context-graph: release the read path; publish hygm - #428

Bump agent-context-graph to 0.3.0, actions-graph to 0.3.0 and sessions-graph to 0.6.0, raising sessions-graph's floors on the first two to the versions that carry message text and the tool entry point. hygm has never been published and unstructured2graph 0.6.0 depends on it, so add release-hygm.yaml. The release skill now lists every published package and the order to release them in.
