Phase D3 of #8994 — v1 OPTIONAL / lower priority.

Surface SSO provider health: failed-auth counts, last-success per provider. Parent sub-umbrella #8994 (under #9930). Design: docs/superpowers/specs/2026-06-15-enterprise-sso-federation-design.md (branch issue-8994).
