feat(extensions): security hardening (pin, CSP, permissions) - #227

Conversation
Pin each switched-on extension to a folder digest; changed files stop it until re-enabled. Add manifest permissions with event filtering, CSP and security headers, a sandboxed QR page, icons as CSS masks, capped replies and logs, cross-site request rejection, and a --no-extensions switch.
Raise css budget for the number input, pairing progress and Elgato restart row. Use queuing. Match <script> tags case-insensitively (CodeQL).
