LIbrary allows create two groups with same name

Hi,

If i run twice

$this->authorize->createGroup('backend.dashboard');

The system create two groups in DB with same name and different id, the system should prevent this and do nothig or generate a error.

In permissions same problem, if i assign same permission, create a duplicate row in database.
