[ESI][Cosim] Track actual instance names in metadata

Cosim advertises endpoints via SV instance hierarchy. So the metadata needs to be informed of the actual instance names rather than the requested ones. Particularly problematic when a name isn't given in the port request since no instance name is requested on the ep instances.

Track the instance names using the verbose string formatting feature in export verilog. (Just like the tcl output.)
