Lots of updates regarding keying

Still unfinished but some important bugs fixed.

Namely:

• installation parameters honoured during instance creation
• instance creating fixed for CAP-s
• rename packag to pro.javacard.gp
• display CAP content with -cap <file> -v
• show a more informative version string with -version
• more privileges and applet locking/unlocking
