perf(windows): skip the ACL mutation when the DACL is proven compliant - #2963
Conversation
Propagating owner grants and inheritance removal repeated on already-compliant secret trees, making startup cost grow with descendant count. Add an opt-in, single-path read proof that accepts only the cached effective token name with the exact explicit Full Control ACE and falls back on every ambiguity. Cache the token SID and resolved name together so mutation still grants by SID while readback can match icacls account names. Read-only success bypasses mutation without entering the post-mutation memo; the default flag-off sequence is unchanged.

📝 WalkthroughWalkthroughThe change adds cached Windows account identity data and an opt-in ACL verification path. When ChangesWindows ACL hardening
Estimated code review effort: 3 (Moderate) | ~20 minutes Merge Risk: 🟡 Moderate · up to The Windows fast path may skip permission hardening based on a cached account name instead of the stable security identifier, and it can perform an extra ACL check before timeout protections apply. This creates bounded security and availability risk, so the PR needs owner acceptance or fixes before merge. Sequence Diagram(s)sequenceDiagram

Prove inheritance protection before accepting the DACL
When inheritance is enabled but the parent currently contributes no inheritable ACEs, icacls can print exactly the explicit owner ACE accepted here, so the function returns ok:true without executing /inheritance:r; a later inheritable grant on the parent can then propagate to the secret. Microsoft documents (I) only as indicating that an ACE was inherited, while /inheritancelevel:r is the operation that removes inherited ACEs, so absence of (I) does not prove that the DACL is protected. Use an API/probe that verifies the protected-DACL control bit before allowing the skip. Microsoft icacls documentation.

Keep a valid SID sufficient for ACL mutation
On Windows tokens whose SID cannot be translated to an account name—for example, a still-running token for a deleted or temporarily unresolvable domain identity—the SID remains valid for icacls, but the new mandatory second line converts the lookup into EACLIDENTITY. This affects the default flag-off path too, causing required secret writes or startup to fail where the previous SID-only resolver succeeded; cache the SID independently and make the name optional so only the optimization is disabled when name translation fails.

Revalidate the object after the ACL query
If the pathname is atomically replaced after icacls reads the compliant old object's DACL but before this awaited call resolves, the verifier returns true for the replacement without checking its ACL. The existing mutation path explicitly observes the object before the operation and validates it afterward to reject this substitution; apply the same object-identity check around the read-only proof before returning success.

Check the timeout memo before starting ACL verification
With OPENCODEX_ACL_VERIFY_EXISTING=1, a path that previously timed out has a cached identity, so every later harden call launches another full-budget icacls query here before consulting timeoutMemoErrorIfBlocked. If that query stalls, the timeout memo no longer prevents repeated startup/load delays, and verification can also consume a full timeout in addition to the subsequently created hardening budget; consult the memo first and include verification in the single hardening deadline.

🩺 Stability & Availability | 🟠 Major | ⚡ Quick win
Apply the timeout memo and one deadline before ACL verification.
The verification probe runs before timeoutMemoErrorIfBlocked and before deadline exists. If a prior hardening attempt timed out, the next call can block on a new read-only icacls process for up to the full ACL timeout instead of returning the memoized result. For a non-compliant ACL, verification can also consume one full timeout before mutation receives a second full timeout.
src/lib/windows-secret-acl.ts#L791-L791: create the deadline and apply the timeout memo before the sync probe. Pass only the remaining shared budget to the probe.src/lib/windows-secret-acl.ts#L844-L844: apply the same ordering and shared-budget behavior to the async probe.
Add sync and async regression tests for a memoized timeout with OPENCODEX_ACL_VERIFY_EXISTING=1. Assert that no verification command starts after the memo blocks the path.

Summary
On Windows, securing a secret path always ran the full mutation sequence —
/grant:r,/inheritance:r, and broad-SID removal — even when the DACL already matched the policy exactly. That is measurable multi-second startup cost spent to reach a state the path was already in.
This adds an opt-in read-only proof gate: when the DACL can be proven compliant, skip the
mutation. Anything less than a proof falls through to the existing code path unchanged.
Closes #1298.
Why opt-in and strict
This is a permission path, so the failure mode of a wrong "already compliant" answer is a secret
left readable. The gate is therefore built to be hard to fool rather than easy to satisfy:
OPENCODEX_ACL_VERIFY_EXISTINGunset is byte-for-byte what it istoday, which the tests assert directly.
unexpected principal. A parse failure, a localized
icaclsoutput it cannot read, an inheritedACE, a broad principal, or an identity cache miss all fall through and mutate.
is not a record that this process set it.
The cached effective SID/name lookup this needs already exists in
windows-user-principal.ts, sothe gate spends no extra identity budget.
Verification
Based on
dev@47b8d1643.bun test tests/windows-secret-acl.test.ts tests/windows-user-principal.test.ts→ 185 pass, 0 failbun x tsc --noEmit→ cleanreturns them to green, so the gate is not vacuous.
New cases cover same-line ACEs, localized output, an inherited ACE, an unexpected broad principal,
an identity cache miss, both the sync and async paths, and the assertion that the read-only success
path leaves the memo untouched.
Review boundary
This changes when a Windows ACL mutation is skipped, so it wants a non-author security review
rather than a routine pass. The Windows-specific assertions run on the dispatched Windows leg.
