Value generator creating duplicate keys

I am getting this type of error :

That is a strange error, because the code protects against duplicate inserts to sequence documents. Is this reproducible? This is happening during insert of "country:1.0.0". There appears to be a sequence defined with sequence name "key_id". What is the metadata definition of this sequence?

Yes, it is reproducing but there no specific sequence of steps that can reproduce it. It just happens at some random time.

I don't see anything wrong with the sequence definition. What's more interesting is that sequence generation code does not throw exceptions for insert. So that makes me think this is not sequence generation related. Could you paste the full metadata definition for the country object? Is it by any chance writing to the sequences collection? If that is the case, then this might happen. The "sequences" collection is an internal dedicated collection for sequences, you are not supposed to store data there.
