phy: mapphone-mdm6600: Handle USB interface wake-up

UART and USB interfaces use separate gpio pins to wake-up the host.
Currently only UART gpio is handled, which leads to AT and QMI interfaces
to miss events.

Fix that by handling USB wake-up gpio as well. PM resume AT interface on
USB wake-up so both AT and QMI interfaces to not miss events.

bq2415x_charger: report events to userspace (#12)

The bq2415x does not have interrupts or other means to indicate charger
insertion and removal events in real-time. In order to offer userspace an
option to avoid polling, unconditionally trigger an update of the charger
state whenever the notify device reports any events.
