Chat history not refreshing after creating new session - requires extension restart

Description:
After starting a new chat session and having a conversation with Copilot,
the new session doesn't appear in the chat history list. The only workaround
is to disable the GitHub Copilot Chat extension, restart extensions, and
re-enable AI features to refresh the session list.

Steps to Reproduce:

1. Start a new chat session
2. Have a conversation with Copilot
3. Return to the chat sessions list
4. The latest session is not visible

Expected Behavior:
New sessions should appear automatically in the session list without
requiring an extension restart.

Workaround:
Extensions → GitHub Copilot Chat → Disable → Restart Extensions →
Enable AI features
