knowledge_facts.py (new) — the knowledge_facts table. NO DATA LOSS: one additive table, no Redis key touched, has_table-guarded, with a working downgrade.
  • knowledge/fact_store.py (new) — the durable reads and writes. Deliberately not best-effort: a projection failure is repaired by the reconciler, but a row that was never written is a fact the user believes they stored.
  • knowledge/facts.py — _store_fact_in_redis becomes _project_fact_to_redis, and _store_and_vectorize_fact writes the row first. _read_fact_for_write reads Redis and falls back to the row, which is the knowledge: browser empty + 'unvectorized on every reload' — facts (Redis) wiped 43→0 while 22 ChromaDB vectors remain; facts not durably persisted; vectorized status read from Redis flag not the vector store #12733 case: hashes gone, so every fact "did not exist" while the knowledge was still recoverable. Both dedup lookups fall back the same way — answering "no duplicate" from an empty index is how one fact becomes two. rebuild_fact_projections() makes rule 2's promise executable.
  • knowledge/vector_membership.py (new) — vectorized_ids(), moved out of api/knowledge_vectorization.py so the endpoint and the reconciler answer the question identically. background_vectorization now decides from membership; knowledge/vector_repair.py's docstring is corrected, because its premise was the completed stamp and it remains necessary for a different reason (an empty document is still a document).
  • repo_tests/store_authority_test.py + store_authority_baseline.txt (new) — nine tests, shaped after audio_extension_allowlist_test.py. The last is the regression: a new dual-store module with no declaration fails. Eight existing ones are baselined, shrink-only.

Verification

The detector, both directions. 5438 tracked .py files walked; 27 dual-store modules found, 24 write sites declared, 8 undeclared and baselined — the baseline file matches the detector's output exactly, so test_no_undeclared_dual_store_module_exists and test_the_baseline_only_shrinks both pass on this tree. Removing the ast.Attribute shape drops the count to 24 and knowledge/facts.py disappears from it, which is the regression the module docstring records.

Rule 3, structurally. test_vectorized_status_is_derived_not_stored parses background_vectorization.py and fails if either deciding function names "vectorization_status" as a string literal in its body (docstrings excluded by AST, not by text matching — the function is called _get_vectorization_status, so a substring check would have been unfalsifiable). It also asserts the endpoint imports the shared helper.

The #12733 scenario, as a test. test_a_fact_survives_its_redis_projection_being_gone empties Redis and asserts the fact still reads; test_the_durable_row_is_written_before_the_redis_projection pins the order; test_every_redis_key_is_rebuilt_from_the_rows exercises the rebuild. test_an_unknown_fact_is_still_unknown holds the other direction, so the fallback cannot invent facts.

Updated rather than deleted. test_background_vectorization_pending_set.py's filter test now expresses membership, with a second case for the unreachable store; test_facts_dedup.py patches the durable write it now goes through.

Lint. flake8 and black clean on every changed file. CI is the authority for the suite.

Newly surfaced, and how it was handled. Eight modules write two stores with no declared authority. One is not a bookkeeping gap: services/autoresearch/store.py writes experiments with hset/zadd/sadd, no TTL and no second store — the same shape as #12733, on a different datum. Filed rather than fixed here, and recorded at the baseline entry.

Summary by CodeRabbit

  • New Features
    • Knowledge facts are now stored durably, supporting persistence, updates, deletion, deduplication and recovery.
    • Derived cache and vector-search data can be rebuilt from durable knowledge records.
    • Legacy knowledge records can be adopted into durable storage automatically.
  • Bug Fixes
    • Knowledge remains available when cached data is missing.
    • Vectorisation status now reflects facts actually present in the vector store.
    • Processing continues safely when the vector store is temporarily unavailable.
  • Reliability
    • Improved consistency between persisted knowledge and derived search data.
    • Durable records are now prioritised over rebuildable cached data.

…ecord (#15663)

No datum declared which store owned it, so the same fact lived in Redis,
Postgres and ChromaDB with nothing saying which copy wins. #12733 is the bill:
facts/by_category went 43 -> 0 while 22 ChromaDB vectors survived, because the
Redis hash was the only place a fact had ever been written.

autobot_shared/store_authority.py states the rule and names the authority for
thirteen concepts; system_of_record() makes it reachable from the write site
rather than from a document. Knowledge facts move to a knowledge_facts table as
their system of record, with the fact:* hashes and the vectors as projections
rebuilt by rebuild_fact_projections(). Vectorised status is asked of ChromaDB
through knowledge/vector_membership.py by both readers -- the status endpoint
and the reconciler, which used to skip any fact stamped completed and so could
never repair a vector that had gone missing underneath one.

repo_tests/store_authority_test.py fails when a module starts durably writing a
second store without declaring which copy is authoritative; the eight modules
already in that state are recorded in a shrink-only baseline.

Merge Risk: 🟡 Moderate · up to 94c8e

This change makes PostgreSQL the durable source for knowledge facts and rebuilds Redis and vector projections, but unresolved concurrency and projection-consistency issues can still produce duplicate, stale, missing, or altered user knowledge. These risks should be addressed before merge.

Add durable fallbacks to the public read paths.

After Redis projection loss, get_fact returns None and get_all_facts returns an empty list even when durable rows exist. Only write-oriented reads currently recover from PostgreSQL. This leaves stored user knowledge invisible until an operator manually rebuilds Redis.

  • autobot-backend/knowledge/facts.py#L956-L957: load the durable row when the Redis hash is absent, then restore the projection if appropriate.
  • autobot-backend/knowledge/facts.py#L1090-L1093: enumerate durable rows when Redis has no fact keys, while preserving collection and pagination semantics.

Is Inspector exported at the top level of the sqlalchemy package in SQLAlchemy 2.0?

💡 Result:

Yes, the Inspector class is exported at the top level of the sqlalchemy package in SQLAlchemy 2.0 [1]. You can access it directly via sqlalchemy.Inspector [1]. However, the recommended and idiomatic way to obtain an Inspector object in SQLAlchemy 2.0 is to use the sqlalchemy.inspect function, which provides a consistent, public-facing interface for runtime inspection [2]. While you can import Inspector directly, creating instances via inspect(engine) or inspect(connection) is the standard practice [3][2][4].

Maintain updated_at on fact updates. The migration defines only an INSERT-time default, and KnowledgeFact does not map the column. Therefore, persist_fact and update_fact can leave updated_at at its creation time. Add onupdate or set the timestamp explicitly.

Make content_hash deduplication atomic.

FactsMixin uses the global content_hash key, but fact_store.persist_fact performs a read followed by session.add(...). Concurrent creates can therefore commit different fact IDs. Add a database uniqueness constraint and use a conflict-aware insert that returns the existing fact ID before projecting the fact. Reconcile any existing duplicate rows in the migration.

…r both ceilings (#15663)

The file-size ratchet caught two things. knowledge/facts.py is a grandfathered
file, and a grandfathered exemption freezes the size it was granted for rather
than licensing more — it had grown 1857 -> 1933. api/knowledge_vectorization.py
shrank 1720 -> 1702 when the membership helper moved out, and an unlowered
ceiling re-licenses the lines just cut.

knowledge/fact_projection.py takes the methods that only make sense once the row
rather than the hash is the fact: the read that settles existence from the row,
the two lookups that fall back to it, the vector observation, and the rebuild.
The seam is real independently of the line count — all of it is about the
relationship between the stores, not about what a fact is.

fact_store is imported inside each method rather than at module scope: it pulls
SQLAlchemy, which requirements-ci.txt does not install, so a module-level import
would break every importer of facts.py in the startup-import smoke environment
instead of only the paths that touch the durable store.

Ceilings lowered to the measured sizes in both scripts/python_file_size_known_large.py
and repo_tests/python_file_size_ratchet_baseline.py; the audit reports 5116 files
scanned, 497 grandfathered, all live and at size.

Preserve the durable creation timestamp during projection rebuild.

rebuild_fact_projections calls this method for every durable row. Line 657 then replaces each Redis timestamp with the rebuild time. get_all_facts and pruning use this value, so a rebuild makes all facts appear newly created and delays eligible pruning. Add durable fallback to public fact reads.

These paths still treat an absent Redis projection as an absent fact. Add a contrast pair for stores_written.

stores_written is the detector. No test calls it on a known input. test_the_baseline_only_shrinks and test_no_undeclared_dual_store_module_exists both run against the live tree, so the matching logic at lines 110-112 is never pinned.

The failure mode is silent. If a receiver hint or a method name in _WRITES stops matching, for example after a rename of redis_client or a switch from hset to a pipeline helper, Redis detection disappears. dual_store_modules() then shrinks and test_no_undeclared_dual_store_module_exists still passes.

_is_test exempts production modules whose filename contains "test".

Bind the vacuity floor to parse reach, not to path discovery.

_MIN_MODULES_SCANNED counts the paths that git ls-files returns at line 118. It does not count the files that stores_written parsed.

The gap is reachable. If ast.parse starts failing across the tree, the except SyntaxError branch at lines 102-103 returns an empty set for every file, dual_store_modules() returns {}, and test_no_undeclared_dual_store_module_exists passes on a tree it never read. test_the_walk_reaches_the_tree still passes, because the paths are still listed. test_the_baseline_only_shrinks covers this only while the baseline holds entries, and the baseline is shrink-only.

Maintain updated_at on fact updates.

Line 67 defines server_default=sa.func.now(), which applies at INSERT only. PostgreSQL does not refresh a server_default on UPDATE.

persist_fact updates an existing row by assigning the columns that _row_values(content, metadata) returns (autobot-backend/knowledge/fact_store.py lines 62-72). If _row_values does not include updated_at, an updated fact keeps its creation timestamp. Add onupdate=sa.func.now() to the column, or set the timestamp explicitly in the write path.

Enforce uniqueness for unique_key.

Line 70 creates a non-unique index. Nothing else enforces one row per key.

persist_fact reads the row with session.get and then inserts a new KnowledgeFact when the read returns None (autobot-backend/knowledge/fact_store.py lines 62-72). That is a check-then-insert with no constraint behind it, so two concurrent ingests of the same unique_key can both insert. fact_id_for_unique_key() then resolves one of several durable rows for the same key, and the Redis unique_key:* projection points at whichever won last.

Create a unique index instead. PostgreSQL allows multiple NULLs in a unique index, so the nullable column stays workable. Mirror the constraint in the ORM model, and handle the conflict in persist_fact().

LlamaIndex 0.14 VectorStoreIndex.insert Document doc_id node id ChromaVectorStore node_to_metadata_dict

💡 Result:

In LlamaIndex, the insertion and metadata management processes are handled through a combination of the VectorStoreIndex class and utility functions that bridge nodes with specific vector stores [1][2][3]. VectorStoreIndex.insert The insert method allows you to add a new Document object to an existing index after it has been initialized [4]. When you call index.insert(doc), the index breaks the document down into nodes (if not already defined) and ingests them into the underlying vector store [4]. While the Document has its own id_ (or doc_id), it is important to note that the index decomposes this into individual Node objects, each with its own unique node_id [4][5]. ChromaVectorStore and node_to_metadata_dict The ChromaVectorStore integration uses the node_to_metadata_dict utility function to process nodes before they are added to the Chroma collection [2][3]. This function is responsible for converting BaseNode objects into a dictionary format compatible with metadata storage [6]. Key aspects of this process include: - Metadata Serialization: node_to_metadata_dict extracts metadata from the node and includes serialized node content (as a JSON string under _node_content) and the node type [6]. - ID Mapping: The utility explicitly maps the node's reference document ID (ref_doc_id) to standard metadata keys like document_id, doc_id, and ref_doc_id to ensure compatibility across different vector store backends [6]. - Chroma Integration: Inside the ChromaVectorStore.add method, this dictionary is further processed to ensure no values are None (converting them to empty strings) before being passed to the underlying Chroma collection [2][3]. The node's node_id is used as the primary ID for the vector record in Chroma, while the processed metadata dictionary is stored alongside the embedding and document content [2][7]. If you are experiencing issues where a custom document ID is not appearing correctly, ensure that the node_id is explicitly set on the node object during creation, as LlamaIndex may otherwise assign a default UUID [5].

Query ChromaDB by the stored record ID or metadata fact ID.

When VectorStoreIndex.insert creates a node, ChromaDB uses the node UUID as the record ID and stores fact_id in metadata. collection.get(ids=wanted) can therefore miss existing vectors and trigger repeated vectorisation. Reuse the metadata-aware reachability logic and test both record-ID shapes.

backfilled by ``FactsMixin.rebuild_fact_projections(direction="from_redis")``,
which is a runtime repair rather than a migration step: it needs a Redis
connection, and the migration gate installs no autobot packages at all.

Add a Redis-to-database backfill before merging. FactsMixin.rebuild_fact_projections accepts no direction parameter and rebuilds Redis from knowledge_facts; it does not write Redis facts to the table. This migration therefore leaves existing Redis-only facts without durable rows and can expose them to data loss.

Enforce atomic uniqueness for non-null unique_key values.

Concurrent /research requests can both pass store_fact's duplicate check before persist_fact commits. The migration creates only a non-unique index, so both rows can persist. fact_id_for_unique_key() then returns an arbitrary row because it uses LIMIT 1. The separate content_hash check does not enforce unique_key idempotency. Add a partial unique index and handle conflicts atomically.

Persist share metadata before updating Redis.

_share_single_fact changes shared_with, shared_by, and shared_at only in the Redis projection. A full Redis loss followed by rebuild_fact_projections reloads the unchanged knowledge_facts metadata, so the accepted share is not reconstructed.

Load and update the durable fact row first. Then regenerate the Redis hash and recipient indexes from that durable metadata.

The PR objectives require knowledge facts to use a durable system of record.

Validate Redis projection hits against knowledge_facts.

A Redis projection can remain stale after an interrupted projection write or failed cleanup. _read_fact_for_write then returns stale content and metadata, and update_fact writes that stale state back to the durable row. The two deduplication paths can also return false duplicates from stale Redis indexes.

Preserve the recorded timestamp during projection rebuild.

iter_facts supplies the fact metadata, and _prepare_fact_metadata stores its timestamp there. _project_fact_to_redis writes the current time as the Redis hash timestamp. A rebuild therefore changes each fact timestamp. consolidate_facts uses that hash timestamp before metadata, so old facts become ineligible for age-based pruning after a rebuild.

Pass the recorded timestamp through the rebuild payload, or make _project_fact_to_redis use metadata["timestamp"] when available.

…lifecycle results (#15663)

Review found the gap that mattered: the migration creates knowledge_facts EMPTY
and cannot do otherwise — the migration gate has no Redis connection — so every
fact that existed before this change stayed Redis-only. The durable system of
record was a promise about future writes while the facts already at risk stayed
at risk, and rebuild_fact_projections only ever ran Postgres -> Redis.

adopt_legacy_facts() is the other direction, wired into knowledge_factory so it
runs whichever way the KB is constructed rather than living as a script somebody
must remember. Idempotent: a fact whose row exists is skipped.

Four more findings, each verified against the code first:

* iter_facts paged on a UUID4, which carries no order — a row committed below
the cursor was never seen again and the rebuild reported success having
skipped it. Now keyset-paged on (created_at, id) under a captured ceiling.
* update_fact and delete_fact returned a bool that nothing read. An update now
distinguishes a pre-#15663 fact (adopt it) from a concurrently deleted one (do
not resurrect its projections), and a delete that removed neither row nor hash
stops before decrementing the counters twice.
* _is_test matched "test" anywhere in a filename, exempting seventeen production
  modules — env_registry_testing.py, testing_pattern_analyzer.py and others —
from the dual-store detector. Now anchored to the naming convention.
* The vacuity floor counted files git listed, not files that parsed, so a
tree-wide parse failure would have read as a clean tree. It counts parses now,
and a contrast pair pins the detector on known input in both directions.

updated_at was reported as unmaintained; it is not — Base declares
onupdate=func.now() and KnowledgeFact inherits it.

facts.py stayed under its ceiling by moving the lifecycle helpers into
fact_projection.py rather than by raising the number, which is what the ratchet
exists to prevent.

Review response — 10 findings, 7 fixed, 1 rejected, 2 deferred with an issue

Pushed in c7f575a. Each was checked against the code before touching anything.

Fixed

Legacy facts were still Redis-only (20260825_085:73, Critical) — the finding that mattered. The migration creates knowledge_facts empty and cannot do otherwise: the migration gate installs no autobot packages and has no Redis connection. So on the first boot after this change every pre-existing fact was still Redis-only, rebuild_fact_projections() only ran Postgres→Redis, and the durable system of record was a promise about future writes while the facts already at risk stayed at risk. adopt_legacy_facts() is the other direction, wired into knowledge_factory so it runs whichever way the KB is constructed rather than living as a script somebody must remember. Idempotent — a fact whose row exists is skipped. The migration docstring named a direction="from_redis" parameter that never existed; that was a lie and it is corrected.

iter_facts could skip a row (fact_store.py:132) — confirmed. id is a UUID4, so it carries no order and a row committed below the cursor was never seen again; a rebuild would report success having silently skipped a fact. Now keyset-paged on (created_at, id) under a ceiling captured at the start. Rows created during the walk need no chasing — they are projected by their own write path.

Conditional durable outcomes were ignored (facts.py:1140) — confirmed, both sites. An update now distinguishes the two ways to get False: a fact created before #15663 never had a row, so its update is the moment to adopt it; a fact deleted concurrently must not be resurrected as projections with no fact behind them. The Redis key tells them apart. A delete that removed neither the row nor the hash stops before decrementing the counters a second time — Redis DEL returns the count, so no extra round trip.

_is_test exempted production modules (store_authority_test.py:124) — confirmed and worse than the examples given: seventeen tracked modules in this tree match, including autobot_shared/env_registry_testing.py and code_intelligence/testing_pattern_analyzer.py. Anchored to the naming convention. None of the seventeen is currently dual-store, so the baseline is unchanged.

The vacuity floor measured discovery, not parse reach (store_authority_test.py:72) — confirmed. It now counts files that actually parsed, which is the number that would collapse if ast.parse began failing tree-wide.

No contrast pair for the detector (store_authority_test.py:113) — added, and it immediately earned its place: the first version asserted p.write_text(text) yields {DISK} and the pre-push hook failed it. The detector requires a path/file receiver, so that shape was never matched. The test now pins the real behaviour and the limitation, with the reason widening it would be wrong.

Unmocked durable lookup in the dedup tests (test_facts_dedup.py:225) — confirmed and fixed at both sites.

While fixing these I found a related problem review did not flag: five patches in test_fact_store_authority.py still named knowledge.facts.asyncio.to_thread after the methods moved to fact_projection.py, so they were silently applying to nothing. Repointed.

Rejected

"Maintain updated_at on fact updates" (20260825_085:46) — not valid. Base declares updated_at with onupdate=func.now() (autobot_shared/user_management/models/base.py:89-93) and KnowledgeFact inherits it, so both the ORM path and the Core update() in record_vector_seen stamp it.

Deferred — #15673

unique_key uniqueness (:61) and content_hash atomicity (knowledge_fact.py:49) are both real, and both change what the system accepts, which is a data decision rather than a refactor.

unique_key cannot gain a constraint before the adoption runs: the Redis index it replaces is last-write-wins, so two facts sharing a unique_key is a state today's system reaches without complaint, and adopt_legacy_facts() would fail partway on any deployment carrying such a pair — trading a soft inconsistency for hard data loss.

content_hash is SHA-256 truncated to 64 bits to match the Redis key width. Made unique, a truncated-hash collision would reject a legitimately different fact rather than merely mis-deduplicate it, which is the worse failure; the column has to widen first, or dedup stays advisory behind an application-level lock.

Neither race is introduced here — the Redis path had the same read-then-write shape.

🗄️ Data Integrity & Integration | 🟠 Major | 🏗️ Heavy lift

Serialise legacy adoption and deletion for each fact.

_durable_update_or_adopt checks Redis with exists before persist_fact, while _durable_delete deletes the durable row and Redis hash in separate operations. If adoption sees the legacy hash, deletion removes it, and adoption then calls persist_fact, the deleted fact is recreated. Use synchronisation shared by all callers, or an atomic durable-store/Redis protocol that prevents adoption after deletion.

In `@autobot-backend/knowledge/fact_projection.py` at line 164, Synchronize the
adoption and deletion paths for each fact so they cannot interleave between the
Redis existence check and persist_fact. Update _durable_update_or_adopt and
_durable_delete to use shared per-fact synchronization or an equivalent atomic
durable-store/Redis protocol, ensuring deletion cannot be followed by recreation
through persist_fact.

Review pass on handed-over work. I did not write this PR and nobody on my side had reviewed it, so every claim in the body was treated as unverified and checked against the code.

Verdict: sound on everything load-bearing. Two structural gaps fixed or filed; neither is a data-loss risk in this change.

Confirmed by reading the paths, not the docstrings

Claim Verified
Durable write first, not write-all-three-and-hope _store_and_vectorize_fact writes Postgres (allowed to raise), then the Redis projection, then ChromaDB
The migration survives a rolling update Purely additive, has_table-guarded, with a working downgrade that drops only the new table and never touches Redis. A node running new code before the migration surfaces an error response, not silent loss — and the deploy path runs Alembic before the service restart anyway
None ≠ empty set for vectorized_ids() Exactly two callers repo-wide, both test is None explicitly. No caller reads it as falsy
The reconciler cannot delete on an unreachable store _filter_pending_facts(batch, None) returns the whole batch with no completed keys, so nothing is ever SREM'd. Worst case is redundant re-embedding

That third and fourth row are the #12733 shape specifically — a reconciler reading empty and acting on it — and they are handled the right way round.

Fixed here

The baseline had no growth ceiling. test_the_baseline_only_shrinks compares live output against the baseline and checks entries are still dual-store; both hold for a change that adds a new undeclared module and its baseline line together. That is not hypothetical — python_file_size_ratchet_test.py grew test_known_large_entry_count_may_not_grow after exactly that shape slipped 509 entries in.

Added MAX_BASELINE_ENTRIES = 8 and a count test mirroring that precedent, and demonstrated the gap: with a two-sided addition the set comparison returns [] while the ceiling fails.

The header promised something the parser rejected. It says "see the issue linked beside it" and no entry had one — because baseline_entries() took the whole line, so an inline # #15670 would have been baked into the path and every entry would have read as both an undeclared finding and a stale record. I hit that when adding them. The parser now strips an inline comment, and each entry carries its tracking issue.

Filed, not folded in — #15692

rebuild_fact_projections() is the promised rule-2 reconstruction, unit-tested, and nothing calls it. by_category still reads only the Redis index.

So the data now survives a wipe and the screen still goes blank exactly as in #12733 — recovery needs an operator who knows the function exists. Two of the three things that made that incident bad are still true, and #12733's own recommendation 4 is still open.

That needs a decision about where the rebuild runs (read-time fallback, startup reconciler, or admin endpoint), which is why it is an issue with options rather than a commit here.
