Remove non-standard features of madevent application.

Main example (probably not the only one): the input file that is piped to madevent in

madevent < input_file

should not have the two leading arguments that have been added for cudacpp.

These could be added via environment variables or better solutions such as the runcard (edited: runcard, not parameter card). @roiser has been working on this in #620, I think using the parameter card file.

This is part of point 3 in #576.

Yes, I have added them as parameters in the card file but I'm also consuming them then to produce the numbers in the input_app.txt and input_sg.txt files that are piped into madevent (plus admittedly in one case I use environment variables when doing the refine.sh ;-))

Hi @roiser I will now start to work on this as I just completed the gtest MR #666 (took a bit longer than I was hoping - so sorry I could not discuss this with you yet...).

I think that this issue #658 is really a container for sevarl sub-issues. One is how to choose fortran vs cpp vs cuda MEs in the launch infrastructure. I moved that to a separate #674
